27• One of the key areas of planning should identify location of vulnerablepockets in the village- remote habitations, hamlets, colonies of SC/ST,women headed households <strong>and</strong> other areas where families are unableto access services.• Status of environment – safe drinking water, proper sewage disposal,improved sanitation, housing <strong>and</strong> settlements, attention to pollution- soil,water <strong>and</strong> air (pesticides, groundwater contamination), specific to localareas,• Mapping of Resources that offer high quality primary, secondary, <strong>and</strong>tertiary services.• Plans <strong>for</strong> community health education• Action <strong>for</strong> the following: Improved Nutrition, Universal coverage ofmaternal <strong>and</strong> child health services, identification <strong>and</strong> management ofblindness, TB, malaria, filaria, <strong>and</strong> other communicable diseases,improved environmental sanitation, universal coverage <strong>for</strong> water <strong>and</strong>sanitation facilities, <strong>and</strong> promotion of rational drug use, includingpreservation <strong>and</strong> use of proven indigenous health practices.• Specific action to reduce discrimination on grounds of gender, caste <strong>and</strong>religion (including neglect of girl children, dowry, domestic violence)• Enumerate births, deaths <strong>and</strong> marriages• Supporting village level surveillance systems; <strong>and</strong> maintaining a VillageHealth Register at the AWW level which lists key programmes <strong>and</strong>services provided.• Conducting audits of maternal <strong>and</strong> under five deaths.After the finalization of the Village Health Plan, the Gram Panchayat willpresent the plans at the Panchayat Samiti (Intermediary Panchayat) where aconsolidated block health plan is developed, with flexibility to addresscommon health problems across the block <strong>and</strong> individual needs of each GramPanchayat.7.
