In addition, we acknowledge thatthe City of Monterey reserves the right to disconnect, order disconnection, or terminate normal response to the alarm device when,in the opinion of appropriate authority in the City of Monterey, continued operation of the alarm device would constitute adetriment to the public health , safety, and welfare.This permit shall be subject to the fees and conditions specified in Section 22-9.1 of the Monterey City Code, and those on thereverse side. CONDITIONS:Permits are issued pursant to the terms and conditions specified in Section 22-9.1 of theMonterey City Code, and on this application form. Failure to abide by those terms andconditions is ground for revocation of the permit. All alarms installed and/or operatedunder a permit issued by the City of Monterey must be maintained in good order, andoperated only for the intended purpose. All persons having access to the alarm systemmust be adequately trained to prevent false activations.By signing this application, the subscriber acknowledges his/her agreement to abide bythe conditions, provide adequate training, pay any fee applicable, subject to appealprovisions, maintain the alarm in accordance with industry standards and to surrender anypermit issued upon request of appropriate authority whenever continued operation isfound to constitute a detriment under Subsection (e) of Section 22-9.1 of the MontereyCity Code, subject to appeal provisions.ACTION:APPROVED:(DATE)DISAPPROVED(DATE)REASON FOR DISAPPROVALPERMIT TERMINATED(DATE)BY(SIGNATURE)REASON FOR TERMINATION
