CLINICAL REPORTSURGICAL REPAIR OF UPPER EYELID LACERATION IN A HORSE*1 2Mir Aamir Ali and Honnappa T.GEqu<strong>in</strong>e Hospital Bangalore Turf Club ltd, Bangalore, IndiaINTRODUCTIONUpper <strong>eyelid</strong> damage is more significant <strong>in</strong><strong>horse</strong>s because the <strong>upper</strong> lid moves 75% more on theequ<strong>in</strong>e cornea than does the lower <strong>eyelid</strong> (Brooks2002). Eye Lid trauma needs to be corrected earlyand accurately as possible to prevent undesirable lidscarr<strong>in</strong>g and secondary corneal desiccation andulceration.HISTORYAND CLINICAL EXAMINATIONA three-year-old <strong>horse</strong> was presented to equ<strong>in</strong>ehospital with the history <strong>of</strong> fall and <strong>in</strong>jury around theorbit and cl<strong>in</strong>ical exam<strong>in</strong>ation revealed a lacerated <strong>eyelid</strong>with an <strong>in</strong>jury <strong>in</strong> the periorbital area. (Fig 1)layer was sutured a close to the lid marg<strong>in</strong> as possiblewith 3-0 polypropylene. Simultaneously the periorbitalarea was sutured with sta<strong>in</strong>less steel staples.(Fig2)JIVA Vol. 9 Issue 2 August 2011Fig 1. Lacerated <strong>upper</strong> <strong>eyelid</strong>RESULT AND DISCUSSIONHorse was anesthetised us<strong>in</strong>g xyalaz<strong>in</strong>e(@0.5mg/kg body wt i/v (Ilium xyazil-100, Troylabs) and ketam<strong>in</strong>e @2mg/kg body wt i/v (ketamil,Troy labs) and the wound and <strong>eyelid</strong> marg<strong>in</strong> wasaseptically prepared for surgery .The <strong>laceration</strong> wasclosed <strong>in</strong> two layers and the first layer <strong>of</strong> sutures wereplaced midway between conjunctiva, <strong>eyelid</strong> stromaand <strong>eyelid</strong> marg<strong>in</strong> <strong>in</strong> a simple <strong>in</strong>terrupted patternus<strong>in</strong>g 4-0 polyglact<strong>in</strong>- 910. The closure <strong>of</strong> the<strong>laceration</strong> was accomplished with the suturescommenc<strong>in</strong>g from the <strong>eyelid</strong> marg<strong>in</strong>. The sutures were removedon 12th postoperative day and the wound healeduneventfully.Eyelid <strong>laceration</strong>s are frequently encountered<strong>in</strong>juries <strong>in</strong>volv<strong>in</strong>g ma<strong>in</strong>ly the lower <strong>eyelid</strong> and the<strong>in</strong>cidence <strong>of</strong> <strong>upper</strong> <strong>eyelid</strong> <strong>laceration</strong> is not common.Any abnormality <strong>in</strong> the function <strong>of</strong> the <strong>upper</strong> and lower<strong>eyelid</strong>s predisposes the globe to secondary diseases dueto loss <strong>of</strong> the lipid part <strong>of</strong> the tear film, unevendistribution <strong>of</strong> tear and excessive exposure <strong>of</strong> the globeto UV light, which is provided by the <strong>eyelid</strong>s (Brooks1999,2002,2007,2008; Gilger 2005).
