Almost allof the time, the ADA is an advocate for me and for you.That is one reason I am a member and encourage othersof our profession to join and be involved.Why is it I feel the ADA has dropped the ball involvingthe Alaskan <strong>Dental</strong> Health Aide Therapists (DHATs)?You may recall that the state of Alaska, because of theirunique expanse and very remote villages in some of theharshest environment, has a dental access problem.There are not enough dentists to meet the needs of thesecitizens. The <strong>LDA</strong> highlighted this issue in our Winter 2006issue of the <strong>LDA</strong> Journal.So the DHATs were created/authorized by the IndianHealth Service to meet these needs in the Alaskan outbackareas. These DHATs have minimal training and yet areperforming dentistry that took us years of training andboard certification to be able and licensed to carry out.Our ADA (including the Alaska <strong>Dental</strong> Society) fileda lawsuit to stop DHATs from practicing invasive, nonreversibledentistry. This seemed the only way to protectthe Alaska natives from receiving substandard dentaltreatment. Now, the decision has been made to “mediate”with the Alaskan Native Tribal Health Consortium(ANTHC). Sounds like our ADA has decided to backoff and let the ANTHC have its way. Not a good idea.Why would I want to tell the good state of Alaska howto practice dentistry? It is because only dentists can practicedentistry in the United States, and the last time I looked,Alaska was a state just like <strong>Louisiana</strong>.Alaska does have a unique environment and thereis an access problem to dentistry in remote areas. I alsobelieve there are unique areas with access problems inArizona, North and South Dakota, Montana, Wyoming,even parts of South <strong>Louisiana</strong>, especially with our recentdevastating storms, as well as many other areas of ourcountry.What will stop DHATs from practicing in the uniqueareas of these states as well as perhaps within the stateof Wal-Mart?I believe the original lawsuit should not be abandonedbut pressed on to stop this from continuing. Our formerADA chief counsel wanted to fight. Increases in membership figures translate intoavailable dues dollars – dues dollars that can be used tocontinue improving membership benefits. Some of themore notable benefits include: our new and improvedWeb site, continuing education, insurance coverage(PDP), the <strong>LDA</strong> has been able to provide many benefitsto its members that are not paid for from dues.Through LDS, the <strong>LDA</strong> endorses many high-qualityproducts and services that provide money-savingopportunities to our members, as well as revenue to theassociation every time a member uses them. Not only has the site helpedus to fulfill many of our strategic planning goals of theassociation, but through the financial support of LDS,we were able to unveil a site that moves us forwardtechnologically, but also gives us a basis for so manyother options that you’ll see in the future, includingresources that allow us to survey our members to findout more about your needs and wants regardingorganized dentistry.And, I would like to give special thanks to all of youwho supported the first-ever <strong>LDA</strong> Annual Session andNew Orleans <strong>Dental</strong> Conference. Preliminary numberslook good as over 2,600 people registered for theevent, including 771 dentists (528 are <strong>LDA</strong> members),681 hygienists, 342 business assistants, 502 chairsideassistants, 13 lab techs, 78 guests and 268 dental spouses.Attendees were able to browse the exhibit hall to seethe latest and greatest equipment and services availableto dentists from over 125 vendors, and, the meetingwas an incredible deal at $300 each for <strong>LDA</strong> members!Special thanks should be given to the <strong>LDA</strong> volunteersand committees that have worked with me as we passeda budget for this year during the <strong>LDA</strong> House of Delegates.I am proud of our delegates for supporting the 2007 NODCby paying certain expenses of the conference, with theadvice of the NODC chairman as to which expenses needto be paid, up to a total of $30,000. <strong>LDA</strong>executive directorWard Blackwell, M.J., Executive Director, <strong>LDA</strong>Ten Year Milestone a Chance to Note BothConstants and ChangeRecently, in a moment or two of calm reflection, it dawnedon me that I recently surpassed 10 years with the <strong>LDA</strong>.As this realization sunk in, I recalled more and morethings that have changed (mostly for the better) in thedecade since I first walked into the association office.For the record, one thing that hasn’t changed much is thenovelty of those moments of calm reflection. If anything,they’ve been rarer than ever since the twin terrors Katrinaand Rita visited. And, since the combined Annual Session/New Orleans <strong>Dental</strong> Conference concluded right as theunparalleled zaniness of a legislative session began, there’sbeen no let up lately.So there’s a lot going on at the <strong>LDA</strong> right now, as thereis most of the time. But that got me wondering how muchof that activity is attributable to new programs, projectsand benefits the <strong>LDA</strong> initiated in the past 10 years? Considerthat in the past ten years:• The <strong>LDA</strong> Web site first came online (July 1, 1997).• The <strong>LDA</strong> Council on the New Dentist, Council on<strong>LDA</strong> Sessions, Medicaid Task Force and <strong>Dental</strong>Access and Prevention Political Action Committee(DAPPAC) were all created.• The <strong>LDA</strong> Fishing Rodeo, Summer EducationConference, Game Day Seminar and LADPAC GolfTournament were all inaugurated.• The <strong>LDA</strong> joined the LSU School of Dentistry’sCommittee of 100 and began sponsoring theLSUSD awards luncheon, “Lunch & Learns” forthe LSUSD ASDA chapter, and the LSUSD-ASDAskeet shoot.• The <strong>LDA</strong> began presenting the New Dentist Awardand attaching a monetary prize to the <strong>LDA</strong> StudentAward.• A three-year statewide public relations campaignwas undertaken and Give Kids a Smile Day began.• The <strong>LDA</strong> Assistance Fund was created to providelow-interest loans to dentists seeking treatment forchemical dependency.• The <strong>LDA</strong> began soliciting sponsorships of <strong>LDA</strong>conferences and events, eventually growing fromnothing to more than $25,000 per year.That’s a fairly impressive, though not comprehensivelist of new initiatives originating in the past 10 years.However, that’s not to say that all other activities inwhich the <strong>LDA</strong> engages have remained static. In actuality,there are many services provided and functions served bythe <strong>LDA</strong> continually over the past decade that have seennoteworthy innovations and improvements. For instance:• The <strong>LDA</strong> Annual Session is now combined withthe New Orleans <strong>Dental</strong> Conference.• The <strong>LDA</strong> Web site has been re-designed with morefeatures, information and members-only content.• The <strong>LDA</strong> C.E. calendar went from listing a fewevents in the back of the Journal to includingvirtually all major C.E. events in the state and isnow also posted in an interactive format on the<strong>LDA</strong> Web site.• The list of companies entering into agreementswith our for-profit subsidiary, LDS, and endorsedby the <strong>LDA</strong> grew to include Acadian Ambulance,Amalgaway, Bank of America Practice Solutions,Chase Cabinetmakers, DHL Express, D-MMEX,the Institute of Medical Emergency Preparedness,Liberty Mutual Insurance, Office Depot, Paychex,Select Choice Student Loans, Solmetex and RobertEllis and Associates/Fortress Insurance.• The <strong>LDA</strong> Relief Fund created a disaster grantprogram to provide widespread assistanceimmediately after the hurricanes of 2005.• The <strong>LDA</strong>’s computers have been networked, mostboard communications have become electronic andwe began managing our grassroots lobbying thougha web-based system called VoterVOICE thatfacilitates instant advocacy for virtually all <strong>LDA</strong>members.• The <strong>LDA</strong> Strategic Plan is now reviewed annually.14 <strong>LDA</strong> Journal


<strong>LDA</strong>Eric J. Hovland, D.D.S., M.Ed., M.B.A.Dean, LSU School of DentistryLSU school of dentistryGoing Home!It is official—we are returning to New Orleans latesummer or early fall. The past two years have been filledwith challenges and we are anxious to return home. Ournewly renovated facility will be even finer than before thehurricane! The influx of goodwill from individuals andcorporations, coupled with available public disasterfunds, has allowed LSUSD to take its operations to thenext level. Even the school’s two mainclassroom auditoriums—A & B—areundergoing a facelift with new chairs,tables and Internet connections. I amsure that everyone will enjoy the newfacilities.I am pleased to report that ourgraduation activities progressed asplanned at the Ernest Morial ConventionCenter in New Orleans on May 19. Thecelebration was especially rewarding asthe 2007 class had a difficult transitionas a result of Katrina. Also, the hurricaneaftermath has not affected our recruitingefforts—once again, we have admitted oneof our strongest freshman classes ever!Although we have not hearddefinitively, the chances of retaining theBaton Rouge campus look promising. Weanticipate the campus will require approximately $1.4million annually to operate and it is expected that about$700,000 in annual clinic income can be generated. Studentand resident rotations, a distance-learning dental hygieneprogram, and a pediatric clinic program are the top threepriorities for the campus.All students and residents are expected to beginthe school year in New Orleans. If there are unforeseenissues related to renovations, classes and clinics will beginbriefly in Baton Rouge and continue in New Orleans assoon as possible. However, I am pleased to report thateverything is on track for our return in August. Acquiringpatients in New Orleans seems to be of no concern.As of June, nearly 1,000 people have requested dentalscreenings.As mentioned in the last <strong>LDA</strong> Journal, a well-representedgroup of faculty, students and staff have developed a draftstrategic plan that will be reviewed by our faculty ata planning retreat in June. Faculty shortages, lack ofdiversity, lagging local and state governmentsupport, changing technology, and escalatingcosts are challenges that face all dental schoolsnationally and LSUSD is no exception. Facultyrecruitment and retention continues to be aprimary focus for the school as excellent,committed faculty directly correlates withour fourfold mission of success in education,research, patient care and communityoutreach.As always, I sincerely thank the entiredental community for their collective support.Please keep the LSU School of Dentistry inmind as you consider your philanthropicpriorities each year. The livin’ is easy, fish are jumpin’,and yo moma’s good lookin’ (well…) Ah yes,summertime! It is once again time to put down the pickand pick up the pole, head for the beach or the bayouand forget for a while that you even have a dental office.That said, your membership, recruitment and retentioncommittee is hard at work finding ways to recruit and“recreate.”First up, it is now time for our annual pilgrimageto Grand Isle. What started a few years ago as a smallget together for an adventurous few has exploded intoa major closely contested event. Over 100 fisherman,workers and volunteers, sponsors and friends enjoyedlots of fun, food, parties, prizes, beer, a whole lot of lies,great fishing and (did I mention) beer, all rolled up intoone great weekend. If you want continuing education,you may be able to convince Drs. Smith, St. Pierre andDunaway to explain the intricacies of the relationshipof tidal movement to fish-fry batter, but other than thatforget it, this weekend is just for fun.And, did I mention competition? The last few yearshave been dominated somewhat by Jerry and the Acadianaboys and they promise to keep up the tradition. Therehave been rumors though that the other components aregearing up to take them down. It’s all good, it’s all fun,and if you have not previously joined in, you need to.So, mark your calendars now and register for this event,Friday and Saturday, July 13 and 14 at Sand Dollar Marinain Grand Isle. You can fish off a pier, fish off the beach,fish from a boat or not at all, as long as the fish you enterdon’t have an expiration date stamped on them! And, forthe first time ever, you can register online for the FishingRodeo, as well as any other <strong>LDA</strong> events, at www.ladental.org.Then, less than two weeks after the Fishing Rodeo isthe <strong>LDA</strong> Summer Education Conference. It is expectedto be another great meeting, including a variety of speakers,great golf, awesome beaches and lots of relaxation. You canget up to 16 C.E. hours while you write off your familyvacation - can’t beat that!Another project I am working on is putting togethera Spanish Course for dental professionals. In the unfortunate event thatsomething happens to you, it is necessary to know howthis income loss will affect your family. How do you puta price on your most valued asset?Determining an estimated amount of necessary lifeinsurance can actually be quite simple. The first step istaking inventory of your current assets and your currentliabilities. Here’s an example:Dr. ProviderAssetsLiabilitiesHouse: $350k Mortgage: $250kCar: $25k Car Loan: $20kInvestment Acct: $50k Student Loan: $120kSavings: $35k Personal Loans: $10kTotal Assets = $460k Total Liabilities = $400k(Assets - Liabilities = Net Worth) Net Worth = $60kMost likely, Dr. Provider’s spouse will not sell the houseand car, greatly decreasing the significance of its fair marketvalue in the life insurance equation. Let us look at thecurrent liabilities in the example: $400k. If you subtractfrom this amount the investment account and savingsassets, you are left with approximately $315,000 of debtupon your death. Then, include an additional $30,000for burial expenses, which gives a new total of $345k.So, Dr. Provider needs at least $345,000 just for hisfamily to break even.The next step in estimating an adequate amount of lifeinsurance is to calculate the income amount necessary tokeep the family at a certain standard of living. Dr. Provider’sfamily needs a minimum after tax total of $6k per monthto maintain their current lifestyle. That adds up to $72kper year (not including inflation).The simple equation for calculating the entire amountof life insurance is to divide the yearly amount of $72kby 4.5% (reasonable rate of return) equaling $1,600,000.Add in the $345,000 of debt to the $1,600,000 to reacha grand total of $1,945,000.Dr. Provider should obtain a life insurance policythat will insure his life for at least $2 million. InLoving Memory:Dentists in the Northwest Honor One of Their OwnOn March 16, 2007, dental colleagues honored thememory of Dr. Donald D. Cope during a dedicationceremony in Shreveport. A granite plaque was placed atthe front of the Donald J. Zadeck, Jr. & Percy A. Sharp IIIMemorial Gardens of the Feist-Weiller Cancer Center inhonor of Dr. Cope.Dr. Cope passed away from prostate cancer in 2006.After his passing, Dr. Cope’s colleague and friend, Dr. L.M.Harrison, Jr., asked fellow dentists from the Northwestcomponent to contribute a donation and the responsewas overwhelming. These dentists donated well over$5,000 to the Feist-Weiller Cancer Center, which is thecancer department and research center at the LSU Healthand Sciences Center in Shreveport.Dr. Harrison expressed that Dr. Cope was a great dentist,a great individual, and always had a smile on his face. Hesaid the plaque was “a permanent memorial that peoplewould see forever.”During the dedication ceremony, the director of theFeist-Weiller Cancer Center, Jonathan Glass, M.D., gave awelcome speech to the guests. The ADA Seal ofAcceptance helps consumers make informed decisionsabout safe and effective products, such as toothpastes,toothbrushes and floss.For a list of oral care products that have received the ADASeal of Acceptance, see www.ada.org/ada/seal/index.asp.ADA Approves New Guidelines Related toDentistry and Infective Endocarditis (IE)For decades, the American Heart <strong>Association</strong> (AHA)recommended that patients with certain heart conditionstake antibiotics shortly before dental treatment. Thiswas done with the belief that antibiotics would preventinfective endocarditis (IE), previously referred to asbacterial endocarditis. The AHA’s latest guidelines werepublished in its scientific journal, Circulation, in April2007 and there is good news: the AHA recommendsthat most of these patients no longer need short-termantibiotics as a preventive measure before their dentaltreatment.The American <strong>Dental</strong> <strong>Association</strong> participated in thedevelopment of the new guidelines and has approvedthose portions relevant to dentistry. The guidelines werealso endorsed by the Infectious Diseases Society of Americaand by the Pediatric Infectious Diseases Society.The guidelines are based on a growing body of scientificevidence that shows the risks of taking preventive antibioticsoutweigh the benefits for most patients. The risks includeadverse reactions to antibiotics that range from mild topotentially severe and, in very rare cases, death. Inappropriateuse of antibiotics can also lead to the development ofdrug-resistant bacteria.Scientists also found no compelling evidence thattaking antibiotics prior to a dental procedure prevents IE inpatients who are at risk of developing a heart infection.Their hearts are already often exposed to bacteria fromthe mouth, which can enter their bloodstream duringbasic daily activities such as brushing or flossing. Thenew guidelines are based on a comprehensive review ofpublished studies that suggests IE is more likely to occuras a result of these everyday activities than from a dentalprocedure.For more information and guidelines, go to the ADAWeb site at www.ada.org.Warning: Hand Washing Scam fromthe “<strong>Louisiana</strong> Healthcare ComplianceCenter”The <strong>Louisiana</strong> Department of Health and Hospitalsis issuing a warning to health care providers about a falseadvisory in circulation which states health care providersare required by state and federal regulations to posthand-washing signs.The source of the advisory has been identified as LaborLaw Posters out of Michigan, which offers to sell the signsto the recipient of the notice at high prices. The companyreferences the “<strong>Louisiana</strong> Healthcare Compliance Center,”which is a fake agency that is being used as a front to deceiveproviders into believing they are required to purchase handwashingsigns.This advisory is considered particularly damaging dueto the expensive nature of the signs and the pretense thatthey are required by both the Centers for Disease Controland state health codes. The advisory also states that ifestablishments do not post these signs, they will be subjectto fines and penalties. Attempts to contact the sourcehave proved difficult due to anonymous P.O. boxes andout-of-state phone numbers.While hand-washing signs are encouraged to promotecleanliness, they are not required. These grants require communityparticipation through a commitment from thecommunity requiring them to purchase fluoride forthe water system, maintain equipment, monitoringand keep up compliance according to the standardsset by the Fluoridation Program.By providing technical support and assistance,the program assists systems in maintaining optimallevels of the fluoride ion. The program monitorsthe fluoridation levels of all water systems that arefluoridating and reports these values to the WaterFluoridation Reporting System within the Centersfor Disease Control and Prevention for public access.The Fluoridation Program notifies dental andhealth care providers of their community waterfluoridation to enable the judicious use offluoridation supplement.Dr. John Taylor of Crowley is a member of theFluoride Advisory Board and helped guide localefforts in Crowley.The LAWCO/Crowley Water system, whichserves over 22,000 people, contained some naturalfluoride, but not optimal levels to prevent toothdecay. Pearce, D.D.S.President, <strong>Louisiana</strong> State Board of DentistryA New Horizon in Clinical LicensingExaminationsThe <strong>Louisiana</strong> State Board of Dentistry has contractedwith the Council of Interstate Testing Agencies(CITA) to begin administering the clinical licensingexaminations for dentistry and dental hygiene in thestate of <strong>Louisiana</strong> beginning in calendar year2008. This is a major step forward for a statethat has maintained its independence forapproximately 130 years. Bob Dylan oncewrote, “The times, they are a-changing” andthose changes are now here.Let me give you a little history of theCouncil of Interstate Testing Agencies. Thistesting agency grew from an earlier organizationcalled the Coalition of Independent Testing Agencies,which was formed back in the 1990s. The originalcoalition was a creation of the independent states thathad been administering their own clinical examinationsin response to the national framework which was beingaltered by the regional licensing examination agencies.Those agencies are the Western Regional ExaminingBoard, Northeast Regional Examining Board, CentralRegional <strong>Dental</strong> Testing Agency, and the SouthernRegional Testing Agency. Furthermore, in the last fewyears, the American <strong>Dental</strong> Examining group has alsoformed. All in all, very few states administer their ownindependent examination and those states are expectedto join regional testing agencies in the near future.Much of the regional concept is designed to allowyounger dentists and hygienists more freedom of mobilitydue to our more mobile society. Many states now acceptall of the regional examinations, as does <strong>Louisiana</strong>.The decision to contract with CITA was made afterboard members had reviewed the examinations of all theregional testing agencies, as well as the other independentstates during the American <strong>Association</strong> of <strong>Dental</strong> Examinersmeetings over the last several years. Though there are slightdifferences in the examinations, all test on the same corecurricula, and are extremely similar in content. There aresome differences in scoring methods, but all seem toadequately protect the public from the incompetentpractitioner.CITA has been working with the LSUSD over the pastyear in order to give time to the LSU seniors tounderstand the new examining process whichwill be very patient friendly, although it willbe a different scenario from what ourseniors have been used to in the past. TheCurriculum Integrated Format (CIF) allowsjuniors in their second semester to begintaking the licensing examination on themannequin portion of the examination. Thiswill take a lot of pressure off of them when theytake the clinical examination that is patient based.Presently, the states of Alabama, <strong>Louisiana</strong>, Mississippi,North Carolina, and Puerto Rico have CITA administertheir dental and dental hygiene examinations.The aforementioned states and Massachusetts, Maine,Missouri, New Hampshire, North Dakota, Oregon,Washington, Virginia, and West Virginia also accept theresults of the CITA examination. However, Virginia willnot begin accepting the results until after September 1,2007. More states are in discussions with CITA at thepresent time.The examinations in each of the states will still beadministered by some of the board members from thosestates. However, in order to assure uniformity, examinersfrom other states will also participate in the examinationsgiven in each state. This means that <strong>Louisiana</strong> boardmembers will be testing in North Carolina, and the otherstates of CITA as well. We will also utilize the services ofdentists from other states in the <strong>Louisiana</strong> examinations.We encourage anyone who wishes to participate as anexaminer with CITA to contact either our board office, orCITA directly. Farrar, R.D.H., M.S.H.C.M.Immediate Past President, <strong>Louisiana</strong> <strong>Dental</strong> Hygienists’ <strong>Association</strong>What’s Happening in the World of the<strong>Dental</strong> Hygienist?Many new and exciting things are happening in theworld of hygiene. The American <strong>Dental</strong> Hygienists’<strong>Association</strong> (ADHA) is in the midst of a great brandingcampaign. If you are not aware of “branding” it is thenew word for marketing. It is called “One Voice OneVision.” The campaign’s goal is to bring all hygieniststogether for education and information and to changeperceptions of the ADHA. The ADHA wants to be moreinclusive of all dental hygienists, no matter what paththeir career has taken them.Students are to be included to make them feel thatthey are an integral part of ADHA. For some reason,some hygienists think that the ADHA is anti-dentist,which is absolutely not true. We embrace the dentalcommunity and love being able to be a part of theirevolving profession. The president of the ADHA,Marge Green stated, “ADHA and its members will workin partnership with dentists to advance the oral healthof our patients.”This February, during the Chicago Midwinter <strong>Dental</strong>Meeting, another meeting was held with the ADA president,president-elect and executive director at the ADHAheadquarters. The purpose was to open dialogue andto share the results of the branding research. This wasthe first time in 20 years that the ADA officers had beeninvited to the ADHA headquarters for dialogue. There isa true commitment to keeping communications openon both ends.In Maryland in February of 2007, 12-year-old DeamonteDriver died as a result of an abscessed tooth. It couldhave been solved with an extraction but turned intomajor brain surgery. The infection spread from the toothto the brain and after the brain surgery was completed,physicians still could not save the child. This is just oneexample of how the lack of access to oral health servicescan have deadly consequences.<strong>Dental</strong> caries (not asthma) remains the most commonchronic disease of childhood. There is a silent epidemicof oral disease. The dental hygienist, as the preventionspecialist, understands the connection between oral healthand total health. The hygienist can prevent disease and treatproblems while still manageable. As I have written in pastarticles, the ADHA is working to establish the Advanced<strong>Dental</strong> Hygiene Practitioner (ADHP). This hygienist coulddeliver diagnostic, preventive, therapeutic and minimallyinvasive services to underserved Americans.Green has spoken to many groups and stated, “It wasmy pleasure to speak before the U.S. House ofRepresentative Appropriations Subcommittee on Labor,Health and Human Services, Education and RelatedAgencies in the necessity of improving access to oralhealth care in the United States.” It has been illustratedthat the current oral health care delivery system is simplynot meeting America’s oral health care needs. Whilemeeting with all these agencies, ADHA’s response to theSurgeon General’s challenge that a solution be found toenhance the workforce capacity and flexibility, was tourge the Subcommittee to provide funding to HRSA fora demonstration project which would test the feasibilityof using an ADHP.The ADHA is also working on a state-based initiativeto encourage its members to visit their member of Congressin their home districts.It has been my pleasure to serve as president of LDHAfor the past two years. I have been able to work withdedicated and knowledgeable people, and I would liketo thank the <strong>LDA</strong> for inviting me to their board meetings,which have been very informative. You can feel the energyfrom the <strong>LDA</strong> Board of Directors. I feel that the <strong>LDA</strong> andLDHA have a very good relationship and I am confidentit shall remain that way for many years to come. I leaveour organization in the capable hands of our newly electedpresident, Ashley Stockwell-Nix.And, one last reminder for the ADHA Annual Session,which will be held at the Sheraton New Orleans, June20-27, 2007. tSummer 2007 35
