Young people continue to identify transition fromcare as a major issue, and as such have identified and requested more practical life skills training as apotential solution.HistoryIn light of <strong>the</strong> findings of <strong>the</strong> 2008 CREATE Report Card, Transition from Care, CREATE managementcommissioned a review of <strong>the</strong> existing CREATE Your Future Program. At <strong>the</strong> 2009 Inaugural NationalYouth Advisory Council Summit, young people again identified Transition in <strong>the</strong> top four issues facedby young people in care, and recommended that more workshops and practical skill training wasrequired.To meet <strong>the</strong> needs of <strong>the</strong> target population In April 2009 CREATE Your Future was redeveloped in linewith what young people across <strong>the</strong> country were clearly telling us <strong>the</strong>y needed.The initial model has been redeveloped into an innovative new program with one-off funding from<strong>the</strong> Commonwealth Government (FAHCSIA). The program was developed into a national package,that takes a more holistic hands-on approach to working with disengaged and highly disadvantagedyoung people.
