6.1 Payment for the contracted service will be within 10 - 15 days upon receipt of invoiceand the using agency’s approval. Payment will be tied to specific milestones ordeliverables.Invoices are to be submitted upon completion of contract milestones (if known, list);using agency approval will be upon satisfactory completion as determined by theagency’s contract monitor.6.2 Vendors may propose an alternative payment option; alternative payment options must belisted on Attachment A of the RFP. Alternative payment options will be considered ifdeemed in the best interest of the State, project or service solicited herein. 1370 South Curry StreetCarson City, NV 89703Proposals shall be clearly labeled in a sealed envelope or box as follows:REQUEST FOR PROPOSAL NO.: 1004-REHABPROPOSAL OPENING DATE: July 23, 2012 @ 2:00 p.m.FOR: ORIENTATION VIDEO PRODUCTION7.4 Proposals must be received at the above-referenced address no later than July 23,2012 Pacific Time, 2:00 p.m. Proposals that do not arrive by proposal opening time anddate WILL NOT BE ACCEPTED. Vendors may submit their proposal any time prior tothe above stated deadline.7.5 The State will not be held responsible for proposal envelopes mishandled as a result ofthe envelope not being properly prepared. Facsimile, e-mail or telephone proposals willNOT be considered; however, at the State’s discretion, proposal may submitted all or inpart on electronic media, as requested within the RFP document. Proposals may bemodified by facsimile, e-mail or written notice provided such notice is received prior tothe opening of the proposals.7.6 Although it is a public opening, only the names of the vendors submitting proposals willbe announced NRS §333.335(6). Technical and cost details about proposals submittedwill not be disclosed. Assistance for handicapped, blind or hearing-impaired persons whowish to attend the RFP opening is available. If special arrangements are necessary, pleasenotify the Rehabilitation Division’s designee as soon as possible and at least two days inadvance of the opening.7.7 If discrepancies are found between two or more copies of the proposal, the master copywill provide the basis for resolving such discrepancies. If one copy of the proposal is notclearly marked “MASTER,” the State may reject the proposal. However, the State mayat its sole option, select one copy to be used as the master.7.8 For ease of evaluation, the proposal should be presented in a format that corresponds toand references sections outlined within this RFP and should be presented in the sameorder. Responses to each section and subsection should be labeled so as to indicatewhich item is being addressed. Exceptions to this will be considered during theevaluation process.7.9 If complete responses cannot be provided without referencing supporting documentation,such documentation must be provided with the proposal and specific references made tothe tab, page, section and/or paragraph where the supplemental information can be found.7.10 Proposals are to be prepared in such a way as to provide a straightforward, concisedelineation of capabilities to satisfy the requirements of this RFP. Expensive bindings,colored displays, promotional materials, etc., are not necessary or desired.
