DOI: 10.3310/hta16060Health Technology Assessment 2012; Vol. 16: No. 6163• Do ensure that ALL transparent plastic sheets <strong>for</strong> EACH patient [Colour& Fluorescein im<strong>age</strong>s <strong>for</strong> both eyes] are inserted into the envelope in thefollowing order:§ For EACH patient, the transparent plastic sheets <strong>for</strong> the Colourim<strong>age</strong>s should be placed at the top (Right Eye first), with Fluoresceinim<strong>age</strong>s underneath (Right Eye first).§Transparent plastic sheets must not be folded.• Do use toughened envelopes or bubble-wrap to protect the transparentplastic sheets when preparing <strong>for</strong> posting.§If large numbers of transparent plastic sheets are to be sent at onetime, the use of a small box may be advised (following the samepostal safeguards).• Do use the full address of CARF (as shown below). The DP nameshould be marked clearly on the back of the envelope(s).• Do notify CARF of parcel dispatch.Using transmittal logs, CARF will confirm receipt of the transparent plastic sheets,and will also confirm that im<strong>age</strong>s have been successfully scanned & converted todigital <strong>for</strong>mat, and are suitable <strong>for</strong> grading.Any problems will be relayed back to the DP <strong>for</strong> amendment, and the submissionprocess repeated until ALL problems have been resolved.----------------------------------------------------------------------------------------------------------------Notes:If digital im<strong>age</strong>s from a DP need to be retrieved, it will be the responsibility of theoriginating Treating Centre to ensure that adequate photographic trackingin<strong>for</strong>mation has been recorded.It is the responsibility of EACH DP Clinician to ensure that photographers aretrained to a standard that will furnish im<strong>age</strong>s of the standard required <strong>for</strong> im<strong>age</strong>grading.If a Clinician has any concerns about photographer competency, additionalphotographic training may be available from CARF (<strong>for</strong> a fee).If it is found that photographs from a DP consistently do not meet the standardsrequired <strong>for</strong> grading, the DP will be contacted.Post<strong>age</strong> costs to CARF will be borne by the originating DP.CD-R’s will be stored at CARF.CARF will return transparent plastic sheets to the originating DP (post<strong>age</strong> costswill be borne by CARF).© Queen’s Printer and Controller of HMSO 2012.
