Basic <strong>in</strong>formati<strong>on</strong>1) Proposal for <strong>in</strong>clusi<strong>on</strong> submitted by:2) Name <strong>of</strong> compound; chemical name(s):3) Identificati<strong>on</strong> <strong>of</strong> (additi<strong>on</strong>al) data (toxicology, metabolism, occurrence, food c<strong>on</strong>sumpti<strong>on</strong>) which could beprovided to JECFA:4) List <strong>of</strong> countries where surveillance data are likely to be available, and if possible list <strong>of</strong> c<strong>on</strong>tact pers<strong>on</strong> whocould provide such data, <strong>in</strong>clud<strong>in</strong>g quality assurance <strong>in</strong>formati<strong>on</strong> <strong>on</strong> the data.5) Timel<strong>in</strong>e for data availability:2.
