<strong>CHECKLIST</strong> <strong>OF</strong> <strong>DOCUMENTS</strong> <strong>FOR</strong> <strong>OCI</strong> <strong>CARD</strong> (GENERAL)<br />
Online <strong>OCI</strong> Regestration application form to be filled in English. Completed form to be submitted<br />
along with documents.<br />
Please fill the online application form completely, as per the instruction on the form<br />
(Part A & Part B).<br />
The requisite amount of <strong>OCI</strong>, ICWF Fee & <strong>VFS</strong> Service charges to be paid.<br />
Photographs - 4 Four recent color passport size(about 51 mm x 51mm) photograph, showing full<br />
frontal view of face against a white background, 01 photograph to be pasted on the application<br />
form & other 03 to be attached with the form.<br />
Existing foreign passport (valid for 180 days) in original, along with a photocopy.<br />
Renunciation/ Surrender cetificate of Old Indian Passport from Indian Embassy should be<br />
submitted in Original with a photocopy. Please get the certificate from Indian Embassy before<br />
applying at India Application Centre.<br />
OR<br />
Declaration(affidavit) statment in english declaring & affirming about the surender of<br />
passport to the Indian Embassy from concerned dutch authirity upon becoming foreign<br />
National.<br />
Indian Birth certificate of the applicant, showing nationality / old Indian passport copy require duly<br />
Apostilled or attested by CPV Division, Ministry of External Affairs, Patiala House, New Delhi. OR<br />
Nativity certificate issued by state govt. In India by an authority not below a District Collector/ District<br />
Magistrate about an applicant being Person of Indian Origin eligible for registration as <strong>OCI</strong> OR<br />
All other relevant documents and their photocopies that may be helpful in establishing Indian origin<br />
which may include, inter alia, duly Apostilled / legalized ,revenue records, domicile certificate/school<br />
or college leaving certificates which shows nationality of the applicant.<br />
Original Uitterksel issued by respective Gemeente in Netherlands for residence proof, date of Dutch<br />
nationality & showing previously nationality as Indian.<br />
NON Dutch national who are resident for more than 01 YEAR are eligible & can submit the below<br />
noted additional documents<br />

Telex form should be filled without fee.<br />
Original Uitterksel issued by the respective Gemeente in the country of origin showing date of<br />
becoming the national of respective country & previously nationality as Indian<br />
Original Uitterksel issued by respective Gemeente in Netherlands for residence proof.<br />
Above requirements are only indicative and attempt has been made to cover requirements for a major section<br />
of applicants. However, the Consular Officer may ask for additional documents and /or could call the<br />
applicant for an interview also after examining the application for issuance of <strong>OCI</strong> Card<br />
In special cases when the name of the applicant is changed. A certificate from respective authorities in<br />
Netherlands is needed.
