<strong>KEMENTERIAN</strong> <strong>KESIHATAN</strong> <strong>MALAYSIA</strong><strong>CRITERIA</strong> <strong>FOR</strong> <strong>APPLYING</strong> CLASS H LICENSE (CERTIFICATION)1.1 ScopeClass H licence is issued to applicants to certify that radiation equipment andfacilities comply with performance and safety standards.1.2 Qualification and Experience1.2.1 Consultant Medical Physicist(i)(ii)(iii)A minimum of a master’s degree in medical physics, health physics orradiological physics or equivalent.Five years experience in the application of radiation in medicine, atleast 2 years of which should have been spent within the specialty forwhich the licence is applied.Be a Malaysian citizen or a permanent resident in Malaysia. Suchperson or persons must also be fully conversant with all relevantMalaysian regulations, legislation and codes of practice. Persons notachieving these criteria may not sign documents which report theresults of test and quality assurance measurements; non endorsedreports are not admissible for the purposes of certification. Shouldthere be only one full time physicist/consultant in an organisation thenpart time consultants of equivalent qualification may be authorised bythe relevant full time physicist/consultant to sign such documentsduring any period of absence. Absence of the full time consultantshould not exceed a period of three months in any calendar year;accreditation of the organisation will be voided by failure to retainsufficiently qualified in-post staff.1.2.2. Technical Support StaffTechnical support staff who will be assisting in performing thecertification activities, including carrying out testing and qualityassurance procedures, shall have(a)A minimum qualification of a diploma in Science, Engineering,Radiography/Radiotherapy or related subjects and havecompleted at least 6 months full-time training and an additionalone year of full time work experience in the conduct of radiationmeasurement activities under the supervision of the licensee.Version : 1.0 Date Issue : 03-01-2000 1


(b)(c)Have attained Atomic Energy Licensing Board RadiationProtection Officer status or Radiation Protection OfficerCertificate.Have demonstrated competency in practical quality assurancetesting activities.1.3 Administrative Organisation And Office Accommodation(i)(ii)Organisations seeking accreditation should have an appropriateoperational organisation structure and permanent premises. Suchpremises should include provisions for the handling and storage offacilities used for the purpose of certification, including suitable recordkeeping facilities. Sub-contracting agencies may be used to supportactivities but sub contracting facilities and experience may not beused as a substitute for those of the applicant.There shall also be a radiation protection programme which shallinclude at least the following(a)(b)(c)Organisation chartResponsibility and job descriptionRelevant operation limits there should be(iii)Procedures for record keeping and reporting mechanism(a) Record of radiation exposure for workers(b) Medical record of workers(c) Radiation survey reports(d) Records of irradiating apparatus and isotopes(e) Certification reports(iv)Training programmes for the workers (radiation).1.4 Measurement Activities(i)Dosimetric facilities and test equipment used in any assessment ofirradiating apparatus should carry a current certificate of calibrationtraceable to a calibration centre recognized by the Board. Newlyacquired dosimetric apparatus carrying a manufacturer’s calibrationmust be calibrated and recalibrated within twelve months of receipt ofthe equipment by a laboratory recognized by the Board.1.5 MaintenanceThere shall be appropriate maintenance records for all test equipment anddosimetric facilities.Version : 1.0 Date Issue : 03-01-2000 2
