UNIVERSITY OF KENTUCKY HOSPITALPHARMACY SERVICESPOSITION DESCRIPTIONPOSITION TITLE:PH-14-21Revised 1/12CLINICAL PHARMACIST SPECIALIST – INFECTIOUS DISEASESPOSITION PURPOSE AND SUMMARY:1.0 DESCRIPTIONThe Antimicrobial Stewardship Program (ASP) clinical pharmacist specialistprovides collaborative pharmaceutical care to hospital patients through the ASPwith an <strong>Infectious</strong> Disease physician, faculty and house staff physicians,pharmacists and clinical microbiology staff.2.0 QUALIFICATIONS2.01.01 Must possess a Pharm.D. Degree.2.01.02 Completed a Pharmacy Practice Residency (or equivalent) withexperience in <strong>Infectious</strong> Disease. A specialized clinical residencyin <strong>Infectious</strong> Disease is preferred.2.01.03 Licensed as a pharmacist in the Commonwealth <strong>of</strong> Kentucky.2.01.04 Maintains competency in unique aspects <strong>of</strong> infectious disease drugtherapy.3.0 RESPONSIBILITIESThe major job responsibilities (MJRs) <strong>of</strong> this position are: pharmaceutical care tohospital patients receiving antimicrobial therapy, clinical consultations, costeffectivecare analysis, teaching, and pharmacy representation in UK HealthCarecommittees.The following activities are components <strong>of</strong> the position to assist in meeting the majorjob responsibilities:3.01 Provide cost-effective pharmaceutical care to patients receiving selectantimicrobial therapy.3.01.01 Participate in selection <strong>of</strong> patients receiving targetedantimicrobials and monitor these patients on a daily basis.3.01.02 Discuss medication order changes/clarifications with theinfectious disease physician or the prescriber, documentany changes in patient and pharmacy records, and informothers <strong>of</strong> medication order changes.


3.01.03 Monitor drug therapy to evaluate appropriateness <strong>of</strong> use,dose, dosage form, regimen, route, therapeutic duplication,and drug interactions. Switch patients to IV to PO therapyas quickly as feasible.3.01.04 Provide pharmacokinetic consultations as required.3.01.05 Detect, monitor, document, and report adverse drugreactions and medications errors.3.01.06 Promote the use <strong>of</strong> the formulary by converting nonformularyorders to formulary when possible, butcoordinates procurement <strong>of</strong> non-formulary drugs whennecessary.3.01.07 Maintain and update a patient pr<strong>of</strong>ile with demographics,diagnosis, allergies, and current medications.3.02 Facilitate discharge for home care patients.3.02.01 Anticipate discharge date and medications.3.02.02 Facilitate discharge planning and smooth transition toalternate site <strong>of</strong> care or the outpatient pharmacy.3.02.03 Work collaboratively with the <strong>Infectious</strong> Disease or facultyservice to ensure appropriate practice in the ambulatorysetting.3.03 Provide limited operational support to the pharmacy.3.03.01 Review select patients antimicrobial therapy for accuracyand appropriateness3.03.02 Communicate with pharmacy to avoid waste in preparation<strong>of</strong> antimicrobials.3.04 Promote and demonstrate rational, cost-effective drug therapy.3.04.01 Identify and implement drug therapy where positivepharmacoeconomic impact could be achieved. Theultimate goal is to develop and assure cost effective use <strong>of</strong>antimicrobials, including antibiotics, antifungals, antiviralsand intravenous immune globulin.3.04.02 Develop and conduct target drug programs and drug usageevaluations as needed and report results to the Pharmacyand Therapeutics Committee.3.04.03 Serve as secretary <strong>of</strong> the Antimicrobial Subcommittee <strong>of</strong>the Pharmacy and Therapeutics Committee.3.04.04 Document clinical consultations and cost avoidance as part<strong>of</strong> the justification for the position/service.3.04.05 Monitor the use <strong>of</strong> expensive medications to assure there isuse consistent with approved criteria.3.04.06 Anticipate changes in drug therapy and report the potentialimpact it will represent to the overall hospital and drugbudget.


3.05 Facilitate investigational drug studies.3.05.01 Provide inservices for <strong>Infectious</strong> Disease investigationalstudies to the nurses and pharmacy staff.3.05.02 Anticipate the pharmacoeconomic impact <strong>of</strong> investigationaldrug studies.3.05.03 Provide financial forecasts for the Director <strong>of</strong> the <strong>Infectious</strong>Disease and the Department <strong>of</strong> Pharmacy Services.3.06 Take a role in the teaching programs <strong>of</strong> the Department <strong>of</strong> PharmacyServices and College <strong>of</strong> Pharmacy as they relate to infectious disease.3.06.01 Precept Pharm.D. clerkship students.3.06.02 Precept and mentor pharmacy practice and infectiousdisease specialty residents.3.06.03 Serve as Residency Program Director for the <strong>Infectious</strong>Disease Specialty Pharmacy Residency (PGY2)3.06.04 Provide staff development support to nurses andphysicians.3.06.05 Provide presentations, publications, and other informativeactivities on infectious disease.3.06.06 Seek and maintain appropriate academic appointment in theCollege <strong>of</strong> Pharmacy.3.07 Participate in appropriate departmental quality improvement initiatives4.0 RELATIONSHIPS4.01.01 The position will primarily funded by the hospital withsupport from the College <strong>of</strong> Pharmacy.4.01.02 Will report to the Associate Director <strong>of</strong> <strong>Clinical</strong> PharmacyLeadership in the Department <strong>of</strong> Pharmacy Services.Works collaboratively with <strong>Infectious</strong> Disease medical staffand other pharmacy staff.4.01.03 As a faculty member, will report to the Division Directorfor the Division <strong>of</strong> Pharmacy Practice & Science in theCollege <strong>of</strong> Pharmacy.5.0 PATIENT POPULATION SERVEDThe Pharmacy Department provides pharmaceutical care to neonatal, pediatric,adolescent, adult and geriatric populations. This position provides antimicrobialmanagement services for pediatric, adolescent, adult and geriatric patients.
