Achieving good end of life carefor people with dementiaFacilitatorsMin Stacpoole, Research Nurse, End of Life Care for People withDementia, St Christopher’s HospiceMin managed a long stay ward for older people before coming to work atSt Christopher’s Hospice. She worked in the in-patient unit and then asa Clinical Nurse Specialist in Lambeth, Southwark, Lewisham, Bromleyand Croydon. She managed a caseload of palliative care patients inCroydon Care Homes for 3 years. From 2009, she worked for theModernisation Initiative’s End of Life Care Programme as NurseFacilitator for end of life care for people with dementia.Jessica Barnes-Webb, Dementia and End of Life Clinical NurseSpecialist, Trinity HospiceJessica has a degree in Mental Health Nursing from Oxford BrookesUniversity and initially worked as a staff nurse on an acute mental healthward for older adults. She then moved to London in 2006 and worked for4 years as a CPN in a mental health care of older people team. She alsoworked as a research nurse on clinical trials for anti-dementiamedication and, enjoying working with people with dementia, decided tospecialise in this area. Jessica started work at Trinity Hospice as aDementia and end of life Clinical Nurse Specialist in March 2010. This isa new and developing post recognising the need for improved access topalliative care for people with dementia.Target audience: GP’s, District Nurses, Community Matrons, CPNs,Care Home nurses, Domiciliary Care Managers, Social Workers, AlliedHealth Professionals, and other care workers who would like theopportunity to reflect on their work with people with dementia.Estimates suggest that by the end of the decade there could be up to amillion people living with dementia in the UK. All of us, especially healthand social care professionals, need to understand the impact ofdementia on the lives of individuals and families.Research has found that end of life care for people with dementia isoften poor and that they are often hospitalised inappropriately becauseof lack of planning and co-ordination. The primary care team, carehome staff and social care professionals play a key role in recognisingdisease progression and supporting the family to plan ahead and ensurethat the end of life care wishes of the person with dementia arerespected and the person dies peacefully and comfortably.End of life care for people with dementia does not usually needspecialist palliative care intervention. However, planning end of life carefor someone who lacks capacity to make their own decisions is differentfrom advance care planning with someone who is cognitively able;assessing pain and other physical symptoms is complicated when theperson cannot communicate their distress verbally; and supportingfamilies through the long dwindling end stages of dementia can bechallenging. This course is intended to help health and social careprofessionals adapt their existing skills to achieve excellent end of lifecare for people with dementia and their families.Topics will include:Dementia: diagnosis, epidemiology, national policyExperience of the person with dementiaUnderstanding behavioural and psychological symptomsAssessing symptoms when someone cannot communicateverballyDisease progression and decision makingEnd of life careFor more information and booking form visitwww.stchristophers.org.uk/education
