For the 12 months ending30th June 2004, four requests were received and onewas withdrawn by the applicant. The three remainingrequests received partial access. The most frequentlyused exemption was section 33(1) which exemptsthe unreasonable disclosure of the personal affairs ofany person. One internal review was also completed in2003/04.Freedom of Information requests should be madein writing describing the documents requested andincluding payment of the $20 application fee. Furthercharges may apply. Charges other than the applicationfee can be waived if the request is a routine requestor for access to a document related to the applicant’spersonal affairs. The application fee can be waivedor reduced if the applicant would be caused hardshipbecause he/she had to pay the fee.Requests should be forwarded to Chief OperatingOfficer, NGV, PO Box 7259, Melbourne Vic 3002.Building maintenanceIn 2003-04 NGV occupied premises as a tenant atFederation Square and at 112 Macaulay Road NorthMelbourne and reoccupied the St Kilda Road building.The St Kilda Road building was under builders defectswarranties until April 2004. NGV complied with themaintenance provisions of the Building Act 1993.
156 Report NGV Annual Report 2003/04Availability of additional informationThe following information relating to the NGV, relevantto the financial year, has been prepared and is availableto the Minister, Members of Parliament and the publicon request:• Declarations of pecuniary interests duly completed byall relevant officers• Details of publications produced by the NGV aboutthe NGV and the places where the publicationscan be obtained• Details of changes in prices, fees, charges, rates andlevies charged by the NGV• Details of any major external reviews carried outon the NGV• Details of overseas visits undertaken including asummary of the objectives and outcomes of each visit• Details of major promotional, public relations andmarketing activities undertaken by the NGV todevelop community awareness of the NGV and theservices it provides• Details of assessments and measures undertakento improve the occupational health and safety ofemployees• A general statement on industrial relations withinthe NGV and details of time lost through industrialaccidents and disputes• A list of major committees sponsored by the NGV, thepurposes of each committee and the extent to whichthe purposes have been achievedHealth and safetyThe Council of Trustees and management of the NGVplace a high priority on the health and safety of theirstaff members. The Occupational Health and SafetyCommittee met on six occasions. New initiativesincluded:• Safety and evacuation training for staff to coincidewith the move to NGV International at St Kilda Road• Commissioned an Occupational and Safety RiskManagement Plan for all NGV sitesThe Committee undertook a vigorous educationprogram. 36 days were lost as a result of work relatedaccidents compared to 65 days in 2002/03.PrivacyNGV has complied with all requirements under theInformation Privacy Act 2000. For the 12 monthsending 30 June 2004 there were no complaintsagainst the NGV in relation to breaches of privacy.Whistleblowers Protection Act 2001This section of the report contains information that isrequired to be published annually under section 104 ofthe Whistleblowers Protection Act 2001 (‘the Act’).a) The current procedures established by the NGVunder Part 6 of the Act can be found directly below;b) The NGV has received no disclosures duringthe year;c) The NGV has not referred any disclosures to theOmbudsman for determination as to whether they arepublic interest disclosures during the year;d) The Ombudsman has not referred any disclosedmatters to the NGV during the year;e) The NGV has not referred any disclosed matters tothe Ombudsman to investigate during the year;f) The Ombudsman has not taken over anyinvestigation of disclosed matters from the NGVduring the year;g) The NGV has made no request under section 74 ofthe Act to the Ombudsman to investigate disclosedmatters during the year;h) The NGV has not declined to investigate a disclosedmatter during the year;i) There have been no disclosed matters that weresubstantiated on investigation. There has been noaction required to be undertaken arising from aninvestigation, since there have been no investigations;andj) The Ombudsman has not made any recommendationunder the Act that relates to the NGVNGV Whistleblowers Procedures1. Statement of support to whistleblowersThe NGV is committed to the aims and objectives ofthe Whistleblowers Protection Act 2001. It does nottolerate improper conduct by its employees, or thetaking of reprisals against those who come forward todisclose such conduct.The NGV recognises the value of transparency andaccountability in its administrative and management
