OFC joint(stone) marker = 10 no#sCheck ListName of Contractor :Sr. No. Required Documents Submitted (Yes/ No) Remarks1 Single work order including SOR#s of value not less than 26 Lacs in Last 5 years2 Completion certificate of submitted work order The completion certificate shall be consisting of work orderreference, contract value and executed value etc.3 TDS certificate of submitted work order4 EPF & ESI registration certificate5 Turnover duly certified by CA6 All the above documents from Sr No.1-5 is duly attested by gazzted officer/ notary publicSEAL & SIGNATURE OF CONTRACTORBid Evaluation Criterion:BID EVALUATION CRITERIA (BEC)The following shall be bid evaluation criteria#1- The bidder shall have executed at least one single job of the value not less than 26 (Twenty Six) Lacs for OFCmaintenance job including OFC splicing/jointing , Hop testing in any of the last 05 preceding years ending with RFQdate .The copy of work order, it's completion certificate and TDS shall also be submitted.2- The bidder shall submit the latest valid permanent EPF code number document & ESI registration certificate.3- The annual turn over of the bidder shall be minimum 35 Lacs in any of the last 5 preceding financial years endingwith March 05. This document shall be submitted duly certified by CA.All the above documents shall be submitted duly attested by gazztted officer /notary public. Non attestation shall leadto rejection of bids.Bids complete in all respects should reach office of Incharge (C&P), GAIL (India) Ltd., at the above addresson or before 14.00 hrs of the scheduled date.
