Satisfaction <strong>of</strong> the hold isrequired be<strong>for</strong>e a release can be given.Address ChangesA student’s legal home permanent (mailing address) address is taken from the application <strong>for</strong> admissionand subsequently from the student in<strong>for</strong>mation <strong>for</strong>m completed during registration. It is the student’sresponsibility to notify the <strong>St</strong>udent Services <strong>of</strong>fice <strong>of</strong> any changes to their address. Address changesshould be made through the myUSA portal <strong>St</strong>udents tab or in person at the <strong>St</strong>udent Services <strong>of</strong>fice (on theFlorida, Cali<strong>for</strong>nia or Texas campuses).Name ChangesRequests <strong>for</strong> a change in the name as it appears on the respective student’s academic record must be madein writing and accompanied by a copy <strong>of</strong> one <strong>of</strong> the following: social security card, drivers’ license,marriage certificate, divorce decree, adoption papers, or other suitable legal document. Name change<strong>for</strong>m may be found on the myUSA portal <strong>St</strong>udents tab.Change in <strong>St</strong>udent <strong>St</strong>atus<strong>St</strong>udents who find it necessary to withdraw from the program or take a leave <strong>of</strong> absence must notify theirprogram director in writing and complete the required documentation <strong>for</strong> the <strong>St</strong>udent Services <strong>of</strong>fice.Withdrawal or leave <strong>of</strong> absence <strong>for</strong>ms are available on the myUSA portal <strong>St</strong>udents tab.Release <strong>of</strong> <strong>St</strong>udent In<strong>for</strong>mationThe <strong>University</strong> <strong>of</strong> <strong>St</strong>. <strong>Augustine</strong> <strong>for</strong> <strong>Health</strong> <strong>Sciences</strong> complies with the provisions <strong>of</strong> Public Law 93-380,the Family Educational Rights and Privacy Act (FERPA) <strong>of</strong> 1974, in reference to student records.<strong>St</strong>udents must consent to the release <strong>of</strong> any student in<strong>for</strong>mation other than directory in<strong>for</strong>mation to anyperson or agency. This consent must be in writing, signed, and dated. The consent must specify thein<strong>for</strong>mation to be released, the reason <strong>for</strong> release, and the names <strong>of</strong> the individual or agency to whom thein<strong>for</strong>mation is to be released. For more detailed in<strong>for</strong>mation see pages 23-24 “Confidentiality and Release<strong>of</strong> <strong>St</strong>udent Records”.Enrollment CertificationsTo confirm enrollment in the <strong>University</strong>, students should <strong>for</strong>ward documentation related to thecertification and/or make a written request to the <strong>St</strong>udent Services Office. For loan defermentcertifications contact llasko@usa.edu.TranscriptsTo ensure confidentiality <strong>of</strong> student records, the <strong>University</strong> issues <strong>of</strong>ficial transcripts <strong>of</strong> academicin<strong>for</strong>mation only by written or electronic signature by the student or graduate using Transcripts onDemand. <strong>St</strong>udents will pay a $3.00 convenience fee <strong>for</strong> each request submitted and an additional $2.00fee <strong>for</strong> each paper transcript requested. For electronic transcript delivery only the $3.00 convenience feeis charged. Transcripts as well as grade reports will not be issued <strong>for</strong> any student with an outstandingobligation to the <strong>University</strong>.
