Hence the proposed model <strong>of</strong> care in South Africa is:1. Community Based Care And Support Services:There are two types <strong>of</strong> models <strong>for</strong> service delivery in a community, viz. Day Care and HomeBased Care Services. This guideline provides a framework <strong>for</strong> the development <strong>of</strong> an integratedsystem <strong>of</strong> community based care and support. This model refers to frail care services providedby a service centre or a frail care facility to frail older persons who live within the community.Older persons are taken to and from their homes daily.In Day Care older persons are provided with a variety <strong>of</strong> services covering the promotion andmaintenance <strong>of</strong> independent living and active ageing, i.e. promotion <strong>of</strong> a healthy life-style,physical exercises, prevention <strong>of</strong> continuous dependency, continuous care, i.e. hygiene,grooming, prevention <strong>of</strong> isolation / socialisation programmes, laundry, transport services,outreach programme <strong>for</strong> the home-bound older persons, provision <strong>of</strong> nutritious meals, provision<strong>of</strong> safety and security, respite care, provision <strong>of</strong> assistive devices and an in<strong>for</strong>mation centre <strong>for</strong>the community. In addition, this model provides training <strong>of</strong> volunteer care workers and caregiversat home, and provisioning and monitoring <strong>of</strong> services rendered by community carers.The Home Based Care Model is a programme on the continuum <strong>of</strong> care level <strong>of</strong> service deliverytargeting the housebound older persons as a result <strong>of</strong> frailty. Services include: basic homenursing, management <strong>of</strong> pressure areas and dressing <strong>of</strong> bed sores, tidying up <strong>of</strong> the frailperson‟s room, training <strong>of</strong> family members with regard to caring <strong>for</strong> the frail older persons, doingshopping <strong>for</strong> basic needs <strong>of</strong> the person, counselling, laundry especially on items such asbandages, night dresses, bed linen if the family cannot do it, help with basic activities <strong>of</strong> livingthat may not have been mentioned, advice and support to clients and families.2. <strong>Residential</strong> FacilityThis is an institution that <strong>of</strong>fers a 24-hour frail care service to frail older persons. Theseindividuals have been declared “frail” in terms <strong>of</strong> the DQ98. <strong>Residential</strong> services include 24-hourfrail care services to older persons, outreach services that are needed in the community wherethe residential facility is and / or a neighbouring community, training <strong>of</strong> volunteer caregivers whowill serve frail older persons, care and supervision services to older persons who are suffering67 | D S D A u d i t o f R e s i d e n t i a l F a c i l i t i e s
