Executive summary: Features derived from examination of patients who died in the UK 2009–10 pandemicExecutive summaryBackgroundFrom April 2009 to January 2010, the pandemicof A/H1N1 influenza affected the UK. Therewere > 30,000 infections and 457 deaths (allages). Reports from other countries had indicatedthat certain comorbidities (young age, asthma,pregnancy, diabetes, obesity) were associated witha higher risk of death from H1N1 infection, andthere was a need to identify these factors in the UKpopulation as knowledge of them could lead toimproved treatment in the current epidemic andreduced mortality in future epidemics. In additionto clinical observation research in life, examinationof autopsy data would provide information on theimportant comorbidities.Objectives• To gather all the available clinical pathologyinformation from autopsies performed onpatients – adults and children – dying withknown or suspected influenza A/H1N1infection, across the UK.• To evaluate comorbidities present in thesedeceased patients; correlate them with theH1N1-related pathology and treatmentassociatedpathology, determine their relativecontributions and estimate the significantfeatures associated with death.MethodsTo obtain the autopsy reports, which wouldcomprise the results of both medico-legal autopsiesand hospital/consented autopsies, help wasobtained from the Royal College of Pathologists(RCPath), the Coroner Society of England andWales, the Crown Office and Procurator FiscalService and the Centre for Maternal and ChildEnquiries.Standard request letters were sent by e-mail to allhistopathologists in the UK on the RCPath list,all the coroners’ jurisdictions in England, Walesand Northern Ireland, and to procurators fiscalin Scotland. The letters asked for autopsy reportswith the following case definitions of the autopsieddeceased:• From pathologists:––those with H1N1 infection, proven beforeor after death, and those in whom swine fluwas unproven but most likely to have beenpresent––those in whom H1N1 was a minorpathology, as well as those in whom it wasthe immediate cause of death.• From coroners and procurators fiscal:––mention of ‘swine flu’, ‘swine influenza’ or‘H1N1 infection’ in any part of the cause ofdeath, i.e. in Part 1 or in Part 2 of the causeof death statement––any age from infancy to old age.ResultsSixty-eight autopsy reports were received: 19children (0–15 years) and 49 adults (16 + years).All but two autopsies were medico-legal, and onlytwo (3% of the total) were consented. This samplethus represents 15% of the known 457 deaths fromH1N1.The total number of autopsied H1N1-associateddeaths was not identified. The informationobtainable from autopsy reports was dependenton the amount provided therein, but, overall, thestandard and quality of the medico-legal reportswas higher than the average for this type ofautopsy.Median age for children at death was 6 years, foradults it was 41 years.Deaths in children were associated with congenitaldiseases (47%, 9/19), particularly of the heart andcentral nervous system. The autopsied childrenwere not obese. Death in adults were associatedwith pregnancy (three cases in the study, butnationally 12/457 H1N1-associated deaths werenoted), obesity (50% of adults had a body massindex ≥ 30 kg/m 2 ), and chronic respiratory disease(12%, 6/49 adults). Diabetes did not emerge as arisk factor for death, but learning difficulties did.
