After Action ReviewLessons Learned / Action Items• G<strong>at</strong>e design modific<strong>at</strong>ion issues: Many design modific<strong>at</strong>ions were discussed –Engineering Construction (EC) will provide draft <strong>of</strong> changes and follow upmeeting• Pre‐inspection to identify interferences w/ existing lock walls, plumbing, handrail,dredging needs in auxiliary, etc.• Purchase ROV Camera (budget dependent)• Purchase Sawzall, chipping hammer, radios for dive team• Combined pre/post meeting each day including “all hands” (lock, dive team, EC,Structures)• Grease / wrap anchor bar threads (EC is looking <strong>at</strong> altern<strong>at</strong>ives)• Modify existing contract to revise barge loading configur<strong>at</strong>ion (2 g<strong>at</strong>es per barge)• Look <strong>at</strong> value <strong>of</strong> lock dew<strong>at</strong>ering <strong>at</strong> LD 20 and LD 22• Consider altern<strong>at</strong>ive loc<strong>at</strong>ion to accept g<strong>at</strong>es, maybe place on spare g<strong>at</strong>e barge• Safety –No divers near unsecured g<strong>at</strong>e


After Action ReviewContinu<strong>at</strong>ion <strong>of</strong> Good Practices• Have EC onsite and rel<strong>at</strong>ed experts during difficult install<strong>at</strong>ions• Good access to drawings/plans/photos both onsite and <strong>at</strong>District Office• Good Communic<strong>at</strong>ion and meetings: Good upfront coordin<strong>at</strong>ionmeetings during design, fabric<strong>at</strong>ion, and install<strong>at</strong>ion phasesbetween EC and Project Office• Successful salvaging <strong>of</strong> old critical parts from miter g<strong>at</strong>es such as“step‐ups” for use in future repair efforts.
