Social Work ServicesWelcome to Ambulatory Care! I am your social worker for floor 2 and floor 4. Feelfree to call with any questions!Your Specialty and Primary Clinic Social Worker/Case Manager has specializededucation, knowledge, and experience in health care, and will work closely with theclinic providers and nurses to help in coordinating the highest standard of care.Your social worker can provide you with assistance in the following forms:• Supportive counseling(related to new diagnoses, family stressors, mental healthissues, etc.)• Medication access• Insurance education/negotiation• Referral to various government programs and education about criteria forprograms• Transportation to medical appointments• Future planning in regards to medical and financial power of attorney• Dental services• Durable medical equipment and supplies(eyeglasses, walkers, etc.)• Alternative placement such as assisted living, adult home, or nursing homeYour social work case manager can also assist you in the brokerage role by referringyou to community agencies and providing handouts for these agencies for nonmedically related assistance. The following are some of the examples:• Food• Support groups• Vocational guidance• Housing• Emergency assistanceMany of the resources in the community are available in limited quantity and mayrequire multiple identifying forms to show that one meets eligibility criteria.Notes regarding social work referrals:On any home health or DME referral, diagnosis is needed. On home health,specifics regarding wound care, frequency, type of service(RN and/or PT) isneeded. On all orders name that is legible and dea # is needed. These canbe written on a Rx pad or order sheet and attached with referral.On oxygen orders, they want to have order for oximetry study if patient hasnot had one in past 30 days.

Supportive counseling can be offered, but per my supervisor, this can onlybe offered short-term(approximately 3 or 4 sessions) before a referral ismade out to other agencies.It is VERY IMPORTANT to attach the demographic sheet to each and everyreferral. Although this may not be current, it does give a starting point foraddress, insurance, emergency contact, dob, etc. If you have currentinformation, please write this on either referral or demographic page.Social work notes will be found under forms in Cerner under progress notesocialwork. These are normally entered within the week. Feel free to callif you do not see a note or something is not clear. There will also be a papernote in chart; those might be under progress note or correspondence section.Things that I can not do:Home visitsProvision for Rx copays and/or transportation homePrior authorizations on meds(due to clinical/medical inventory required)
