Lake Sunapee VNAOn behalf <strong>of</strong> all staff and the trustees <strong>of</strong> Lake Sunapee Region VNAand Hospice (LSRVNA), thank you for continuing to provide us opportunitiesto provide home health and hospice services, long-term careand community health services to all <strong>Warner</strong> residents.Our Mission remains unchanged and centers on providing these servicesto support the dignity and independence <strong>of</strong> everyone in yourcommunity. As in previous years, the LSRVNA Board <strong>of</strong> Directors haspledged that, within its financial resources, the Agency will continueto provide <strong>Warner</strong> residents with all its services, except for long-termprivate duty care, regardless <strong>of</strong> insurance coverage or any patient’sability to pay for care.I am proud to report that, in <strong>2011</strong>, LSRVNA employees:‣ Provided over 1,080 hours <strong>of</strong> nursing, therapy and in-home longterm supportive care to 65 residents;‣ Provided approximately 845 in-home nursing, therapy and socialwork visits to these residents. 116 <strong>of</strong> these visits were providedwithout any remuneration to LSRVNA. 42 visits were providedunder the NH Medicaid program. The NH Medicaid program reimbursesLSRVNA at roughly 60% <strong>of</strong> the Agency’s visit cost;‣ Two residents received 68 visits through our hospice program and,thanks in part to our support, they were able to spend their lastdays at home. Their families and loved ones are provided bereavementsupport and counseling at no cost; and‣ 170 <strong>Warner</strong> residents utilized foot care, flu and blood pressureclinics as well as parent child, bereavement and other supportgroups provided by LSRVNA.LSRVNA is totally committed to fostering not only individual healthand well-being, but to fostering community support which enablespeople to help their friends and neighbors.
