The details of these agencies shall be provided at thetime of signing the MoU with the State Government.(Annexure-3)Note: The qualifying requirements data shall be enclosed with the TechnicalBid only. The bidders who do not fulfill this criterion, will bedisqualified immediately and their bids will not be considered.B. Others:I. Previous Experience with RSBY:If the insurer has any previous experience with running RSBY in anyState(s) they should provide details of that in the prescribed format asgiven in Annexure 4 of the bid document.In the same annexure the Insurer should also provide a brief write-upof their experience with RSBY. The write-up should cover at least thefollowing aspects of RSBY:• Coordination with the State Government• Enrolment of Beneficiary• Empanelment of Health Care Providers• Service Delivery to the beneficiary• Settlement of claims• Experience with TPA/ Smart card vendor(Annexure-4)II.List of Additional Packages for common medical and surgicalinterventions/ procedures:Provisional list of packages with rates is attached in Annexure-6 of thebid document. The package rates for these have been fixed by the StateGovernment /Nodal Agency. Insurer may provide list of additionalpackages for medical and surgical interventions/ procedures withpackage rates as Annexure 5 of the bid document.(Annexure-5)32
