Benefac<strong>to</strong>r Spring 2008MMarshall leads the way in West Virginia inintroducing electronic health recordsWith the incredible explosion <strong>of</strong>technological advances in medicalcare in recent decades, a weak linkin the chain <strong>of</strong> medical care became increasinglyapparent: doc<strong>to</strong>rs’ centuries-old reliance on paperpatient records.Over the past 3 years, doc<strong>to</strong>rs and othermedical staff at Marshall’s medical school havebeen engaged in a quiet technological revolution,implementing electronic health records thatmake patient care safer, more comprehensive andmore efficient.After 2 years <strong>of</strong> research and preparation,Marshall doc<strong>to</strong>rs, during the fall <strong>of</strong> 2007, began“going live” with electronic records, one or twodepartments at a time. As <strong>of</strong> the end <strong>of</strong> January,we had nearly 100 medical practitioners and 100resident physicians using the electronic records;by July 2008 the remaining 150 or so physicianswill be fully on board, making Marshall’sphysician group the largest in West Virginia <strong>to</strong>have a virtually paperless patient record andelectronic prescribing system in place.The undertaking was immense. Assuring thesecurity <strong>of</strong> records, for example, was an absoluterequirement. We’re pleased <strong>to</strong> say our patients’records are protected by security measuresstronger than the ones used by banks online.Thousands <strong>of</strong> hours went in<strong>to</strong> selecting the bestrecord system for our practice, cus<strong>to</strong>mizing it <strong>to</strong>meet the needs <strong>of</strong> each specialty, and trainingdoc<strong>to</strong>rs on how <strong>to</strong> use the program effectively. Inaddition, linkages needed <strong>to</strong> be developed <strong>to</strong> thesystems <strong>of</strong> our partner hospitals.Why have we invested this tremendousamount <strong>of</strong> time, plus the $4.5 million <strong>to</strong> $5million required for the changeover? Quitesimply, electronic records are the future <strong>of</strong>documentation in medicine. There is a strongnational move <strong>to</strong>ward getting them in place, andMarshall is proud <strong>to</strong> be a leader in implementingthem in West Virginia.By Dr. Gretchen E. Meadows, Class <strong>of</strong> 1998, from theDepartment <strong>of</strong> Internal <strong>Medicine</strong>, juggles patientfiles before the transfer <strong>to</strong> EHR.In fact, in conjunction with Marshall’stransition <strong>to</strong> electronic records, our Robert C.Byrd Center for Rural Health is <strong>of</strong>fering all WestVirginia doc<strong>to</strong>rs free use <strong>of</strong> electronic prescribings<strong>of</strong>tware through a partnership in the NationalePrescribing Patient Safety Initiative.E-prescribing is one <strong>of</strong> the best steps <strong>to</strong>improve patient safety.Since implementing it in our ownpractice, we have written more than 12,000prescriptions electronically. For each <strong>of</strong> those12,000 prescriptions, the system gave physiciansproactive access <strong>to</strong> any warnings about allergiesor other situations in which the drug shouldnot be used. It gave them an effective way <strong>to</strong>avoid adverse drug interactions or duplicatinga medication already prescribed by anotherphysician. E-prescribing is easier for patients,<strong>to</strong>o, since the orders go directly <strong>to</strong> the pharmacy,which can contact the doc<strong>to</strong>r when refills areneeded.Electronic health records <strong>of</strong>fer many otherbenefits for patients and doc<strong>to</strong>rs as well. Forexample:* making it possible for patients <strong>to</strong> haveone comprehensive record that all theirphysicians can see


<strong>School</strong> <strong>of</strong> <strong>Medicine</strong>Happenings* reducing unnecessary testing* providing doc<strong>to</strong>rs remarkably fast access<strong>to</strong> results from tests at local hospitals -- insome cases, doc<strong>to</strong>rs have the results lessthan an hour after writing the orders* giving doc<strong>to</strong>rs secure, instant access <strong>to</strong>patient information when they are takingcalls after hours or covering for fellowphysiciansWith these advantages, it’s easy <strong>to</strong> see whyin a national survey, more than half <strong>of</strong> the peoplesurveyed said that, other fac<strong>to</strong>rs being equal,they would choose a physician who was usingelectronic health records.Looking <strong>to</strong> the future, we hope <strong>to</strong> implementthe necessary training and security steps needed<strong>to</strong> allow local emergency room physicians<strong>to</strong> access patients’ records, providing instantinformation when every minute counts. Wealso plan <strong>to</strong> implement a module that allowspatients <strong>to</strong> interact with our practice online --scheduling appointments, refilling prescriptions,communicating securely with doc<strong>to</strong>rs and nurses,and checking on insurance payments or payingbills.Getting the electronic health recordssystem operational has not been a bed <strong>of</strong> roses,by any means. Despite all the planning, thewalk-throughs and the dry runs, there havebeen glitches that we are working <strong>to</strong> address.In addition <strong>to</strong> a lot <strong>of</strong> positive feedback, therehas been negative feedback as well. Our surveysshow, fortunately, that satisfaction is increasingas physicians become more experienced with thesystem.One <strong>of</strong> our physicians, Dr. Larry Dial, SOMClass <strong>of</strong> 1999, put it well when he wrote a yearago, “Our current system delivers a high level <strong>of</strong>service with limited resources; but with expandingwork loads, teaching and documentationrequirements, the current system cannot sustainitself. Implementation <strong>of</strong> Touchworks will bechallenging and tiring initially, however, withcontinued utilization <strong>of</strong> the system the rewardswill be exponentially returned.”As we near the point <strong>of</strong> having an essentiallypaperless system, it is my distinct opinion thatthe diligent work <strong>of</strong> our physicians and staff inembracing this very complex process will makeMarshall a model for other academic practices aswe all move forward in the health informationtechnology world.Congratulations <strong>to</strong> Dr. MufsonThe <strong>School</strong> <strong>of</strong> <strong>Medicine</strong> AlumniAssociation board <strong>of</strong> direc<strong>to</strong>rsunanimously voted <strong>to</strong> honor Dr.Maurice A. The groupalso has the only Pentacam corneal scannerin southern West Virginia. The Pentacamuses a rotating camera <strong>to</strong> create a 3-D image<strong>of</strong> the cornea so that surgeons have the mostprecise details available when performingcataract or refractive surgery.Dr. Parveen Nagra, a fellowship-trainedcorneal specialist who was trained at WillsEye Hospital in Philadelphia, will be joiningthe staff in June. <strong>School</strong> <strong>of</strong> <strong>Medicine</strong>Match DayMMarshall medicalgraduates enteringpediatrics,OB at more thantwice national rateMarshall University’s graduatingmedical students learned where theywill be spending the next 3 <strong>to</strong> 5 years<strong>of</strong> their medical training, and the results showedsignificant diversity.The graduates are entering pediatrics andobstetrics/gynecology at more than twice thenational rate, and are entering family medicineMatch day results clearly brought smiles <strong>to</strong> the faces <strong>of</strong>(L <strong>to</strong> R) Niveditha Vilasagar, Kate Williams, Noeet Elitsurand Craig Seaman (back left) and Dwight Saulleand surgery at more than 1.5 times the nationalaverage. They also are above the national averagein the percentage entering emergency medicine.In all, 42 seniors learned their residencyplacements at noon on March 20 when theyand their counterparts nationwide were giventhe envelopes containing their residencyselections. Nearly 65 percent are entering thefields defined as primary care in West Virginia- family medicine, internal medicine, obstetrics/gynecology and pediatrics - 25 percent above thenational average.Four students also learned that they wereaccepted for specialty programs that will beginnext year in anesthesiology, diagnostic radiologyand neurological surgery.More than 40 percent <strong>of</strong> the seniors will bestaying in West Virginia. Others will be spreadfrom coast <strong>to</strong> coast at programs including Bos<strong>to</strong>nChildren’s Hospital, UCLA, Duke University,the University <strong>of</strong> Virginia, and the Children’sNational Medical Center in Washing<strong>to</strong>n, D.C.Amirah Shareef and Erica Shaverare pleased with their residenciesat the Carolinas Medical Centerfor pediatrics and emergencymedicine at West Virginia University,respectively

Benefac<strong>to</strong>r Spring 2008OOrthopaedic surgeryresidency trainingprogram approvedat MarshallMarshall University has become one <strong>of</strong>just two United States schools since1995 <strong>to</strong> have a first-time orthopaedicsurgery residency training program approved,West Virginia Governor Joe Manchin IIIannounced Wednesday, August 15, 2007, inCharles<strong>to</strong>n.The new program will play an essential rolein meeting the health needs <strong>of</strong> West Virginians,Manchin said.“Here in West Virginia, we have the nation’ssecond-oldest population and more than ourshare <strong>of</strong> high-risk occupations, but our state lagsfar behind the rest <strong>of</strong> the nation in the availability<strong>of</strong> orthopaedic surgeons,” he said. “As a result,our people sometimes have <strong>to</strong> travel <strong>to</strong>o faror wait <strong>to</strong>o long <strong>to</strong> get the services they need.Marshall’s new program will bring us importantnew resources, and it should become a pipelinethat helps supply doc<strong>to</strong>rs who can expand <strong>to</strong>pnotchorthopedic services <strong>to</strong> underserved areas <strong>of</strong>the state.”Manchin cited a 2004 national study by theAmerican Academy <strong>of</strong> Orthopaedic Surgeonsthat showed only one state – Michigan – had alower number <strong>of</strong> orthopaedic surgeons for each100,000 people. West Virginia’s rate was 4.5orthopaedists per 100,000 people, compared <strong>to</strong>the national rate <strong>of</strong> 6.2.Marshall’s program will double the number<strong>of</strong> orthopaedic surgeons who can be trained inWest Virginia, bringing the <strong>to</strong>tal <strong>to</strong> 30.MU President Stephen J. One way we may not thinkabout is when a successful physicianlooks back at his medical school days and thinks<strong>of</strong> the financial burden some students accrueduring their studies.Kevin S. Both Smiths are from Logan.Dr. Smith has more than 20 years <strong>of</strong>experience working in the field <strong>of</strong> painmanagement. Choosing this specialty was theresult <strong>of</strong> a life changing experience. “While I wasdoing my residency at the University <strong>of</strong> Louisville,a school bus was hit by a drunk driver,” Smithsaid. “I treated the young children and saw theirpain. I continued <strong>to</strong> see patients with pain thatDr. Smith as amedical studentwasn’t addressed, whodid not know therewere treatments thatcan permanently movethem away from painmedication. This field alsogives me more controlover my pr<strong>of</strong>essional lifeand is more rewarding.With this field you get <strong>to</strong>know your patients morethan when doing anesthesia.”Dr. Smith, whose practice is Integrated PainSpecialists in San Diego, Calif., is a Diplomate<strong>of</strong> the American Board <strong>of</strong> Anesthesiology, with aSubspecialty Certification in Pain <strong>Medicine</strong>. DrSmith is also a Qualified Medical Evalua<strong>to</strong>r withthe Department <strong>of</strong> Industrial Relations.Tammy Smith worked formerly as a nationalbuyer for Sears. Kenneth WolfeWilburn and Mary “Burkie” Cook White know the importance <strong>of</strong> having good medicalpr<strong>of</strong>essionals in the community. Wilburn grew up near Holden Hospital in Holden,W.Va., and knew many doc<strong>to</strong>rs in his neighborhood. Their son, Mark, is a graduate <strong>of</strong>West Virginia University <strong>School</strong> <strong>of</strong> <strong>Medicine</strong> and has a family practice near Fayetteville, W.Va.Their daughter, Carole, is a surgical nurse in North Carolina.“I have always known the value <strong>of</strong> medicine and the need for good doc<strong>to</strong>rs and nurses,”Wilburn said. “I felt the Marshall University <strong>School</strong> <strong>of</strong> <strong>Medicine</strong> was a good place <strong>to</strong> helpout.”As a retired accountant for Southern Public Service Company in Salem, Va., Wilburnhas been financially comfortable since he retired. Phillips, Class <strong>of</strong> 1987, after almost 20years in emergency medicine, plans <strong>to</strong> retire fromactive practice and open RoxboroMedAccess,the first Urgicare Center in Roxboro, N.C. Thepractice, which will open in late summer, willfocus on acute care and minor emergencies,but will also have a component <strong>of</strong> occupationalmedicine as well as primary care. MedAccess willbe the focus practice in a small medical park heis developing.P. Mitchell Charles, Class<strong>of</strong> 1988, has been recognizedas a “Hero <strong>of</strong> Emergency<strong>Medicine</strong>” by the AmericanCollege <strong>of</strong> EmergencyPhysicians (ACEP). Theaward, which is part <strong>of</strong> ACEP’s40th anniversary, recognizesemergency physicians whohave made significant contributions <strong>to</strong> theirfield, <strong>to</strong> their communities, and <strong>to</strong> their patients.Dr. Charles is an emergency physician at CabellHunting<strong>to</strong>n Hospital and direc<strong>to</strong>r <strong>of</strong> the MS-4Clerkship in Emergency <strong>Medicine</strong>. He is alsoa faculty sponsor <strong>of</strong> the Emergency <strong>Medicine</strong>Interest Group at Marshall’s <strong>School</strong> <strong>of</strong> <strong>Medicine</strong>.He reviews articles for Annals <strong>of</strong> Emergency<strong>Medicine</strong> and AAMC, and currently serves asthe West Virginia ACEP chapter president. Hehas received the American Heart Association’s“Legacy <strong>of</strong> Life” award, the WVACEP “Excellencein Emergency <strong>Medicine</strong>” award and numerousteaching awards. Procop, Class<strong>of</strong> 1992, has returned <strong>to</strong> theCleveland Clinic as chairman<strong>of</strong> the Department <strong>of</strong> ClinicalPathology and direc<strong>to</strong>r <strong>of</strong>Molecular Microbiology,Mycology and Parasi<strong>to</strong>logy.He was there previously inseveral capacities, including associate pr<strong>of</strong>essor<strong>of</strong> pathology at the clinic’s Lerner College <strong>of</strong><strong>Medicine</strong> <strong>of</strong> Case Western Reserve. While in theposition <strong>of</strong> section head at the Cleveland Clinic,he received three Innova<strong>to</strong>r’s Awards and theQuest for Quality Award, and was named <strong>to</strong> theBest Doc<strong>to</strong>rs® in American List, 2005-2006.In addition <strong>to</strong> his medical degree, Procopreceived a master’s degree in biomedicalsciences from Marshall. He went on <strong>to</strong> completehis pathology residency at Duke UniversityMedical Center, followed by a fellowship inmedical microbiology at the Mayo Clinic. Hethen went <strong>to</strong> the University <strong>of</strong> Miami Miller<strong>School</strong> <strong>of</strong> <strong>Medicine</strong> <strong>to</strong> serve as pr<strong>of</strong>essor <strong>of</strong>pathology and direc<strong>to</strong>r <strong>of</strong> clinical microbiologyat Jackson Memorial Hospital. While at Miami,he was awarded the 2007 American Society forMicrobiology BD Award for Research in ClinicalMicrobiology, one <strong>of</strong> the nation’s <strong>to</strong>p awards inthat field.Procop recalls being interested in infectiousdiseases and microbiology as early as the eighthgrade. “Movie day would <strong>of</strong>ten consist <strong>of</strong> filmsconcerning the adventures <strong>of</strong> medical pioneers inthe heart <strong>of</strong> Africa,” he said. “Watching physiciansand nurses detect and identify the causes <strong>of</strong> exoticinfections, and then administering therapeuticsand vaccinations, was life-changing. Pursuingthis interest, I recently went <strong>to</strong> Tegucigalpa,Honduras, <strong>to</strong> lecture and teach local groups aboutmedical mycology. I would say teachers definitelyhave an impact in our formative years.”<strong>Medicine</strong> was not a foregone conclusion,


<strong>School</strong> <strong>of</strong> <strong>Medicine</strong>Class Noteshowever. Procop considered becoming a scienceteacher like the ones who influenced him. It wasa friend in a chemistry class and her husband,a urologist, who got him interested in medicalschool, followed by pathology training. “I <strong>to</strong>oktheir advice, and I’m glad I did,” Procop said. “Asit turns out, I believe the solid medical training Iobtained at Marshall afforded me the foundation<strong>to</strong> concentrate on the study <strong>of</strong> disease at DukeUniversity. Striving <strong>to</strong> understand the entireprocess <strong>of</strong> infection is the only way <strong>of</strong> looking atit, in my opinion.”Procop was attracted <strong>to</strong> Marshall for anumber <strong>of</strong> reasons, including small class size andthe excellent pr<strong>of</strong>essor-<strong>to</strong>-student ratio. “Thisenables thorough understanding <strong>of</strong> the material,rather than just memorization,” he said. “That’swhat you really want in a physician, someone whotruly understands the disease process, rather thansomeone who was good at memorizing a lot <strong>of</strong>facts. I remember vividly not understanding thefunctioning <strong>of</strong> one <strong>of</strong> the extraocular muscles, soI approached Dr. Mitchell L. Berk after class andhe quickly clarified the process. It is not as easy <strong>to</strong>get that in a school with a large class size.“Three pr<strong>of</strong>essors stand out in my experienceat Marshall” he added. “Doc<strong>to</strong>rs Patrick Brownand Terry Fenger, although not my <strong>of</strong>ficialadvisors, have provided friendship, guidanceand opportunity for which I am forever in debt.Dr. Bryan Larsen was my senior pr<strong>of</strong>essor for mymaster’s degree, and in addition <strong>to</strong> guidance,he afforded me the opportunity <strong>to</strong> work in hislabora<strong>to</strong>ry where I developed many valuableskills. Also, because I was a little different andinterested in some <strong>of</strong> the basic sciences taught inthe first two years, I was pleased <strong>to</strong> find an activelyengaged basic science faculty who afforded methe opportunity <strong>to</strong> explore these areas morethoroughly.”Procop <strong>to</strong>ok time <strong>to</strong> enjoy leisure activitieswhile at Marshall. “There are numerouswonderful memories,” he said. “These includeattending tailgate parties hosted by the medicalschool prior <strong>to</strong> a Marshall football game at theold stadium, participating in Research Day and,again, the numerous interpersonal interactionswith the faculty and fellow students.”Today, his lovely family keeps him busy withhome-related activities. He also enjoys reading,woodworking and sailing.When asked <strong>to</strong> <strong>of</strong>fer one piece <strong>of</strong> advice <strong>to</strong>current and future medical students, he replied,“Follow your heart. Identify the area <strong>of</strong> medicinethat excites you most and dedicate yourself <strong>to</strong>thorough study <strong>of</strong> that area.”Jeffrey T. St. Clair, Class <strong>of</strong>1992, has been selected for theMarshall University AlumniAssociation CommunityAchievement Award for2008. He accepted the honorat the Association’s AlumniBanquet in April. Jeffreyis assistant clinical instruc<strong>to</strong>r in emergencymedicine at New Orleans’ Charity Hospital,part <strong>of</strong> the Louisiana State University HealthSciences Center.Timothy G. Cook, Class <strong>of</strong>1994, is deployed with theU.S. Army 325th CombatSupport Hospital at CampSpeicher, Tikrit, Iraq, as anemergency room provider insupport <strong>of</strong> Operation IraqiFreedom. He was active dutyin the Army as a family physician until 2000,when he resigned and purchased a small, busy,walk-in clinic, Convenient Medical Care, inRutland, Vt. He did this primarily <strong>to</strong> be near hisparents, both in their eighties. Then in January2007, after watching President George Bushmake the case for his surge strategy, he decided <strong>to</strong>sign back up as an Army reserve medical <strong>of</strong>ficer,with the specific request <strong>to</strong> be mobilized <strong>to</strong> Iraq.

Kim, Class <strong>of</strong> 2000, has beenselected along with Bryan D. Springer, Class <strong>of</strong>1999, as the two North American representativeorthopaedic surgeons for the John Insall TravellingFellowship. Four surgeons (two North Americanand two international) are chosen each year <strong>to</strong>participate in the fellowship. This involves travels<strong>to</strong> 12 renowned knee arthroplasty centers inNorth America <strong>to</strong> observe and learn techniquesin knee replacement, and participate in researchand teaching. He completed his residency inorthopaedic surgery at The Mayo Clinic.Enrique M. Sta Ana, Class <strong>of</strong> 2001, is performinggeneral, bariatric and minimally invasive surgeryin New<strong>to</strong>n, Mass. He did his general surgeryresidency at St. Elizabeth’s Medical Centerin Bos<strong>to</strong>n, and a minimally invasive surgeryfellowship at the University <strong>of</strong> Massachusetts.He and his wife, Dr. Lucrecia Trahanino, willsoon be moving <strong>to</strong> Orlando, Fla., for Lucrecia’sfellowship plans.Amy M. Jean, Class <strong>of</strong> 2002, is completing herthird year <strong>of</strong> fellowship in pediatric endocrinologyat Morgan Stanley Children’s Hospital <strong>of</strong> NewYork-Presbyterian in New York City. She plans<strong>to</strong> stay an additional year for patient-orientedclinical research. Amy is studying the metabolicchanges in morbidly obese adolescents undergoingbariatric surgery. She and her husband, Alan,have a new addition <strong>to</strong> their family. Aaron Peterwas born on Oc<strong>to</strong>ber 11, 2007. Andy and Amyare extremely proud and sleep-deprived newparents.David A. Denning II, Class <strong>of</strong> 2002, is ananesthesiologist at Allegheny General Hospitalin Pittsburgh, Pa. Krista L Denning, Class <strong>of</strong>2004, is doing a cy<strong>to</strong>pathology fellowship, alsoat Allegheny General. Their baby girl, ClaireElizabeth, was born on March 1, 2007. Clairejoins their son, David Tucker, who is now 4.Leela Vadrevu Raju, Class <strong>of</strong> 2003, has accepteda fellowship for one year at Baylor College <strong>of</strong><strong>Medicine</strong>. She will be learning more advancedtechniques in cataract surgery and cornealtransplantation, including DSEK, where only theendothelium <strong>of</strong> the cornea is replaced.B. Hunter Boshell, Class <strong>of</strong> 2003, is in his thirdyear <strong>of</strong> his radiology residency at University <strong>of</strong>Cincinnati. He has plans for an interventionalradiology fellowship beginning in July 2009.

She spent a monthin Scottsdale, Ariz., workingat Mayo Hospital. She spentMarch in Africa, starting witha 10-day climb <strong>of</strong> Kilimanjaro,with the remainder <strong>of</strong> the month in Malawiworking at a mission hospital. During the summer,Nicole got back in<strong>to</strong> cycling and visiting familymembers before attending a medical conferencein Hawaii.D.
