Attachment CREFERENCE QUESTIONNAIREThe State <strong>of</strong> <strong>Nevada</strong>, as a part <strong>of</strong> the RFP process, requires proposing vendors to submit a minimum <strong>of</strong>three (3) business references as required within this document. The purpose <strong>of</strong> these references is todocument the experience relevant to the scope <strong>of</strong> work and provide assistance in the evaluation process.The proposing vendor is required to send the following reference <strong>for</strong>m to each business reference listed.The business reference, in turn, is <strong>request</strong>ed to submit the Reference Form directly to the State <strong>of</strong><strong>Nevada</strong>, <strong>Department</strong> <strong>of</strong> Employment, Training and Rehabilitation, Rehabilitation Division by the RFPsubmission deadline <strong>for</strong> inclusion in the evaluation process. The <strong>for</strong>m and in<strong>for</strong>mation provided willbecome a part <strong>of</strong> the submitted <strong>proposal</strong>.
