Clinical Educationtravel outside <strong>of</strong> their local area <strong>for</strong> full-time clinical rotations. Hours <strong>of</strong> attendance are determined by thesite.Clinical Affiliation CorrespondenceAll USA students will be provided with templates <strong>of</strong> correspondence to be sent to their assigned sites. ForMOPA students, instructions will be given in the classroom. For OT and PT students, the followingapplies:OT <strong>St</strong>udents:Approximately eight weeks be<strong>for</strong>e the Fieldwork experience, the student will write an introductoryletter to the Fieldwork Supervisor (FWS). Letters must follow the specified <strong>for</strong>mat. Thiscorrespondence will include an AOTA <strong>St</strong>udent Personal Data Sheet <strong>for</strong> <strong>St</strong>udent FieldworkExperience. The student will also request in<strong>for</strong>mation on the dress code, contact person, arrival time,parking, directions, etc.A copy <strong>of</strong> the letter and student personal data sheet must be submitted to the Clinical Education Office<strong>for</strong> placement in the student’s file.PT <strong>St</strong>udents:Approximately eight weeks be<strong>for</strong>e placement <strong>of</strong> the internship affiliation, the student will write anintroductory letter to the Clinical Instructor (CI) or Center Coordinator <strong>of</strong> Clinical Education (CCCE).This letter is <strong>for</strong>warded to the clinical site through the Clinical Education Office. This correspondencewill include a current student photograph and an APTA <strong>St</strong>udent Personal Data Sheet <strong>for</strong> <strong>St</strong>udentClinical Experience. The student will also request in<strong>for</strong>mation on the dress code, contact person,arrival time, parking, directions to the facility, etc.A copy <strong>of</strong> the letter must be submitted <strong>for</strong> approval and mailing. Letters must follow the specified<strong>for</strong>mat. A copy <strong>of</strong> the letters will be placed in the student’s file.A student that fails to complete the required paperwork in a timely manner may risk cancellation <strong>of</strong> theirplacement.Failure to submit the required letters <strong>of</strong> correspondences in the timeframe requested will be consideredan act <strong>of</strong> pr<strong>of</strong>essional misconduct and will result in subsequent referral to the <strong>University</strong> Pr<strong>of</strong>essionalMisconduct Committee.The Clinical Education Office will notify the students when they may initiate telephone communicationwith the assigned sites, following the mailing <strong>of</strong> the letters.Clinical Shadowing Experiences<strong>St</strong>udents will participate in clinical affiliation experiences shadowing pr<strong>of</strong>essionals as follows:OT <strong>St</strong>udents:a. First Trimester: Foundations <strong>of</strong> Occupational Therapy – Fieldwork Introductionb. Second Trimester: Fieldwork IAc. Third Trimester: Fieldwork IBMOPA <strong>St</strong>udents:a. First Trimester: Pr<strong>of</strong>essional Issues – Externships I and II. Placements are made by theClinical Education Office. Ef<strong>for</strong>t is made to place students locally (within a two-hour drivefrom their residence).
