UNIVERSITY OF KENTUCKY HOSPITALPHARMACY SERVICESPOSITION DESCRIPTIONPOSITION TITLE:PH-14-04Revised 9/09MEDICINE AMBULATORYCLINICAL PHARMACIST SPECIALISTPOSITION PURPOSE AND SUMMARY:1.0 DESCRIPTIONThe <strong>Ambulatory</strong> <strong>Care</strong> <strong>Clinical</strong> <strong>Pharmacist</strong> <strong>Specialist</strong> provides pharmaceutical careto Internal Medicine patients seen at the Kentucky Clinic and Gill Heart Institutethrough integrated clinical services, teaching, and research.2.0 QUALIFICATIONS2.1 Must possess a Pharm.D. Degree.2.2 Completed a Pharmacy Practice Residency with experience inambulatory care. A specialized clinical residency or equivalentexperience in primary care is preferred.2.3 Licensed as a pharmacist in the Commonwealth <strong>of</strong> Kentucky.2.4 Maintains competency in unique aspects <strong>of</strong> drug therapy forambulatory patients.3.0 RESPONSIBILITIESThe major job responsibilities (MJRs) <strong>of</strong> this position are: provision <strong>of</strong>pharmaceutical care to Internal Medicine and Cardiology clinic patients, clinicalintervention, training <strong>of</strong> pharmacy residents and students, teaching, and research.The following activities are components <strong>of</strong> the position to assist in meeting the majorjob responsibilities:3.1 Provide pharmaceutical care to Internal Medicine and Cardiology clinicpatients.3.1.01 Collaborate with physicians and other health care providersat the Kentucky Clinic and Gill Heart Institute3.1.02 Obtain and critically evaluate patient medication regimens,allergies, and past/current medical history3.1.03 Design, recommend, and monitor patient specifictherapeutic regimens by applying the principles <strong>of</strong> evidencebased medicine3.1.04 Document all clinical interventions in the patient’spermanent medical record


3.1.05 Prevent, detect, monitor, document, and report adversedrug reactions and medications errors, including basicphysical assessments techniques3.1.06 Provide pharmacokinetic consultations as necessary3.1.07 Serve as a drug information resource for patients, healthcare providers, and staff pharmacists at the KCP3.1.08 Establish and maintain a pharmacist-managedpharmacotherapy clinic, including performance <strong>of</strong> point <strong>of</strong>care testing3.1.09 Obtain and maintain billing privileges for clinicalpharmacy services provided in the pharmacotherapy clinic3.1.10 Work to improve continuity <strong>of</strong> care between the inpatientand outpatient setting3.2 Provide leadership in promoting and expanding pharmacist-managedpatient care services in the ambulatory care setting3.2.01 Aid in the design and implementation <strong>of</strong> disease statemanagement programs in the outpatient setting3.2.02 Respond to deficiencies in patient care coverage to improveoverall performance and patient outcomes3.2.03 Work toward getting Kentucky Clinic staff pharmaciststrained and actively involved in direct patient care activities3.2.04 Ensure physician and other health care provider satisfactionwith pharmacy services provided in clinic3.3 Promote and demonstrate safe, efficacious, & cost-effective drug therapy.3.3.01 Measure performance <strong>of</strong> pharmacist-managed drug therapy(i.e. appropriateness <strong>of</strong> therapy selected, patient outcomes,cost)3.03.02 Perform necessary quality assurance on all point <strong>of</strong> caredevices used in pharmacist-managed clinics3.03.03 Document clinical interventions and generate patientcharges where appropriate3.4 Facilitate outcomes research in the ambulatory care setting3.4.01 Participate in the design, execution, and publication <strong>of</strong> outcomesresearch in the ambulatory care setting3.4.02 Provide in-services for ambulatory care related outcomes studies tophysicians, nurses, and pharmacy staff3.5 Take a leadership role in the teaching programs <strong>of</strong> the Department <strong>of</strong>Pharmacy Services and College <strong>of</strong> Pharmacy as they are related toambulatory care3.5.01 Serve as Program director for the <strong>Ambulatory</strong> <strong>Care</strong> PharmacyResidency


3.5.02 Precept Pharm.D. clerkship students in the internal medicine,women’s health, and pharmacotherapy clinic3.5.03 Precept and mentor pharmacy residents3.5.04 Provide staff development support to staff pharmacists andtechnicians in the Kentucky Clinic Pharmacy3.5.05 Provide didactic lectures at the College <strong>of</strong> Pharmacy.3.5.06 Provide presentations, publications, and other informativeactivities on ambulatory care and other drug-related topics to thehealth care community and general public.3.5.07 Seek appropriate academic appointment with the College <strong>of</strong>Pharmacy.3.6 Participate on Medical Center and College <strong>of</strong> Pharmacy committees.3.6.01 Participate actively in team meetings, which include ResidencyExecutive Committee4.0 RELATIONSHIPS4.01.01 For this 80% position, the position will be funded 10%hospital, 10% college and 80% Internal Medicine.4.01.02 Will report to the Assistant Director <strong>of</strong> Pharmacy Servicesin the Department <strong>of</strong> Pharmacy Services. Workscollaboratively with medical, nursing and other pharmacystaff.4.01.03 The faculty title will be in the Adjunct Title Series.4.01.04 As a faculty member, will report to the Department Chairfor the Division <strong>of</strong> Pharmacy Practice & Science in theCollege <strong>of</strong> Pharmacy.5.0 PATIENT POPULATION SERVEDThe Pharmacy Department provides pharmaceutical care to neonatal, pediatric,adolescent, adult and geriatric populations. This position provides ambulatory careservices to adult and geriatric patients.
