COMPRESSION GARMENTS FOR<br />
MIDLINE LYMPHOEDEMA<br />
Compression garments can be used to<br />
treat <strong>lymphoedema</strong> <strong>of</strong> <strong>the</strong> head and neck,<br />
breast, trunk or genitalia. These garments<br />
may be custom made or ready to wear.<br />
Garments <strong>for</strong> <strong>the</strong> torso are usually<br />
classified as providing medium<br />
compression (25-30mmHg), while lower<br />
pressures are used on <strong>the</strong> head. However,<br />
<strong>the</strong>re is no recognised agreement on <strong>the</strong><br />
appropriate level <strong>of</strong> compression <strong>for</strong> <strong>the</strong>se<br />
patients.<br />
Leotard or bodice style garments may be<br />
useful <strong>for</strong> patients with truncal oedema and<br />
flat knit construction is preferable. Patients<br />
with breast <strong>lymphoedema</strong> may require a<br />
ready to wear or custom made bra. For<br />
patients with scrotal swelling, scrotal<br />
supports can be used. Anatomically<br />
contoured foam padding inserted into<br />
compression pantyhose or shorts can be<br />
used in female genital <strong>lymphoedema</strong>. Groin<br />
swelling is <strong>of</strong>ten accompanied by tissue<br />
thickening, and may occur in combination<br />
with lower limb <strong>lymphoedema</strong>; one- or twolegged<br />
closed gusset pantyhose angled<br />
across <strong>the</strong> groin with foam chip stasis pads<br />
may be helpful.<br />
OTHER COMPRESSION DEVICES<br />
Inelastic adjustable compression devices<br />
are available <strong>for</strong> <strong>the</strong> treatment <strong>of</strong><br />
<strong>lymphoedema</strong>. The compression <strong>the</strong> device<br />
applies can be adjusted by altering how<br />
tightly <strong>the</strong> straps used to fix <strong>the</strong> garment in<br />
place are pulled. They can be used to<br />
contain swelling in patients with moderate<br />
or severe <strong>lymphoedema</strong> <strong>of</strong> <strong>the</strong> upper or<br />
lower limb and <strong>the</strong> torso, and are useful self<br />
<strong>management</strong> tools.<br />

NB The compression applied by knitted armsleeves is graduated. The compression applied at <strong>the</strong> proximal end <strong>of</strong> <strong>the</strong> garment is 50-80% <strong>of</strong> that applied at <strong>the</strong> wrist.<br />

ADVISORY NOTE: UPPER LIMB<br />
LYMPHOEDEMA<br />
It is advisable <strong>for</strong> patients who wear<br />
armsleeves to also wear a handpiece when<br />
travelling by aeroplane.
