iii) Those facilities undertaking surgical operations should have a fullyequipped Operating Theatre <strong>of</strong> their own.iv) Fully qualified doctors and nursing staff under its employment roundthe clock.v) Maintaining <strong>of</strong> necessary records as required and providing necessaryrecords <strong>of</strong> the insured patient to the Insurer or his representative/Government/Nodal Agency as and when required.vi) Registration with Income Tax <strong>Department</strong>.vii) Telephone/Fax, Internet/ Any other connectivity to the InsuranceCompany Server. Each hospital/health service provider shall posses aPersonal Computer, 2 smart card readers and a fingerprint verificationmachine or a standalone machine with minimum configurationspecified as per Annexure 16.c. Package Rates – Both Public and Private empanelled hospitals shouldagree to the cost <strong>of</strong> packages for each identified medical/ surgicalintervention/ procedures as approved under the scheme. These packagerates will include:I. Bed charges (General Ward),II. Nursing and Boarding charges,III. Surgeons, Anesthetists, Medical Practitioner, Consultants fees etc,IV. Anesthesia, Blood, Oxygen, O.T. Charges, Cost <strong>of</strong> SurgicalAppliances etc,V. Medicines and Drugs,VI. Cost <strong>of</strong> Prosthetic Devices, implants,VII. X-Ray and other Diagnostic Tests etc,VIII. Food to patientIX. Expenses incurred for consultation, diagnostic test and medicinesup to 1 day before the admission <strong>of</strong> the patient and cost <strong>of</strong>diagnostic test and medicine up to 5 days <strong>of</strong> the discharge from thehospital for the same ailment / surgeryX. Transportation Charge <strong>of</strong> Rs. 100/- is payable to the beneficiary atthe time <strong>of</strong> dischargeTherefore, the package should cover the entire cost <strong>of</strong> treatment <strong>of</strong> thepatient from date <strong>of</strong> reporting to his discharge from hospital and 5 daysafter discharge and any complication while in hospital, making thetransaction truly cashless to the patient. The Package rate also coversRs. 100 which shall be paid to the beneficiary at the time <strong>of</strong> discharge.d. Additional Benefits to be Provided by <strong>Health</strong> Care ProvidersIn addition to the benefits mentioned above, both Public and PrivateProviders should be in a position to provide following additional benefits to10
