<strong>Hospice</strong> News<strong>Hospice</strong> NewsDay centre extended and refurbishedThe <strong>Hospice</strong> Day Centre was recently refurbished and expanded.Pictured are staff with patients in the day centre, which isnow much lighter and brighter. It’s amazing how much extraspace and light has been created, giving day patients a greatlyimproved environment. In feedback, 80% of patients rated thechanges as “excellent”. But each of our highly trained specialistnurses has about 40 patients to visit at anytime. They liaise with our <strong>hospice</strong> doctors,hospitals, the patient’s own GP and DistrictNurses to ensure that each patient receives theservices they need. But what they do not havethe resources to do is spend time overnightwith patients, at the patients home to helpsupport the patient and their relatives.So we now also have a <strong>Hospice</strong> @ Homeservice at <strong>St</strong> Raphael’s.The service extends <strong>hospice</strong> care into thecommunity to support dying patients in theirown homes, if that is their wish.WARD SISTER:Pauline MorrisThis is our Ward Sister PaulineMorris. She is responsible for theresidential in-patient wardof 14 bedrooms.During each year over 350patients come to the <strong>hospice</strong> asan in-patient. Her role primarilyfocuses on the day to day runningof the Ward, ensuring that patientsreceive a high standard of nursingcare. In conjunction with the <strong>hospice</strong>Practice Development Facilitator,Pauline will continue the ongoingdevelopment of contemporary palliativecare nursing practice.Pauline is herself a shiningexample of how the <strong>hospice</strong>helps to develop staff to theirfull potential. She returned to nursingin November 1999 after having fourchildren. She joined the <strong>hospice</strong> as aThe <strong>Hospice</strong>@Home staffemployed by <strong>St</strong> Raphael’s area team of specially trainedand motivated healthcareassistants, who will workclosely alongside existingcommunity services such asthe district nursing teams.The <strong>Hospice</strong>@Hometeam aims where possibleto provide care tailored tomeet the needs of individualpatients and their families.The team will be able toprovide important supportive care at home, aswell as helping with practical aspects such asassistance with personal care, making simplemeals and beverages, and enabling families andcare assistant and worked for a yearuntil she was able to do a ‘return tonursing course`. Following that, Paulineworked as a staff nurse on night dutyfor five years, following which sheworked days, and after several yearsexperience, became team leader, seniornurse and now junior ward sister.carers to have a rest.These improvements have been madepossible by extra support from the Suttonand Merton Primary care Trust (PCT).Over the years, there have beenmany changes in Palliative carebut the care and attention givento patients remains the same. The Services provided by <strong>St</strong> Raphael’s <strong>Hospice</strong>The <strong>Hospice</strong> gives palliative (end of life) care to nearly 900patients every year. This is all provided entirely free of chargeto patients and families, and largely paid for by charitablefundraising, donations and legacies.We offer six core servicesDay Care<strong>St</strong> Raphael’s patients who are living at home canbe referred to our Daycare centre which is openfour days a week. Volunteer transport and aMinibus is available for those without transport.It gives patients a chance to meet others withsimilar problems and to make new friends; toexperience complementary therapies, art andcrafts. Trained nurses are always present tohelp with any health issues and concerns. It is a‘day out’ for the patient and a day’s respite fromworry for family and friends.Bereavement andFamily SupportAs well as our bereavement service, which is inpro-active contact with over 300 relatives eachyear, there is a nurse counsellorwho advises families, a socialworker whose speciality iscounselling children who havebeen bereaved, and a PastoralCare Sister for spiritual support.There are no religiousrequirements for patientscoming into <strong>St</strong> Raphael’s; wehave patients of all faiths andnone. Our Pastoral Care Sisterhas a close working relationshipwith chaplains and priests fromall faiths, who regularly comeinto the <strong>hospice</strong>.•It costs a total of approx £4 milliona year to provide these services ona “24/7” basis. That’s nearly £11,000per day.•Of these costs the NHS throughSutton and Merton Primary CareTrust (PCT) provides a grant of£1 million or 25% of the total.•This leaves £3 million (75%) to beraised from charitable donations,fundraising and legacies.•The <strong>Hospice</strong> relies on your continuedsupport. Without donations, wecould not provide the servicesthat make <strong>Hospice</strong> care special.Care for 900 patientsa yearCare for 339 in-patients(3,566 days)3,693 home visits1,198 day centreattendancesBereavement and familysupport for over 300familiesCommunity PalliativeCare Team<strong>St</strong> Raphael’s <strong>Hospice</strong> has a team of highlytrained specialist nurses, who visit patientsat home. They provide symptom controland advice to enable patients to remaincomfortable and functional at home. Theteam liaise with other services that providepractical assistance such as social servicesand the district nurses, to ensure patientshave co-ordinated care. The CommunityPalliative Care nurse is the first point ofcontact for newly referred patients. Theywill maintain contact with each patientfor as long as necessary. Referrals tothe <strong>hospice</strong> come from GP’s, hospitalconsultants and hospital based clinicalnurse specialists.In-patient careThe ward has 14 bedrooms, and peopleare often surprised that it is not more. Butover the course of a year, those 14 roomswill be utilised by over 300 patients. Themedical, nursing and housekeeping staff,run the <strong>hospice</strong> on a “24/7” basis. We areopen 365 days a year 24 hours a day. Manypatients are admitted to the ward for endof life care, but many are also admitted toenable their condition to be assessed by thedoctors, and pain relieving drugs prescribedfor the patient to then be able to returnhome for a further period of time, havinghad their pain brought under control.Education servicesThe <strong>Hospice</strong> is an acknowledged centreof excellence for palliative care, and offersregular training to other health careprofessionals such as GP’s, District nursesand staff from local care homes. We alsoreceive regular visits from trainee doctors at<strong>St</strong> Georges Hospital Tooting and <strong>St</strong> Helier’sHospital Carshalton to see the work andenvironment of a <strong>hospice</strong>.<strong>Hospice</strong> at Home ServiceThe work of the Community Palliative CareTeam is augmented by a <strong>Hospice</strong>@Homeservice, offering patients care in their ownhomes, for the last four weeks of life.The <strong>St</strong> Raphael’s Family Support Team<strong>St</strong> Raphael’s <strong>Hospice</strong> welcomes your support. Caring for Patients and FamiliesWe treat a wide range of illnessesPatients are referred by GP’s, hospital consultants,or hospital clinical nurse specialists (CNS)Every year, <strong>St</strong> Raphael’s <strong>Hospice</strong>provides specialist care for some 900patients, suffering a wide range ofillnesses. While the vast majority of patientshave some form of advanced cancer,around 10 per cent are suffering fromother conditions. The <strong>hospice</strong> also providesadvice, support and care for patients withmotor neurone disease, multiple sclerosis,end-stage organ failure and HIV/AIDS.The <strong>hospice</strong> cares for patients from allbackgrounds and age groups [18 years andabove]. While most of the patients areolder, it is not unusual for <strong>St</strong> Raphael’s toprovide care for people in the 20s or 30s.To receive the care of the <strong>hospice</strong>, apatient is referred by a GP or hospitalconsultant or hospital CNS – and thereferral criteria are simple and few.According to Medical Director, Dr MarieJoseph, a <strong>hospice</strong> referral is suitablefor those patients with a progressivelyworsening, incurable illness with anestimated prognosis of several monthsrather than years.Once referred, the patients will bevisited initially by a clinical nurse specialistSome of the <strong>St</strong> Raphael’s Medical Team, led by Medical Director, Dr Marie Joseph.who will assess their needs and liaise withthe GP and district nurse to arrange thenecessary medical and nursing or practicalsupport.<strong>St</strong> Raphael’s <strong>Hospice</strong> takes amultidisciplinary approach to providingthe best treatment for those who comeunder its care. Dr Joseph is an experiencedconsultant in palliative medicine, and sheheads up a team of consultants, physicians,consultant psychiatrist, specialist withanaesthetic experience, specialist registrarsand senior house officers/medical trainees.A network of GPs and specialists to provideExpert Symptoms Control Is CrucialOne of the fundamental aims ofcare at <strong>St</strong> Raphael’s is to providecomfort and dignity to dyingpatients; and as such, pain control is amajor focus for the medical and nursingteams.Headed by Medical Director, Dr MarieJoseph, the medical team has a wealthof experience in pain and symptomcontrol. In particular, the team specialisesin managing complex pain, and has anumber of pharmacological agents andtherapies to help them in their work.Dr Joseph explains: “Pain is one of themost common reasons GPs refer patientsto us. We get patients with a variety ofcomplex symptoms, and more than a few‘Pain is oneof the mostcommonreasons GPsrefer patientsto us’on-call work has also been developed.Supporting this medical team are theexperienced palliative care nurses andclinical nurse specialists, Pastoral CareSister, health care assistants, a part-timephysiotherapist, a clinical pharmacist, anoccupational therapist, a nurse counsellor,social worker, discharge nurse coordinator,bereavement co-ordinator and wardclerk. The entire staff of over 100 arecomplemented by more than 180 <strong>hospice</strong>volunteers, who are overseen by thevolunteer services coordinator.of them come to us with neuropathic pain –which is the worst pain imaginable. In thesesituations, I invite the patient to a painmanagement clinic where we go through theaetiology of the disease and the pain anddetermine the best course of action.Specifically, we can prescribe neuropathicpain relieving agents such as Ketamine andMethadone. These medications are bestinitiated under continual supervision, whichwe can provide at <strong>St</strong> Raphael’s”.Such expert symptom control and painmanagement is crucial, because if these arenot managed, patients can experience severeemotional distress and begin to devaluetheir lives,” stresses Dr Joseph.The <strong>Hospice</strong> needs to raise over £3 million a yearKeith Witham, Director of FundraisingThe <strong>Hospice</strong> is open 24 hours a day, 365days a year. Our services cost over £4million a year – of which three-quarters– over £3 million annually depends on charityfundraising, donations and legacies. On thispage, I have summarised all the various waysin which we raise that money each year.We are grateful to everyone who supports<strong>St</strong> Raphael’s in so many different ways.Without your support, the charity simplywould not be able to do what it does.We particularly depend on gifts thatfriends and supporters leave in theirWills. If this is something that youare able to consider, it would be very muchappreciated. We have a scheme (details at theback of this magazine) whereby a number oflocal solicitors will create or update a Willat any time of year, and donate 10% of theirfee to the <strong>hospice</strong>. There is also a facility whereby if you want to support by having any winnings go back to the <strong>hospice</strong> as a donation,that can also be arranged. There are sponsored walks such as along the River Thames in London, aLadies Only “Midnight Walk” held at the <strong>Hospice</strong> in September, and a fun Santa Dash when everyone taking part is dressed as FatherChristmas! We have an annual “Light up a Life” Appeal in the autumn, up to Christmas in conjunction with a Carol Service held atthe <strong>Hospice</strong> usually on the first Sunday of December, when the <strong>Hospice</strong> Christmas Tree lights are turned on. There is a Summer Fete, aChristmas Bazaar – lots of events, all raising money for the <strong>Hospice</strong>.COMMUNITY FUNDRAISING: We rely a great deal on many local groups and voluntary organizations such as Rotary Clubs, Schools,Churches, Masonic Lodges, youth groups such as the brownies, pubs and clubs, and many more as well as individuals, who raise fundsand then donate the money to <strong>St</strong> Raphael’s. There are other community events, mainly sponsored activities, such as the LondonMarathon each year. We also have ten local volunteer supporter groups covering local areas of the <strong>hospice</strong>, whose role is to raisefunds and increase local awareness of the <strong>hospice</strong>. They are always looking for extra help! All these activities are co-ordinated by ourCommunity Fundraiser and her Team.IN MEMORY DONATIONS: We are fortunate that many supporters of the <strong>hospice</strong> give donations in memory of a loved one. On manyoccasions, the first financial support we receive is when a family member has died, the family ask not to have flowers at the funeralexcept for close family, but for donations to the <strong>Hospice</strong> instead. We have some specially made donations envelopes that are suitable forthis purpose, supplies are available on request from the Fundraising Office.TRIBUTE FUNDS: We have the facility of “Tribute Funds” which are named in memory of individuals. Donations are given to the <strong>hospice</strong>from any form of fundraising or personal donations, and are then recorded under the name of a loved one.TRUSTS AND FOUNDATIONS: A number of local grant giving charitable trusts support the <strong>hospice</strong> by giving donations either towards therunning costs of the <strong>hospice</strong>, or to special projects.LEGACIES: As mentioned earlier, this is a vitally important source of income and support for <strong>St</strong> Raphael’s. In any year, between 25%and 33% of the <strong>hospice</strong>s charitable income can be from gifts that friends & supporters leave in their will. There is information lateron in this <strong>brochure</strong> about how to leave a legacy gift, that will benefit <strong>St</strong> Raphael’s in the future. We have a “Lifelong Friends” scheme,whereby anyone who tells us that they have remembered the <strong>hospice</strong> in their will (not in detail as we recognize that is a very privatematter) but that the <strong>hospice</strong> has been included, can be recognized as a “Lifelong Friend” of <strong>St</strong> Raphael’s, as a token of our appreciation.CORPORATE AND EMPLOYEE FUNDRAISING: People raise money through their work, and sometimes companies also match what theiremployees have raised. There are sponsorship opportunities and an Honorary Patrons Group. Our Corporate Fundraiser co-ordinates these.APPEALS: We have appeals during the year, ranging from our Spring Sunflower Memories Appeal, through the summer when there areSunflower collections at local venues, and our annual Light up a Life Appeal. <strong>St</strong> Raphael’s <strong>Hospice</strong>HistoryHistoryThe history of the <strong>hospice</strong> started long before its opening in 1987.The <strong>Hospice</strong> was built by “TheDaughters of the Cross”, aCongregation founded in Liège,Belgium in 1833. The Sisters who caredfor the sufferers of Cholera and typhoidepidemics in Liège, were brought togetherby the Foundress of the Daughters of theCross, Blessed Mère Marie-Therese (formerlyJeanne Haze), who was beatified by thePope in 1991.In 1863 a group of the Sisters wasestablished in England and in 1869 the<strong>St</strong> Wilfrid’s Nursing Home in Chelsea wasopened, followed by the establishmentof <strong>St</strong> Philomena’s School at Carshalton in1893, the <strong>St</strong> Elizabeth’s Centre for epilepsyat Much Hadham, Herts in 1903, and <strong>St</strong>Anthony’s Hospital, North Cheam in 1904.For most of the last century, includingthe years that saw the formation anddevelopment of the National Health Service,<strong>St</strong> Anthony’s Hospital was the local generalhospital for the area of North Cheam, Suttonand the surrounding areas.But in the early 1970’s the NHS decidedthat no more NHS patients were to be sentto <strong>St</strong> Anthony’s. Reluctant to close, theSisters developed<strong>St</strong> Anthony’s intoa modern privatehospital, whichopened in 1975.Today, <strong>St</strong> Anthony’sis one of the mostsuccessful privatehospitals in the southeast,with more than200 consultants.Disappointed thatthey could no longercare for all the localcommunity the Sistersbuilt <strong>St</strong> Raphael’s toprovide palliative carefor the terminally ill.This they achievedwithout any recourse topublic money but theylaunched an appeal tocover the running costsSister Mary Perpetua helpingto start turn the first turf ofthe <strong>hospice</strong> in 1986.The building work starts – 1986.which were then estimatedto be £1million a year.It is this appeal that hasbeen continued since <strong>St</strong>Raphael’s was opened toits first patient in April1987.During the remainderof 1987 the <strong>hospice</strong> caredfor 128 patients. Today, over 25 years on,<strong>St</strong> Raphael’s supports over 900 patients andtheir families each year.The need for public support is just asgreat as when <strong>St</strong> Raphael’s opened. <strong>St</strong> Raphael’s Charity ShopsNew “Gift Aid”schemesuccessfullylaunchedWe have recently launched a newscheme which enables the <strong>hospice</strong>to re-claim “gift aid” on donatedgoods sold through our charity shops.The new scheme is in operation in all ourshops and in the past year raised an extra£72,000 for the <strong>hospice</strong> from gift aid.Already some 5,000 donors are signed up,It works by the donors ofdonated goods firstly signinga shops gift aid form and thendonating goods to us. Whenwe then sell those goods inour shops, which have beendonated by that individual,and therefore have createda money value, the <strong>hospice</strong>can reclaim gift aid from thegovernment on that value.More potentialAlthough 5,000 supportersare now giving donatedgoods to us under gift aid,that represents just 3 outof every 10 donations ofgoods made to the <strong>hospice</strong> shops.If more of the other 8 out of 10 donorsof goods can also sign up to gift aid, thevalue for <strong>St</strong> Raphael’s would be even more.Our target is to raise an extra £100,000 forthe <strong>hospice</strong>.Can you sign up for gift aid?The only qualifications for taking part inthe gift aid scheme is that you pay UKincome tax (or Capital Gains Tax) and thatyou pay at least the amount of tax whichthe charity will claim in gift aid on yourcharitable donation. For people who are inemployment or self employed, that is notusually going to be a problem!Some supporters do not think they payincome tax, perhaps because they do nothave a paid job, but if you have a privatepension, or have savings in a buildingsociety which earn interest you almostcertainly do pay income tax. It may not bevery much, but it could be enough to covera “gift aid” donation.If you have not yet done so, and youdonate goods to our shops, PLEASE givethem via gift aid. One year nearly 1/3rd of theentire income of the <strong>hospice</strong> – over £1.1 million – came through legacy gifts that people gave to <strong>St</strong> Raphael’s. Without this support, the<strong>hospice</strong> could not survive to be here providing much needed care services for terminally ill patients , year after year.These are just a few of the legacies and gifts we have received. Shewas born in Cheshire and moved toLondon before World War 2, duringwhich she served in the police,later marrying John Siegel, a winemerchant. They lived in Kingston untiltheir retirement when they moved toWimbledon. John died in 1985, andBetty lived until she was 99.A legacy of £1,000 was received from theestate of Margaret Hilbourne. We thankher son, Andrew, for the kind use of thisphoto of his mother.Mrs Gwendoline Prince left a Legacy of£28,740 to <strong>St</strong> Raphael’s <strong>Hospice</strong>. Originallyfrom Lower Morden, she remembered theoriginal <strong>St</strong> Anthony’s Hospital in the early1900’s.She married in 1938, and she and herhusband Jack who was an optician,thentravelled the world conducting medicalresearch of the human eye. Having livedin Australia, New Zealand, the USA andCanada, Gwen decided to come home whenshe retired to live in Sutton, where she wasa volunteer at the Royal Marsden Hospital.She died at the age of 98.Our grateful thanks to her niece, MarionBarclay, for the photo and informationabout Gwendoline’s lifeFrances Scott had a Charities Aid Foundation(CAF) account and after she died in 2008at the age of 95, her niece, Mrs JoannaNorth, kindly arranged for the balance ofthat account, £1,869.89 to be donated to <strong>St</strong>Raphael’s. Frances Scott was born in 1913 andtrained as a nurse , after which she joinedthe Overseas Nursing service, travelling toMalaya where she met her husband Douglasin 1940. They were both taken prisoner bythe Japanese, and saw each other in the samecamp, the men’s and women’s sections beingseparated by wire . On returning to the UKthey settled in Scotland.
