WelcomeMike WallaceChief Executive<strong>Sydney</strong> South WestArea <strong>Health</strong> Service02 <strong>Year</strong> <strong>in</strong> <strong>Review</strong>On 10 March 20<strong>06</strong>, I was appo<strong>in</strong>ted Chief Executive of <strong>Sydney</strong> South WestArea <strong>Health</strong> Service (SSWAHS). My appo<strong>in</strong>tment followed the <strong>in</strong>spir<strong>in</strong>gtenure of Professor Diana Horvath AO, who served as Chief Executive ofthe Central <strong>Sydney</strong> Area <strong>Health</strong> Service, and later SSWAHS, for 13 years.Professor Horvath’s achievements were numerous. They <strong>in</strong>cludeddevelopment of the <strong>in</strong>ternationally recognised cl<strong>in</strong>ical stream system;the successful amalgamation of the South Western <strong>Sydney</strong> and Central<strong>Sydney</strong> Area <strong>Health</strong> Services; and the implementation of rigorousbus<strong>in</strong>ess practices where f<strong>in</strong>ancial management was simplified,adm<strong>in</strong>istration streaml<strong>in</strong>ed and IT modernised, allow<strong>in</strong>g healthcareproviders to use the latest technology to make their work more efficientand more accurate.Professor Horvath, who always placed safe and effective patient careat the core of our objectives, has s<strong>in</strong>ce taken up the position of ChiefExecutive of the Australian Commission on Safety and Quality <strong>in</strong> <strong>Health</strong>Care. We all wish her well <strong>in</strong> her new endeavour.It has now been two years s<strong>in</strong>ce the former <strong>NSW</strong> M<strong>in</strong>ister for <strong>Health</strong>, theHon Morris Iemma, announced the reforms to the <strong>NSW</strong> <strong>Health</strong> systemwhich resulted <strong>in</strong> the amalgamation of the South Western <strong>Sydney</strong> andCentral <strong>Sydney</strong> Area <strong>Health</strong> Services. SSWAHS cont<strong>in</strong>ues to work to effectall of the required organisational changes associated with theamalgamation.Staff have worked diligently throughout the year to establish s<strong>in</strong>gleArea-wide cl<strong>in</strong>ical stream structures, determ<strong>in</strong>ed <strong>in</strong> l<strong>in</strong>e with theamalgamation plan, <strong>in</strong> order to deliver the best patient care.SSWAHS has significantly benefited from the merger of these streams,which allows shar<strong>in</strong>g of knowledge and a common approach to thedelivery of patient care. Other flow-on benefits have <strong>in</strong>cluded thecentralisation of adm<strong>in</strong>istrative functions, result<strong>in</strong>g <strong>in</strong> f<strong>in</strong>ancial sav<strong>in</strong>gsof $4.8 million. These sav<strong>in</strong>gs have been redirected to frontl<strong>in</strong>ecl<strong>in</strong>ical services.All staff can be extremely proud of reduc<strong>in</strong>g to zero the list for long-waitpatients, i.e. those patients wait<strong>in</strong>g for elective surgery for more than12 months.At the end of June <strong>2005</strong>, there were 1,384 long-wait patients on thewait<strong>in</strong>g list. As part of the <strong>NSW</strong> Government’s Predictable SurgeryProgram, the list was reduced to zero by 30 June 20<strong>06</strong>, and this can bedirectly attributed to the commitment of our doctors and nurses andthe professionalism and dedication of all our staff.

<strong>Health</strong> Service ProfileOn 27 July 2004, the Hon Morris Iemma,M<strong>in</strong>ister for <strong>Health</strong>, announced therestructure of area health services across<strong>NSW</strong>, <strong>in</strong>clud<strong>in</strong>g the formation of the new<strong>Sydney</strong> South West Area <strong>Health</strong> Service(SSWAHS), amalgamat<strong>in</strong>g the previousCentral <strong>Sydney</strong> and South Western <strong>Sydney</strong>area health services. This is mostnotable <strong>in</strong> Fairfield and Canterbury, whereover 60 per cent of the population do notspeak English at home. A high proportionof new migrants to Australia, <strong>in</strong>clud<strong>in</strong>grefugees, choose to settle <strong>in</strong> south west<strong>Sydney</strong>. There are also considerablevariations between local governmentareas (LGAs) <strong>in</strong> the proportion of thepopulation identify<strong>in</strong>g as Aborig<strong>in</strong>al(highest <strong>in</strong> South <strong>Sydney</strong> andCampbelltown).In addition to the <strong>in</strong>flux of new migrants,the Area’s population is grow<strong>in</strong>g byaround 19,000 new births per annum. Across the Areathere are 180,000 people over the age of65, represent<strong>in</strong>g 17 per cent of thepopulation. There are large numbers ofelderly people <strong>in</strong> some LGAs, with morethan 24,000 people over the age of 65 <strong>in</strong>Bankstown and around 18,000 <strong>in</strong> both0-14 years15-44 years45-64 years65-74 years75+ yearsFairfield and Canterbury. Hospital data<strong>in</strong>dicates that people over the age of 65utilise 45 per cent of all acute hospital beddays. The number of people aged over65 is projected to <strong>in</strong>crease by 45 per centby 2016, when they will represent 13 percent of the population.South west <strong>Sydney</strong> has some of thepoorest communities <strong>in</strong> the state,characterised by a large number of recentmigrants, significantly higher levels ofunemployment and a high proportion offamilies dependent on welfare. The Areahas n<strong>in</strong>e of the ten most disadvantagedmetropolitan <strong>Sydney</strong> postcodes.The age-standardised death rates forSSWAHS residents are higher than thestate average for both males (721.8 per100,000 vs 709) and females (457 per100,000 vs 443).The four major causes of deathare circulatory diseases, cancers,<strong>in</strong>jury/poison<strong>in</strong>g and respiratorydiseases. These comprise about 80 percent of all deaths <strong>in</strong> both SSWAHS and<strong>NSW</strong>. These four causes of death are allapplicable to both males and females, butdeaths due to cancer, <strong>in</strong>jury/poison<strong>in</strong>g andrespiratory diseases were higher amongmales. Balma<strong>in</strong>HospitalA/General ManagerAnn KellyBalma<strong>in</strong> Hospital and its communitybasedservices cont<strong>in</strong>ued to provide animportant aged care and rehabilitationrole <strong>in</strong> SSWAHS eastern zone.Balma<strong>in</strong> Hospital has a total of 78 beds,with an 88 per cent occupancy rate. Totalbed days numbered 24,624 and therewere 1,774 admissions.The General Practice Casualty providestreatment for residents <strong>in</strong> the local area.The Strength Tra<strong>in</strong><strong>in</strong>g RehabilitationOutreach Needs <strong>in</strong> General Medic<strong>in</strong>e(STRONG) Cl<strong>in</strong>ic has treated over 2,000patients between the ages of 30 and 100.Over 500 general practitioners referpatients to the Cl<strong>in</strong>ic. Clients attend<strong>in</strong>gthe Cl<strong>in</strong>ic have experienced improvements<strong>in</strong> strength, balance and endurance.The hospital also provides cl<strong>in</strong>ics <strong>in</strong>diabetes, cont<strong>in</strong>ence, homoeopathy andacupuncture. These services cont<strong>in</strong>ue tocater for patients and clients fromculturally and l<strong>in</strong>guistically diversebackgrounds.Dur<strong>in</strong>g October, the Australian Councilon <strong>Health</strong>care Standards (ACHS)conducted an organisation-wide EQUIPaccreditation survey at Balma<strong>in</strong> Hospitaland Camperdown General Geriatric andRehab Medic<strong>in</strong>e Community, and awardedfull accreditation of four years to bothservices.An Acupuncture Cl<strong>in</strong>ic <strong>in</strong> conjunction withthe Masters of Medical Acupuncturecommenced <strong>in</strong> June and has been verysuccessful <strong>in</strong> the first month of operation.Attendances and outcomes for this cl<strong>in</strong>icwill cont<strong>in</strong>ue to be monitored.Refurbishment is tak<strong>in</strong>g place <strong>in</strong> MortsDock Ward. The STRONG Cl<strong>in</strong>ic willrelocate to this area and a 14-bedTransition Care Unit will open <strong>in</strong>August 20<strong>06</strong>.BankstownHospitalGeneral ManagerGlenda CleaverBankstown Hospital is a pr<strong>in</strong>cipal referralhospital provid<strong>in</strong>g acute services that<strong>in</strong>clude emergency medic<strong>in</strong>e, maternity,general and specialist surgery, generaland specialist medic<strong>in</strong>e, day surgery,outpatients and neonatology, as well as arange of specialist outpatient services.The hospital treated over 28,000 patientsand provided over 390,000 non-admittedpatient occasions of service dur<strong>in</strong>g theyear. There were almost 34,000attendances at the emergencydepartment and 1,893 births.The hospital achieved a four-yearaccreditation <strong>in</strong> the Australian Council on<strong>Health</strong>care Standards organisation-widesurvey <strong>in</strong> March 20<strong>06</strong>.The emergency department <strong>in</strong>troducedthe Fast Track <strong>in</strong>itiative, which reducedwait<strong>in</strong>g times for ED patients. The numberof patients wait<strong>in</strong>g more than 12 monthsfor surgery was reduced to zero <strong>in</strong>June 20<strong>06</strong>.An additional 12 beds were openedacross the hospital, renal dialysisrelocations created two additional chairs,and The Way Forward fund<strong>in</strong>g allowedan additional ICU bed and staff<strong>in</strong>genhancements <strong>in</strong> the unit.An exist<strong>in</strong>g build<strong>in</strong>g is be<strong>in</strong>g refurbishedto accommodate the newly appo<strong>in</strong>tedProfessor of Medic<strong>in</strong>e and his researchteam <strong>in</strong> a new Research Centre.The Arts for <strong>Health</strong> Program participated<strong>in</strong> the Dads 4 Dads project, establish<strong>in</strong>g adedicated exhibition space to display artcelebrat<strong>in</strong>g the role of fathers. The Art ofAge<strong>in</strong>g, another Arts for <strong>Health</strong> Program<strong>in</strong>volv<strong>in</strong>g 150 older people, led to thecreation of writ<strong>in</strong>gs, draw<strong>in</strong>gs andphotographs, which were shown <strong>in</strong> thehospital’s Arts for <strong>Health</strong> galleries.Bankstown Hospital also won theopportunity to host the 20<strong>06</strong> Inter-Hospital M<strong>in</strong>i-Olympic Games.M<strong>in</strong>ister for <strong>Health</strong> John Hatzistergos and<strong>Sydney</strong> City Councillor Verity Firth outsideBalma<strong>in</strong> Hospital.<strong>Sydney</strong> South West Area <strong>Health</strong> Service<strong>Year</strong> <strong>in</strong> <strong>Review</strong> 05><strong>06</strong> 11


FacilitiesBowral and <strong>District</strong>HospitalGeneral Manager Denis ThomasBowral and <strong>District</strong> Hospital provides awide range of services, <strong>in</strong>clud<strong>in</strong>g generalmedic<strong>in</strong>e, obstetrics and gynaecology,paediatric, surgical, orthopaedics,ophthalmology and emergency services.New pre-admission cl<strong>in</strong>ic facilities havebeen established, and the removal ofasbestos across the site has beencompleted.Follow<strong>in</strong>g the appo<strong>in</strong>tment of a full-timestaff specialist <strong>in</strong> obstetrics andgynaecology, antenatal (<strong>in</strong>clud<strong>in</strong>g midwifeonly) and gynaecology cl<strong>in</strong>ics have beenestablished <strong>in</strong> Bowral and Tahmoor.Paediatricians <strong>in</strong>creased their hours andmoved <strong>in</strong>to new consult<strong>in</strong>g rooms on site.The position of VMO Geriatrics was createdand Dr Peter Noonan was appo<strong>in</strong>ted asMedical Director of the EmergencyDepartment.Nurs<strong>in</strong>g services recruited seven overseasnurses and established two cl<strong>in</strong>ical nurseeducator positions. Admissions and GPLiaison Nurses have been established onthe general medical-surgical ward.Plans have been developed for therefurbishment of the Children’s Ward andrelocation of the Short Stay Unit.The Capta<strong>in</strong> Starlight tw<strong>in</strong>s visit a young patientat Bowral Hospital.Photo courtesy of the Southern Highlands NewsCamden andCampbelltownHospitals andQueen VictoriaMemorial HomeGeneral Manager Amanda Lark<strong>in</strong>Campbelltown Hospital, a majormetropolitan hospital with 3<strong>06</strong> beds, andCamden Hospital, a district hospital with83 beds, provide <strong>in</strong>tensive care, cardiology,maternity, gynaecology, paediatrics,palliative care, respiratory and strokemedic<strong>in</strong>e, surgery and emergencymedic<strong>in</strong>e and broad aged care servicesto the Macarthur community.Camden and Campbelltown hospitals alsoadm<strong>in</strong>ister the 100-bed Queen VictoriaMemorial Nurs<strong>in</strong>g Home.Activity at Camden and Campbelltownhospitals cont<strong>in</strong>ued to grow, with a 2.09 percent <strong>in</strong>crease <strong>in</strong> admissions and a 13 percent <strong>in</strong>crease <strong>in</strong> emergency presentations.Campbelltown Hospital opened 27 newbeds and completed the $133.7 millionMacarthur Strategy with the open<strong>in</strong>g of anew stroke ward and new cardiology andallied health and paediatric outpatientareas. Other capital works <strong>in</strong>cludedredevelopment of the renal unit toaccommodate four additional chairs, arestructure of the ED reception area andcompletion of the Mental <strong>Health</strong> Sub-Acute Unit. A second l<strong>in</strong>ear acceleratorand plann<strong>in</strong>g CT were <strong>in</strong>stalled <strong>in</strong> theMacarthur Cancer Therapy Centre.The Camden Midwifery Group Practicecommenced <strong>in</strong> March, provid<strong>in</strong>g women<strong>in</strong> the Camden area with cont<strong>in</strong>uity ofmidwifery care throughout their pregnancy,birth and postnatal period.Additional ED specialists started work<strong>in</strong>gacross Liverpool and Campbelltownhospitals. Plann<strong>in</strong>g began for the Universityof Western <strong>Sydney</strong> (UWS) Medical School,with some teach<strong>in</strong>g staff recruited andplans prepared for a cl<strong>in</strong>ical school on theCampbelltown Hospital site. Professor BradFrankum was named the <strong>in</strong>augural Cl<strong>in</strong>icalDean of the School and Professor ofCl<strong>in</strong>ical Education.CanterburyHospitalGeneral Manager Gary MillerCanterbury Hospital is a majormetropolitan general hospital. The SDH is also amajor referral centre fortertiary oral healthmanagement.The community oral healthcl<strong>in</strong>ics provide general dentalservices to both eligible adultsand children at their cl<strong>in</strong>ics. TheSchool Assessment Programprovides an assessment andtreatment service to children.Integration of the InformationSystem for Oral <strong>Health</strong>database was completed <strong>in</strong><strong>2005</strong>/<strong>06</strong> ahead of schedule.The appo<strong>in</strong>tment of theassociate director cl<strong>in</strong>icaloperations for the CommunityOral <strong>Health</strong> Service, along withthe three service managers forthe eastern, central and southwest clusters, has assisted thecl<strong>in</strong>ical and corporate functionsof the recently merged service.The acquisition of 22 digitalradiography units for theCommunity Oral <strong>Health</strong> Cl<strong>in</strong>icat the SDH has seen thehospital become the biggestprovider of digital radiography<strong>in</strong> Australia and hasdramatically improved cl<strong>in</strong>icalefficiency.20 <strong>Year</strong> <strong>in</strong> <strong>Review</strong>

Malnutritionscreen<strong>in</strong>g was <strong>in</strong>troduced atConcord, RPA and Canterburyhospitals for earlyidentification of patients atrisk of malnutrition, and a newrenal pre-dialysis cl<strong>in</strong>ic was setup <strong>in</strong> conjunction with a nurseand a social worker <strong>in</strong>Campbelltown.At Bankstown Hospital, an<strong>in</strong>flammatory bowel diseaseoutpatient nutrition cl<strong>in</strong>ic wasset up, and a new groupprogram has been establishedfor young male haematologypatients to help themunderstand and cope withtheir cancer.The Women and Children’s<strong>Health</strong> Social Work Team, <strong>in</strong>conjunction with newborn carestaff and the Department ofCommunity Services, ran asuccessful program for fostercarers of babies with neonatalabst<strong>in</strong>ence syndrome.Director ProfessorChris O’BrienCancer Services is responsiblefor approximately 16 per centof the state’s cancer burden,with approximately 5,300 newcancers diagnosed <strong>in</strong> the Areaeach year. There are alsosignificant <strong>in</strong>flows of cancerpatients <strong>in</strong> the eastern zonefrom referrals made fromregional <strong>NSW</strong> to the <strong>Sydney</strong>Cancer Centre.Services are delivered througha network of three centres –the <strong>Sydney</strong> Cancer Centre (acollaboration <strong>in</strong> cancer carebetween RPA, Concord Hospitaland the University of <strong>Sydney</strong>),the Liverpool Cancer TherapyCentre (CTC) and theMacarthur Cancer TherapyCentre. There is also a CancerUnit at Bankstown. Outreachservices are provided throughthe <strong>Sydney</strong> Cancer Centre topatients <strong>in</strong> the Greater WesternArea <strong>Health</strong> Service at Dubbo.The Area Cancer Service isclosely l<strong>in</strong>ked with the CancerInstitute (CI) <strong>NSW</strong>. SSWAHS isone of five areas selected bythe CI <strong>NSW</strong> for development ofthe Cl<strong>in</strong>ical Cancer Registry.Three new l<strong>in</strong>ear acceleratorshave been <strong>in</strong>stalled at RPA, anda new l<strong>in</strong>ear accelerator<strong>in</strong>stalled at Liverpool has beencommissioned. Campbelltownhas two l<strong>in</strong>ear accelerators.Wait<strong>in</strong>g times are now athistorical lows for patientsrequir<strong>in</strong>g radiotherapy.In the western zone, ProfessorMichael Barton has beenappo<strong>in</strong>ted to the Chair <strong>in</strong>Radiation Oncology, jo<strong>in</strong>tlyfunded by CI <strong>NSW</strong>, theUniversity of New South Walesand SSWAHS. Staff arealso <strong>in</strong>volved <strong>in</strong> education at<strong>Sydney</strong> and New South Walesuniversities, and the service hasa significant research program.The laboratory <strong>in</strong>formationsystem has been substantiallyrestructured. Results of alltests are now viewable <strong>in</strong> allSSWAHS facilities, and astructured system ofpasswords ensuresconfidentiality. More than 600 heartoperations were performed bythe cardiothoracic surgicalteam.The Concord CardiologyDepartment has had anoutstand<strong>in</strong>g year of researchand development, with twoPhD students supervised bysenior cardiologists awardedprestigious prizes from theCardiac Society of Australia andNew Zealand. Cl<strong>in</strong>ical GroupsGastroenterology andLiver ServicesCl<strong>in</strong>ical DirectorProfessor Les BokeyGastroenterology andLiver Services offers amultidiscipl<strong>in</strong>ary approach <strong>in</strong>gastroenterology, uppergastro<strong>in</strong>test<strong>in</strong>al, colorectal,hepatobiliary, liver transplantand general surgery at Concord,RPA and Canterbury hospitals.Transplantation servicescelebrated the 20thanniversary of livertransplantation at RPAand pioneered a uniquesurgical technique <strong>in</strong> a tripletransplant procedure of liver,pancreas and kidney.Additional operat<strong>in</strong>g time hasbeen allocated at ConcordHospital’s division of surgery,ensur<strong>in</strong>g more patients haveaccess to surgery with<strong>in</strong>recommended timeframes.RPA’s gastroenterology unit has<strong>in</strong>troduced the technique ofdouble-balloon enteroscopy.Capsule endoscopy has beenused to <strong>in</strong>vestigate small bowelCrohn’s disease.The Department ofGastroenterology andHepatology at Concord is oneof the busiest endoscopydepartments <strong>in</strong> <strong>NSW</strong> and is atra<strong>in</strong><strong>in</strong>g centre for advancedendoscopy techniques.The upper GI Units at RPA andConcord provide tertiaryreferralhepatobiliary andcomplex endosurgicalprocedures. Complex liverresections are offered at RPA,and complex oesophagealsurgery is offered at Concord;both are available as statewideservices.The Concord and RPAColorectal Units are bothtertiary-referral centres. The unit benchmarksits dialysis outcomes dataaga<strong>in</strong>st Australian and NewZealand centres. Outcomes areat least equivalent to the pooledoutcome data <strong>in</strong> the ANZ DATAregistry.Cardiac ServicesArea DirectorDr Andrew Hopk<strong>in</strong>sCardiology units are based atall western zone hospitals.Liverpool Hospital providesdiagnosis and <strong>in</strong>terventionalcatheter laboratory facilities,as well as electrophysiologyservices. Bankstown, Liverpool,Fairfield and Campbelltownhospitals provideechocardiography services,with all except Fairfieldcurrently provid<strong>in</strong>g an activetransoesophagealechocardiography service.Liverpool Hospital has an activeresearch unit, headed byProfessor John French. AnArea-wide cardiac networkcommittee has overseen the<strong>in</strong>troduction of protectednetwork beds at LiverpoolHospital and same-day transferfollow<strong>in</strong>g angioplasty. Thegroup has implementeduniform chest pa<strong>in</strong> protocols<strong>in</strong> all hospitals and the<strong>in</strong>troduction of 24-hourprimary angioplasty for acuteSTEMI at Liverpool Hospital.Morbidity and mortalitymeet<strong>in</strong>gs are active <strong>in</strong> allcardiology units, and all unitsare <strong>in</strong>volved <strong>in</strong> education.There is an <strong>in</strong>creas<strong>in</strong>g demandfor acute cardiology services,and the acquisition andma<strong>in</strong>tenance of cl<strong>in</strong>ical<strong>in</strong>frastructure is an importantongo<strong>in</strong>g task. Cardiothoracicsurgery is a western zoneservice based at LiverpoolHospital. The unit is staffed byfour cardiac surgeons, with oneprovid<strong>in</strong>g most of the thoracicsurgery care. Elective andemergency adult cardiac andthoracic surgery is provided atLiverpool Hospital, and aconsultative service is offeredto Bankstown, Fairfield,Campbelltown and Bowralhospitals. The unit has closework<strong>in</strong>g l<strong>in</strong>ks with allcardiology departments, theLiverpool anaestheticdepartment and the Liverpool<strong>in</strong>tensive care department.The thoracic surgerycomponent has close work<strong>in</strong>gl<strong>in</strong>ks with respiratory medic<strong>in</strong>eand oncology services <strong>in</strong> allArea hospitals. Consultants are<strong>in</strong>volved <strong>in</strong> teach<strong>in</strong>g activitiesfor medical students, juniordoctors and generalpractitioners. Cardiothoracicsurgery research activities areheaded by Dr Rebecca Dignan,and there is considerableliaison with the CardiologyResearch Unit. Dr Dignan isalso the supervisor of tra<strong>in</strong><strong>in</strong>g(RACS). Planscont<strong>in</strong>ue for the development ofCampbelltown as a majorcentre for gastroenterology andcolorectal surgery. A MidwiferyGroup Practice has beenimplemented at Camden and isplanned for Campbelltown.Several of the Henderson-Smart Report (2004)recommendations have beenimplemented, <strong>in</strong>clud<strong>in</strong>g a staffspecialist appo<strong>in</strong>tment to theFeto Maternal Unit. A seniorlecturer has been recruitedto enhance reproductiveendocr<strong>in</strong>e services and provideleadership <strong>in</strong> research. Inaddition, a director of obstetricsand gynaecology has beenappo<strong>in</strong>ted at Fairfield andrecruitment is underway for astaff specialist vacancy atCampbelltown. Cl<strong>in</strong>ical Governance UnitThe Cl<strong>in</strong>ical Governance Unit (CGU) plays a pivotalrole <strong>in</strong> the implementation of the Patient Safety andCl<strong>in</strong>ical Quality Program by provid<strong>in</strong>g support,education and direction for improv<strong>in</strong>g safety andquality of care.Incident Information ManagementThe Incident Information Management System (IIMS) wasimplemented <strong>in</strong> SSWAHS <strong>in</strong> early <strong>2005</strong> as an <strong>in</strong>itiative under the<strong>NSW</strong> Patient Safety and Cl<strong>in</strong>ical Quality Program.The aim of the system is to support health care professionals <strong>in</strong>identify<strong>in</strong>g, track<strong>in</strong>g and manag<strong>in</strong>g cl<strong>in</strong>ical, workforce andcorporate <strong>in</strong>cident <strong>in</strong>formation.More than 1,500 managers are official users of IIMS and nearly1,500 <strong>in</strong>cidents are reported each month. Dur<strong>in</strong>g this first year ofimplementation, the focus of IIMS was to encourage notificationwith<strong>in</strong> the system.While the analysis of serious <strong>in</strong>cidents rema<strong>in</strong>s a priority, theseaccount for less than 1 per cent of all <strong>in</strong>cidents reported.A half-day education program (Cl<strong>in</strong>ical Incident ManagementSkills) was <strong>in</strong>troduced <strong>in</strong> late <strong>2005</strong> to assist <strong>in</strong> management ofless serious <strong>in</strong>cidents. Eight workshops have been run by CGUstaff (assisted by other cl<strong>in</strong>ical staff), with 334 participants.The workshops have been well received and will be a cont<strong>in</strong>u<strong>in</strong>gpart of the CGU education program.Professional Practice Unit andpatient compla<strong>in</strong>tsThe Professional Practice Unit (PPU) is responsible for oversee<strong>in</strong>gthe management of serious compla<strong>in</strong>ts with<strong>in</strong> SSWAHS. The PPUmanager is the designated senior compla<strong>in</strong>ts officer and isavailable to members of the public and staff as required anddirectly responsible for ensur<strong>in</strong>g that appropriate action is takento resolve serious patient compla<strong>in</strong>ts.The PPU currently consists of a manager and an Area patientcompla<strong>in</strong>ts officer, both of whom have a comb<strong>in</strong>ation of cl<strong>in</strong>ical,legal and mediation skills. The PPU meets monthly with patientliaison officers from across the Area to develop a standard bestpractice approach to compla<strong>in</strong>t handl<strong>in</strong>g. The PPU also worksclosely with the Human Resource and Development unit tofacilitate education around professional practice, codes ofconduct and compla<strong>in</strong>t handl<strong>in</strong>g.A Patient Rights and Responsibilities pamphlet and Patient RightsCharter have been developed for the Area; they are available <strong>in</strong>the major language categories and are readilyvisible to patients <strong>in</strong> each facility.An education program designed for the frontl<strong>in</strong>e handl<strong>in</strong>g ofcompla<strong>in</strong>ts has been developed as a two-hour workshop and isbe<strong>in</strong>g progressed across SSWAHS.Root cause analysis –patient safety programRoot Cause Analysis (RCA) is used toreview and analyse an <strong>in</strong>cident toidentify the factors that contributed toit, and to recommend actions toprevent a similar occurrence. SSWAHSundertook 86 RCAs <strong>in</strong>to serious cl<strong>in</strong>ical<strong>in</strong>cidents, and many improvements topatient care have been implementedas a result.Staff must be tra<strong>in</strong>ed <strong>in</strong> the RCAprocess before they can lead an RCA<strong>in</strong>vestigation. Four RCA tra<strong>in</strong><strong>in</strong>gcourses were run dur<strong>in</strong>g <strong>2005</strong>/<strong>06</strong>,with 97 participants. Corporate ServicesNon-cl<strong>in</strong>ical support servicesacross SSWAHS are managed byCorporate Services and <strong>in</strong>cluderisk management, occupationalhealth and safety (OHS) andrehabilitation, procurement andtender<strong>in</strong>g, contract management,oversee<strong>in</strong>g complex <strong>in</strong>vestigations,and adm<strong>in</strong>istrative and legalservices.AmalgamationAmalgamations of supply services, humanresources, payroll, the <strong>in</strong>formationsystems division, capital works, pathology,transport and fleet management havebeen f<strong>in</strong>alised, and amalgamation plansfor f<strong>in</strong>ance, food services, eng<strong>in</strong>eer<strong>in</strong>gservices and OHS services havesubstantially progressed. Sav<strong>in</strong>gs from therestructur<strong>in</strong>g of corporate services havebeen returned to fund cl<strong>in</strong>ical services.SSWAHS has actively participated <strong>in</strong>committees and work<strong>in</strong>g partiesimplement<strong>in</strong>g the <strong>NSW</strong> <strong>Health</strong> sharedcorporate services program.F<strong>in</strong>ancial management has beensimplified with the creation of one generalledger. Facilitiesmonitor performance through thecollection of <strong>in</strong>dicators for cl<strong>in</strong>ical work,sharps waste, general waste, recycl<strong>in</strong>g andOHS <strong>in</strong>cidents related to wastemanagement.Energy managementSSWAHS conducted an audit of waterconsumption at all major sites andjo<strong>in</strong>ed <strong>Sydney</strong> Water <strong>in</strong> their Every DropCounts bus<strong>in</strong>ess program. Thedevelopment and ma<strong>in</strong>tenance ofconsistent, Area-wide f<strong>in</strong>ancial andaccount<strong>in</strong>g policies and procedures isvital for ensur<strong>in</strong>g and enhanc<strong>in</strong>g qualitycontrol <strong>in</strong> f<strong>in</strong>ancials and operations.The implementation of the OracleF<strong>in</strong>ancials Version 11i from version 10.7was successfully rolled out <strong>in</strong> September<strong>2005</strong>, immediately allow<strong>in</strong>g SSWAHS tooperate on a s<strong>in</strong>gle system with an<strong>in</strong>tegrated general ledger and a s<strong>in</strong>gleuniform chart of accounts. Internal AuditDepartmentInternal audit<strong>in</strong>g is an <strong>in</strong>dependentand objective assurance and consult<strong>in</strong>gactivity designed to add value andimprove the operations of SSWAHSby br<strong>in</strong>g<strong>in</strong>g a systematic, discipl<strong>in</strong>edapproach to risk management, controland governance processes.The Internal Audit Departmentprovides an <strong>in</strong>dependent review ofhospital systems, operations, activities,policies and procedures, and, wherewarranted, recommends cost-effectivecontrols and solutions.The audit units at RPA and Liverpoolhospitals have been successfullymerged, and a consolidated audit planhas been put <strong>in</strong> place for the entireArea.Development of the consolidated auditplan allowed identification of auditableareas of sufficient risk which had notpreviously been subjected to an auditreview. In addition, new facilitieswithout established networks haveneeded to develop <strong>in</strong>ternal and externalpartners to help raise the profile of healthparticipation by the community.One key project has been the review of theCommunity Participation Framework thatwill now encompass the Area. The deliveryof orientation and tra<strong>in</strong><strong>in</strong>g to ensure staffand community representatives areclear about their respective roles andresponsibilities has also been amajor focus.Patient feedbackFormal structures are <strong>in</strong> place at SSWAHSto facilitate community participation.All facilities have a Patient Liaison Officerfor patients and carers to approach withsuggestions, compla<strong>in</strong>ts and compliments.Compla<strong>in</strong>t statistics are collected andregularly reported to the Cl<strong>in</strong>ical QualityCouncil and <strong>NSW</strong> <strong>Health</strong> for <strong>in</strong>clusion <strong>in</strong>the statewide database.Facilities also use surveys and suggestionboxes for patient feedback. This allows<strong>in</strong>formation to be exchanged freely andhelps to ma<strong>in</strong>ta<strong>in</strong> a constructiverelationship between staff and patients.Ongo<strong>in</strong>g consumer forums featur<strong>in</strong>g keyspeakers are held regularly <strong>in</strong> order toimprove dialogue between SSWAHS andthe community we serve. F<strong>in</strong>ancial SummaryThe <strong>Sydney</strong> South West Area <strong>Health</strong>Service (amalgamat<strong>in</strong>g the formerCentral <strong>Sydney</strong> Area <strong>Health</strong> Service andthe former South Western <strong>Sydney</strong> Area<strong>Health</strong> Service) was established on1 January <strong>2005</strong> and the audited f<strong>in</strong>ancialstatements cover the 12 monthsended 30 June 20<strong>06</strong>.We spent $2.130 billion provid<strong>in</strong>g healthcare to the people of <strong>NSW</strong>. We also spent$66 million on our build<strong>in</strong>g program.Our money was mostly spent on people,$1.297 billion on our staff and $74 millionon visit<strong>in</strong>g doctors. Goods and servicescost $567 million, <strong>in</strong>clud<strong>in</strong>g drugs,laboratory chemicals, X-ray film, sutures,needles and services provided to ourresidents by other <strong>NSW</strong> health services($171 million).ExpenditureWe spent $60 million on the ma<strong>in</strong>tenanceof build<strong>in</strong>gs and the replacement ofequipment, $27 million <strong>in</strong> grants andpayments to non-governmentorganisations and $26 million to thirdschedule hospitals. Depreciation onbuild<strong>in</strong>gs and equipment accounted for$79 million.Our revenue <strong>in</strong>cluded $99 million frompatient fees, $41 million from grants andcontributions (over $26 million was forthe special purposes and trust fundaccounts), $74 million from the sale ofother goods and services (with $14 millionma<strong>in</strong>ly related to the transfer of fundwith<strong>in</strong> the special purposes and trust fundwas not available for use for generalexpenditure), $12 million from<strong>in</strong>vestments and $11 million fromother revenue.RevenueWe provided health services worth$171 million to residents of other <strong>NSW</strong>health areas (<strong>in</strong>flows) and our residentsreceived care worth $174 million <strong>in</strong> otherarea health services (outflows).
