<strong>APPENDIX</strong> 1<strong>CONSULTANT</strong>1. Eligibility CriteriaIn determining the eligibility of an applicant for licensing with <strong>Dubai</strong> <strong>Health</strong> <strong>Authority</strong> (DHA) asConsultant, applicant must comply with the following eligibility requirements:1.1. Primary medical degree awarded after a course of study comprising at least 5,500 hours(or four years full time equivalent study) from an approved listed Medical Schools listedin one of the following sites:1.1.1. Medical Schools listed on the Avicenna Database (formerly the WHOdirectory of medical schools) http://avicenna.ku.dk/database/medicine/1.1.2. Medical Schools listed on the International Medical Education Directory(IMED) – FAIMER http://www.faimer.org/resources/imed.html1.2. Successful completion of internship program (one (1) year program).1.3. Postgraduate certification of the Recognized Specialty Certificates tier one (1) or tiertwo (2) see appendix 6.1.3.1. Tier 1 certificates: Applicants must have two (2) years experience postobtaining the certificate1.3.2. Tier 2 certificates: Applicants must have eight (8) years experience postobtaining the certificate1.4. Evidence of involvement in academic activities relevant to the speciality within past five(5) years which may includes:1.4.1. Evidence of involvement in medical education and training within the area ofspeciality during the past 3 years (including presentation at scientific events)1.4.2. Evidence of involvement in clinical audit (a minimum of two clinical audits(or equivalent) within the past 3 years)1.5. Evidence of involvement in research activities (a minimum of two publications in aprofessional medical journal relevant to the speciality within past 5 years)1.6. Valid practice license in country of graduation and/or country of last employment.1.7. <strong>APPENDIX</strong> 12. Documents Required2.1. Applicants for licensure as Consultant must provide the following:2.1.1. Evidence of successful completion of an approved medical education program.2.1.2. Valid practice license in country of graduation and/or country of employment/appointment.2.1.3. Evidence of all clinical experience, experience certificates should be signed byHuman Resources/Personal Department or Medical Directors.2.1.4. Recent Good Standing Certificate issued by the medical council/associationfrom the country of last employment (Certificates are only valid for six (6)months from date of issue)2.1.5. Log book (for surgical specialities only)2.1.6. Signed declaration and consent for release of information2.1.7. Applicant’s resume2.1.8. Passport copy2.1.9. One (1) colour passport photographPlease note that additional documents may be asked to justify the application for licensure andregistration.3. AssessmentAll applicants are required to attend and pass the DHA assessment in order to acquire the license topractice with the title of 'Consultant' in the Emirate of <strong>Dubai</strong>.4. Conditions for Issuing a DHA LicenseEvidence of malpractice insurance is required for all approved applications before issuing the license.5. Renewal of Licensure5.1.All physicians are required to renew their professional license annually.5.2.Criteria for the renewal of a professional license the applicant must demonstratecompliance to the Continuing Professional Development (CPD) requirement.5.2.1. 30 credit points of accredited Continuing Professional Development (CPD)every year.5.2.2. CPD points shall be accredited by one of the accrediting bodies recognized byDHA.5.2.3.
