Students wishing to officially change their name must complete and sign a Name Change Form; present official documentation of thenew name; and have the Change of Name Form notarized (Notary Publics are available in each program office).Types of Official Name Changes REQUIRING both Official Documentation and Notarization• Marriage• Divorce• Extending a middle name from an initial• Reducing a first name to a initial• Eliminating a first name• Adding a:o Jr. or number suffix (e.g., II or III)o middle name or initialo last nameTypes of Documentation REQUIRED to Process Official Name Changes• Driver’s license• Passport• Birth Certificate• Marriage license or divorce decree• Verification of student’s name as it appeared on official transcripts received from other institutionsMiddle Name Reductions (does not require notarization)Students wishing to reduce their middle name from:• full middle name TO middle initial;• full middle name TO no middle name; or• middle initial TO no middle initial.Options for Name Changes/Reductions without Official DocumentationOption 1: Use of the “Familiar Name” field in Community Directoryo Students who want to alter or reduce their first name, but do not have official documentation, may do so via the“familiar name” field via the Community Directory.o Middle and last names still may not be altered without proper documentation.Option 2: Create an Email Alias (sometimes called a nickname)o Students interested in creating an email alias can use this feature as a pointer to their default <strong>Booth</strong> email address(userid@<strong>Chicago</strong><strong>Booth</strong>.edu).o Emails sent to student aliases will arrive in the same inbox as emails sent to their userid@<strong>Chicago</strong><strong>Booth</strong>.edu addressin order to the student to receive emails at alias@<strong>Chicago</strong><strong>Booth</strong>.edu in addition to their defaultuserid@<strong>Chicago</strong><strong>Booth</strong>.edu address.How to Claim an Email Alias• Alias claiming is only available for the Exchange Email system (Outlook).• Access the <strong>Portal</strong> Student Home Page and go to “Manage Your Accounts” section on the right hand column.• Aliases take approximately 10 minutes to propagate through the <strong>Booth</strong> systems.• Aliases will propagate to the Exchange Global Address Book overnight.• Students with questions or needing assistance should contact the Help Desk at 773-702-7414 orHelpDesk@<strong>Chicago</strong><strong>Booth</strong>.edu.11/08:ASCG
