Clinical Educationthe course will be required to take the course within the first 30 days <strong>of</strong> entering theprogram. If a student has not taken a First Aid course, one is <strong>of</strong>fered at the beginning <strong>of</strong>the term which is certified by the American HEART Association. Notify the ClinicalEducation Office if this applies.7. <strong>St</strong>udents will be required to complete training in Blood Borne Pathogens, HIV/AIDS andUniversal Precautions within the first 30 days <strong>of</strong> registration. This course will beprovided as part <strong>of</strong> your Practicum I course <strong>for</strong> PT students and Foundations <strong>of</strong>Occupational Therapy – Fieldwork Introduction <strong>for</strong> OT students.Currently Enrolled <strong>St</strong>udents:1. All students MUST maintain medical insurance coverage and provide a copy <strong>of</strong> theircurrent card to the Clinical Education Office and the clinical education site duringclinical affiliation experiences.2. <strong>St</strong>udents are required to submit a two-part <strong>St</strong>udent Medical Record <strong>for</strong>m (comprehensivehealth examination), completed and signed by a physician (MD/DO) ONLY as follows:a. OT students must complete an updated physical examination, including an updatedTB test, no earlier than 4 months prior to participating in Fieldwork II, and allimmunizations must be kept current.b. PT students must complete an updated physical examination, including an updatedTB test, within 30 days <strong>of</strong> the start date <strong>of</strong> Internship 1, and all immunizations mustbe kept current.3. <strong>St</strong>udents must always maintain up-to-date CPR and HIV/Aids/Blood Borne Pathogenstatus.4. <strong>St</strong>udents must have either completed the Hepatitis B vaccine or have the titer verification<strong>of</strong> immunization and provided pro<strong>of</strong> <strong>of</strong> such to the Clinical Education Office or havesigned a waiver. Any waiver is kept in the student’s file. Sites may refuse students thathave not completed the series.5. A facility has the right to refuse to accept a student who has waived or not completed theHepatitis series. A facility may also request additional physical examination criteria inorder <strong>for</strong> a student to participate in a clinical affiliation at their facility. This in<strong>for</strong>mationmay be accessed through the Fieldwork Data Form/Clinical Site In<strong>for</strong>mation Form. It isthe student’s responsibility to comply with the site’s health policy.Post-Graduate <strong>St</strong>udents:Due to confidentiality laws, these health records will not be released to any third party. In addition, theClinical Education Office will not process requests <strong>for</strong> copies <strong>of</strong> medical records post-graduation. It isthe student’s responsibility to maintain original medical records.Failure to submit the required medical records in the time frame requested will be considered an act <strong>of</strong>pr<strong>of</strong>essional misconduct and will result in subsequent referral to the <strong>University</strong> Pr<strong>of</strong>essionalMisconduct Committee.Background Checks / Drug TestingUpon arrival at the <strong>University</strong>, all first-term students are required to complete a Criminal BackgroundCheck and Drug Screen as directed by the Clinical Education Department. All matriculating students whoparticipate in pediatric fieldwork, internship, or rotation clinical education experiences must participate ina fingerprinting check as part <strong>of</strong> the background check process. During the course <strong>of</strong> his or her curriculumat the <strong>University</strong>, the student will be given the opportunity to participate in clinical education rotations atexternal health care facilities. The student may be required to undergo an additional criminal backgroundcheck and/or a drug screen in advance <strong>of</strong> placement at certain facilities. In the event that this check orscreen results in an unsatisfactory report, the health care facility reserves the right to refuse placement <strong>of</strong>the student. The <strong>University</strong> will cooperate with the health care facility at all times in this backgroundcheck and/or drug screening in an ef<strong>for</strong>t to protect its patients, employees, or visitors.
