<strong>ACSAS</strong> <strong>CONSENT</strong> <strong>FORM</strong> – Please Read<strong>Algonquin</strong> <strong>College</strong> Student Advising SystemPlease read the following information, sign on the reverse side, and return to your advisor.<strong>ACSAS</strong> has been developed so that advisors can make notes about advising sessions and organizereferrals and appointments. The information helps the advisor remember what you have discussed ina general way, and it is not shared outside the advising team for your level of your program.In your program, advisors work as a team which may include other faculty advisors, the programcoordinator and the Student Success Specialist for your Faculty or School. Information about youracademic progress may be shared among some members of the team.
