4SECTION 1 / PART DSpecial Needs InformationIn doing placement of students who are accepted, we need to take into consideration factors that may limit a student’s activities, require special assistance or preparation at the institute site, or affect site or assignments.1. Is English a second language for you?* Yes NoSECTION 1 / PART EReleases and Signatures• The information that has been given above by the student is true and accurate.• We have read the RECAP website infomation and understand that the program is academically rigorous and meant for students who are academically talented, actively pursuing an interest in science, self-motivated, and able to handle the responsibility of being away from home for three weeks.• We understand that in addition to the three-week summer program, all RECAP students are required to complete a background literature review paper and researchproposal during March-June and completed research paper by October 1.• We understand that if the student is accepted and wishes to attend, he or she will be required to be in attendance the entire three-week period.• We understand that although tuition, room, board, and fees are provided free of charge the student’s family or guardian must assume responsibility for transportationto and from the site, pocket money for the student’s incidental personal expenses, and payment for any medical expenses incurred by the student during the institute.• If the student is accepted to RECAP, we agree to adhere to its rules and regulations concerning students’ responsibilities in the academic and the residential lifeprograms. We understand that the directors have the right to dismiss any student whose behavior is not consistent with the goals and standards of RECAP.• We agree to let information in this application and other records resulting from the student applying to and attending RECAP be used in approved research and bemade available on a confidential basis to the student’s home school and other educational institutions. If the student is accepted, we agree that this application may begiven to appropriate RECAP personnel as a source of information about the student.• We agree that the student will participate in the completion of questionnaires and other appropriate and approved research projects done as part of the program’sevaluation. We also agree that photographs of the student taken during the program, papers written by the student during the program, and similar items may be usedby RECAP in reports and public information materials. 5SECTION 2: To Be Completed By Science Teacher Who Has Taught the StudentInstructions to Teacher:Thank you for agreeing to fill out this evaluation. Please keep the application intact and pass it on to your school’s guidancecounselor after you have completed Section 2. The RECAP office cannot accept applications that are submitted inseparate pieces.RECAP is a highly competitive program, and we need the help of the state’s science teachers to assure that students whocan best benefit from the program are the ones who are invited and the particularly, that the student will be successfulworking independently in a reserach lab. Before you fill out this evaluation, please read the information on the RECAPwebsite: http://courses.ncssm.edu/bennett/RECAPRECAP will not make your evaluation available to the student, parents, or guardian for review. If you do not feelcomfortable about making this evaluation on your own, feel free to consult with other teachers and the school’s guidancestaff. These points are important to keep in mind in doing this evaluation:1. RECAP is designed specifically for academically talented students who are actively pursuing an interest inscience and mathematics. Participants do not need to be formally identified AG students, but they should be ofhigh intellectual ability.2. This program has been designed for students who are already strongly self-motivated and academicallydisciplined. Some students may not be fulfilling their high academic potential because they may not be truly interestedin academic success or may lack self-discipline. It is unlikely that such students will be able to cope with theacademic demands of RECAP. However, consideration will be given to students who are academically talented butwhose grades or standardized test scores do not reflect this accurately for reasons unrelated to motivation. Someexamples are students with hearing impairments, cerebral palsy, or learning disabilities; students for whom Englishis a second language; students whose education has been disrupted by moving or family problems; and students whohave not had good educational opportunities.3. RECAP students will live for three weeks on the NCSSM campus. Although they will have residential advisors, for the sake of the students themselves it is essential that only those who are able to handle the responsibility ofbeing away from their parents’ or guardians' supervision for three weeks be recommended for the program.4. This program concentrates on chemistry/physics/astronomy. It is designed so that studentsare deeply involved in one of these topics for many hours each day. Students who do well in science but whosemain interests are in other fields may be quite unhappy in this program.Taking all factors into account, please give us your best evaluationof this student. Please supplement the aboveevaluation with a separatewritten statement.Use the scale provided to rate the student in the following areas. In rating the student please compare the applicant withother students you have taught who had strong interest and talent in science and mathematics. If you have not had sufficientexperience with the student in a particular area, circle 0.To what degree is the student: low high unfamiliarStrongly interested in science? 1 2 3 4 5 0Committed to spending three weeks inintensive study of physics, chemistry or astronomy? 1 2 3 4 5 0Likely to be successful working independently in aresearch lab? 1 2 3 4 5 0Academically suited for a three-week intensiveprogram in science? 1 2 3 4 5 0Ready to handle the responsibility of attendinga three-week residential program away from home? 1 2 3 4 5 0Comment below on any responses of 1 or 5 that you have circled above.
