Scholarships Eligibility CriteriaGeneral Information of CYSA:Christian Youth Sport Academy is a non-profit organization that was established in1998 for the purpose of promoting academic excellence through the provision of ascholarship program and to provide individuals with the opportunity to participate insports activitiesThe general eligibility, selection and application requirements for the CYSAAnnual Scholarships are as follows:Eligibility:• A Canadian citizen or a permanent resident• Graduating from High School or have graduated from High School (verificationnecessary)• Enrolled in a full time degree (undergraduate or graduate), or• Enrolled into a Diploma or certificate program at an accredited College orUniversity for the academic year commencing in the year in which thescholarship application is submitted. Students commencing their studies in thefall are eligible• Entering 1 st year of University or CollegeSelection Criteria:• High academic achievement• Financial need (some verification necessary)• Athletic involvement and or contribution to the community (verificationnecessary)• Recognized contribution to the communityApplication Requirements:• A completed application form• A letter (2 pages minimum) describing the reasons why you feel you would be aworthy recipient of a CYSA Scholarship. Include your academic achievements,athletic involvement and contributions to the community, your most importantaccomplishments, and your future goals

• A completed financial information schedule stating your budget for the comingyear including information on your expected sources of funding (e.g. otherscholarships received, student loan, parents, etc.), and related information, toassist us in determining your financial need• A letter of reference from the two individuals named in your application. Onemust be a teacher from your high school. The other must be an individual, otherthan your teacher or a family member, who is familiar with your communityservice• An up to date official transcript• Two passport size photos – head shot (once submitted these photos will be theproperty of the CYSA Scholarship Fund)Deadline:• Applications must be received by the Christian Youth Sports Academy (CYSA)no later than 5:00 p.m. on July 15 st .• Applications sent by e-mail will not be accepted.Decisions:• Scholarship recipients will be notified by late July and will be required toprovide two photographs for publicity purposes• Only successful candidates will be notified• Recipients must be at the Presentation Awards Event/ Rally and will be notifiedof the date, time and venue• The CYSA Board of Directors and the CYSA Scholarship Committee wishes tothank all applicants for their interest in the CYSA ScholarshipsMail or deliver completed applications to:Christian Youth Sports Academy (CYSA)Scholarship Application Department531 Finch Ave. WestWillowdale, OntarioM2R 3X2Please read this information carefully and complete the application in its entirety.For enquiries please contact us at cysa@live.ca
