4. January, 2012 January, 2011 October, 201211.5. The insurance company will have only Four Months to complete theenrolment process in both new and renewal districts. For the new setof districts full premium for all the four months will be given tothe insurer.The salient points regarding commencement & end of the policy are• Policy end date shall be the same for ALL cards in a district• Policy end date shall be calculated as completion of one year from thedate of Policy start for the 1 st card in a district• Minimum 9 months of service needs to be provided to a family in caseof new districts, hence enrollments in a district shall cease 4 monthsfrom beginning of card issuance.• Full 12 months of service needs to be provided to a family in case ofrenewal districts.Note: For the enrollment purpose the month in which first set of cardsis issued would be treated as full month irrespective of the dateon which cards are issued12. Enrolment Procedure:The enrolment of the beneficiaries will be undertaken by the InsuranceCompany selected by the State Government/ Nodal Agency and approved bythe Central Government. The Insurer shall enroll the BPL beneficiaries andother non-BPL beneficiaries (if applicable) based on the validated datadownloaded from the RSBY website and issue Smart card as per RSBYGuidelines..Further, the enrolment process shall continue as per schedule agreed by theState Government/ Nodal Agency. Insurer in consultation with the StateGovernment/ Nodal Agency shall chalk out the enrolment/ renewal cycle upto village level by identifying enrolment stations in a manner thatrepresentative of Insurer, Government / Nodal Agency and smart cardvender can complete the task in scheduled time. The process of enrolment/renewal shall be as under:(a)(b)(c)The Insurer will download the BPL data and other non-BPLbeneficiaries data (if applicable) for the selected districts from theRSBY website.The Insurer will arrange for the smart card. as per the Guidelinesprovided in Annexure-16. The software for issuing smart cards andusage of smart card services shall be the one certified by the MoLE..If the smart card is lost/ damaged within the policy period thenbeneficiary can get a new card issued at District Kiosk by paying a15

(d)(e)(f)pre-defined fee.An enrollment schedule shall be worked out by the Insurer, inconsultation with the State Government/ Nodal Agency, for eachvillage in the project districts.It will be responsibility of State Government / Nodal Agency toensure availability of sufficient number of Field level Governmentofficers who will be called Field Key Officers (FKO) to accompany theenrollment teams as per agreed schedule for verification of BPLfamilies and other non-BPL beneficiaries (if applicable) at the time ofenrolment.Advance publicity of the visit for the enrollment of beneficiaries shallbe done by the Insurer in consultation with the State Government/Nodal Agency in respective villages.(g) List of BPL Beneficiaries and other non-BPL beneficiaries (ifapplicable) should be posted prominently in the village by theInsurer.(h) Insurer will place a banner in the local language at the enrolmentstation providing information about the enrolment and details of thescheme etc.(i)(j)(k)(l)The enrolment team shall visit each enrolment station on the prescheduleddates for enrolment/ renewal and/ or issuance of smartcard.At the time of enrolment/ renewal, the government official (FKO)shall identify the head of the family in the presence of the insurancerepresentative and authenticate them through his/ her own smartcard and fingerprint.The enrolment team shall handover the activated smart card to thebeneficiary at the time of enrolment itself. At the time of handingover the smart card, the INSURER shall collect the registration fee ofRs.30/- from the beneficiary. This amount shall constitute the firstinstallment of the premium and will be adjusted against the secondinstallment of the premium to be paid to the INSURER by the NodalAgency.The Insurer’s representative shall also provide a pamphlet along withSmart Card to the beneficiary indicating at least the following:i. List of the empanelled network hospitals alongwith addressand contact detailsii. Location and address of district kiosk and its functionsiii. The availability of benefitsiv. The names and details of the key contact person/ persons inthe districtv. Toll-free number of call centre.vi. Process of taking the benefits under RSBYvii.
