Clinical EducationContracts – Memorandum <strong>of</strong> UnderstandingA copy <strong>of</strong> the duly executed contract – memorandum <strong>of</strong> understanding must be on file in the ClinicalEducation Office prior to student involvement in direct patient/client contact.<strong>Health</strong> PolicyThe <strong>University</strong>, in concurrence with participating agencies, requires students to provide evidence <strong>of</strong> acomplete health examination and other tests prior to participation in <strong>University</strong>-sponsored activitiesincluding Clinical Education. Each <strong>of</strong> these requirements is completed at the student’s expense. Pro<strong>of</strong> <strong>of</strong>completion is submitted to the Clinical Education Office on registration day and must be kept up-todatethereafter. Medical documentation is confidential and is filed in the Clinical Education Office.Failure to comply with any health policies will preclude participation <strong>of</strong> the student in clinical affiliationexperiences.<strong>St</strong>udents provide health documentation directly to the requesting agency. Due to confidentiality laws,these records will not be released to any third party. It is the student’s responsibility to keep the original<strong>of</strong> all medical records and to have several copies on-hand.Newly Matriculating <strong>St</strong>udents:1. New students are required to submit their medical in<strong>for</strong>mation to the Clinical EducationOffice on the day <strong>of</strong> registration.2. All students MUST maintain medical insurance coverage and provide a copy <strong>of</strong> theircurrent card at registration.3. All students are required to submit a two-part <strong>St</strong>udent Medical Record <strong>for</strong>m(comprehensive health examination).The health examination must be completed, signed and dated by a physician (MD/DO)within 30 days <strong>of</strong> the day <strong>of</strong> registration <strong>for</strong> Trimester I. Nurse Practitioner orPhysician’s Assistant signatures will not be accepted.4. <strong>St</strong>udents must submit pro<strong>of</strong> <strong>of</strong> immunization or immunity to the physician’s <strong>of</strong>fice <strong>for</strong>:a. Measles, Mumps and Rubella (MMR) documentation <strong>of</strong> 2 immunizations afterone (1) year <strong>of</strong> age, immunity established by the three laboratory blood tests orborn be<strong>for</strong>e 1957.b. Varicella Titer (Chicken Pox) <strong>St</strong>udents must have a blood test verifying a positivetiter with the IGg number or two-shot series (history <strong>of</strong> disease is not an acceptableindicator).c. <strong>St</strong>udents must have either completed the Hepatitis B vaccine or have the titerverification <strong>of</strong> immunization and provided pro<strong>of</strong> <strong>of</strong> such to the Clinical EducationOffice or have signed a waiver. Any waiver is kept in the student’s file. Sites mayrefuse students that have not completed the series.d. Tetanus Toxoid/Diphtheria injection within the last seven years.e. PPD (Mantoux) Skin Test within 30 days <strong>of</strong> registration or chest X-Ray if positive.(Please note this test must be read within 72 hours.)5. <strong>St</strong>udents are required to enter the program with a current TWO YEAR BLS (Basic LifeSupport) <strong>for</strong> <strong>Health</strong>Care Providers American Heart Association card in CPR. Thiscertification must be issued not more than 30 days prior to the day <strong>of</strong> registration atthe <strong>University</strong>. Cards issued prior to this date will NOT be valid. <strong>St</strong>udents with invalidcards will be required to retake the course within the first 30 days <strong>of</strong> entering theprogram. Please be aware that some programs <strong>of</strong>fering CPR cards are only good <strong>for</strong> one(1) year. HOWEVER, STUDENTS MUST RECEIVE A TWO-YEAR CARD. Allstudents must also provide a copy <strong>of</strong> their current card at registration. If a student has nottaken a CPR course, one is <strong>of</strong>fered at the beginning <strong>of</strong> the term. Notify the ClinicalEducation Office, if this applies.6. <strong>St</strong>udents are required to enter the program with a current TWO YEAR HEART SAVERFIRST AID card. This certification must be issued not more than 30 days prior to the day<strong>of</strong> registration at the <strong>University</strong>. <strong>St</strong>udents with either invalid cards or who have not taken<strong>University</strong> <strong>of</strong> <strong>St</strong>. <strong>Augustine</strong> <strong>for</strong> <strong>Health</strong> <strong>Sciences</strong> 100
