Develop alternative ways of case planning. Improve worker training, including options and ideas of how to engage young people. Young people need to be given copies of their plan.• Lack of official ID Case workers to support young people to acquire an 18+ card. Drivers Learners Licence at 16 years as part of a young person’s transition planning. All young people to be given their birth certificate before they transition.• Lack of transport Minimum 10 driving lessons as part of a young person’s transition plan.
