162 Appendix 1----------------------------------------------------------------------------------------------------------------Analogue Angiogram Systems:--------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------Details (i) to Film follow Submission:----------------------------------------------------------------------------------------------------------------• Film received by CARF will be scanned <strong>for</strong> digital conversion, and postedback to the originating DP.• Each DP should keep an ongoing record of the following basic details:- The hospital number of patient’s captured using film <strong>for</strong>mat.- Transparent plastic sheet identification label details <strong>for</strong> EACH patient[<strong>for</strong> BOTH Colour & Fluorescein im<strong>age</strong>s in BOTH eyes] (recorded asper photographic protocol: Appendix 7, section 5) [It may be possible toprint ID labels from the DMC preparatory CD].- Identity of person who checked, and verified, the contents of thetransparent plastic sheet.- Date of Post<strong>age</strong> to CARF.The Do’s <strong>for</strong> Successful Film Submission:• Do ensure that slides have been sorted into their stereo pair sets andthat each is correctly labelled and positioned inside the transparentplastic sheets, as per study photographic protocol.• Do ensure that each transparent plastic sheet is appropriately labelled.• Do send the transparent plastic sheets (& appropriate documentation) toCARF within 48 hours of being processed & mounted, and as close tothe capture date as possible (preferably within one week of date ofcapture).• Do submit a hard copy list of the patient identification numberspack<strong>age</strong>d.§ Please keep one copy of this log in the DP.• Do insert transparent plastic sheets <strong>for</strong> post<strong>age</strong> into the envelope inchronological photographic order (most recent at the top).
