VCFS/22Q11 PLAN OF CARE• The VCFS Plan of Care provides guidelines forscreening and management of medical, developmental,and psychosocial issues from infancy to adulthood.• This model was developed based on clinical routinesestablished for patients followed in the VelocardiofacialSyndrome Clinic at Children’s Hospital of Wisconsin.• It represents a concise, visual, user-friendly tool toprovide anticipatory guidance and to monitor the care ofindividuals with VCFS over time.

VCFS/22Q11 PLAN OF CAREHow to Use the Plan of Care• This plan of care consists of three forms: Screening Guidelines,the Medical Follow-Up Form, the Developmental Follow-UpForm and a cover sheet which provides general instructions onhow to use the forms• It is not intended to serve as a stand-alone document in theabsence of effective genetic counseling and primary medicalcare. Before being given the Plan of Care, families should havethe opportunity to discuss the implications of the diagnosis andthe natural history of the syndrome, as it relates to their ownchild’s individual presentation.

VCFS/22Q11 PLAN OF CARE• The Screening Guidelines form is meant tocapture the most common issues associatedwith the syndrome for which there may not bean obvious concern or symptom. It should beused at the time of diagnosis.• The results from the screenings willindividualize each child’s case managementplan.

VCFS/22Q11 PLAN OF CARE• The Medical and Developmental Follow-Up formsrecommend the type, timing and frequency ofadditional assessment and treatment depending onthe child’s specific needs.• Parents are encouraged to discuss all follow-upprocedures with their child’s primary care physician.• For some issues, periodic re-screening is suggested
