KrüsselJ Reproduktionsmed Endokrinol 2010; 7 (6): 470–97.• PrefacePresenting data of the year 2009, theGerman <strong>IVF</strong> Registry (<strong>Deutsches</strong> <strong>IVF</strong>Register, D·I·R.) has already started topublish the annual report in this journal.Since 1996 the structure of this reporthas combined standard analysis and specialcharts focusing on new aspects. Thisyear we have included a statistic concerningthe so-called „ideal patient“ andthe pregnancy rates for each center.In Germany advertising is not allowed inthe medical field. Therefore all datamust be published anonymously.In 2009 the number of centers increasedfrom 119 to 121. The amount of socalledfresh <strong>IVF</strong> and ICSI cycles also increasedby 7% compared to 2008 – includingcycles in which <strong>IVF</strong> and ICSIwas performed simultaneously.This is a positive development after thetremendous decrease in 2004 caused bya dramatic change in the public healthreimbursement system.In 1997, so-called prospective measureswere introduced to the dataset. Usingone of the three different software toolscurrently available, the treatment cyclemust be recorded within 7 days of controlledovarian stimulation. This featureis intended to prevent cycle selection andis one of the most powerful quality toolsimplemented by the German system. In2009 84.3% of all <strong>IVF</strong> and ICSI cyclesfit this criterion.A dynamic link library (DLL) maintainedby the registry allows data plausibilityto be monitored either online orshortly after data input. In 2009 97.24%of all cycles fit this criterion.The average pregnancy rate for all11,715 <strong>IVF</strong> cycles was 29.5%, comparedwith a rate of 28.6% for the 9,830ICSI cycles. A simultaneous treatment of<strong>IVF</strong> and ICSI (<strong>IVF</strong>/ICSI) in one cyclewas performed in 254 cases, while socalledcryopreserved embryo transfer(CPE, embryos derived from cryo-preservedoocytes in the 2-PN-stage) wasperformed in 17,116 cases. Here, thepregnancy rate was recorded as being18.9%.Because of German legal restrictions,oocytes were only allowed to be frozenin the pronuclear stage. No embryo selectionis permitted. The Embryo ProtectionAct, passed in 1991, permits nomore than three embryos to be transferred.A higher rate of multiple births (twins,triplets or more) was documented for amaximum of 7.8% of all pregnancies.The number of lost for follow-up will be40.6% due to the early deadline for dataexport to D·I·R this year. This numberwill be updated and republished as partof the next annual report.Since the registry requests that data forboth the current year as well as for pastyears be sent, this figure generally willdrop to less than 15 % the followingyear.In order to provide additional informationabout pregnancy outcomes, the nationalregistry of assisted reproductivetreatments expends a great deal of effortin following up on induced pregnancies.As in other European nations, numerousforeign couples are also treated in Germany,making it rather difficult to obtaininformation related to the outcome ofthese pregnancies.For the first time, this publication providesan English version of an annual report.In the past, some papers have alreadyprovided details regarding certainspecial aspects. The huge dataset whichis expected to reach one million in 2010also allows us to research specific aspectssuch as lifestyle factors (smoking,weight) or reproductive history (formerpregnancies, miscarriages etc.).From our perspective, the greatest advanceof the German <strong>IVF</strong> Registry lies inthe decision of nearly every <strong>IVF</strong> unit tosupport its work through a prospectivedata collection and through paymentsmade to maintain the Registry files.Correspondence: D .
