ANSI/<strong>NIST</strong>-ITL 1-<strong>2011</strong> - UPDATE 2013 DRAFT VERSION1NM,2N,3NM,5OThis example demonstrates that the laboratory is accredited by ISO (indicated bythe number 1) to process Nuclear DNA (indicated by the letter N). This lab is alsoaccredited by ISO as a Mitochondrial DNA lab (indicated by the letter M). Thenext occurrence of a numeric indicates the next accreditation type (or the use of acomma separated variable), which in this example is a GLA accreditation with ascope of accreditation <strong>for</strong> Nuclear DNA only (indicated by the number 2 followedby the letter N). This laboratory is also accredited by AABB <strong>for</strong> Nuclear andMitochondrial DNA (indicated by the 3 and the letters N and M respectively).Finally, this example shows that the laboratory is accredited by ASCLDlaboratory in an 'Other' scope (indicated by the number 5 followed by the letterO). Specific ordering of the alpha character is not required.• The fourth in<strong>for</strong>mation item is the name of the organization / NOO thatoriginally processed the DNA data. (This may be different from the entry in Field18.004: Source agency / SRC . This is an optional in<strong>for</strong>mation item. It is enteredin Unicode characters and is unlimited in length.• The fifth in<strong>for</strong>mation item is the point of contact / POC who composed the DNArecord metadata. This is an optional in<strong>for</strong>mation item that could include thename, telephone number and e-mail address of the person responsible <strong>for</strong> thisrecord submission. This in<strong>for</strong>mation item may be up to 200 Unicode characters.• The sixth in<strong>for</strong>mation item is optional. It is the ISO-3166-1 code of the sendingcountry / CSC. This is the code of where the DNA was processed -- notnecessarily the nation of the agency entered in Field 18.004: Source agency /SRC . All three <strong>for</strong>mats specified in ISO-3166-1 are allowed (Alpha2, Alpha3and Numeric). A country code is either 2 or 3 characters long. CSC defaults toISO-3166-1 coding. If an alternate code is used, then the name and version of thatalternate list of codes (such as GENC) is entered in Field 1.018 Geographicname set / GNS, which applies to ALL country codes in all records within thetransaction.• The seventh in<strong>for</strong>mation item is optional. It is the international organizationname / ION of the submitting organization. This is completed if the DNA wasprocessed by an organization that is not affiliated with a country (such as a multinationalorganization). This optional in<strong>for</strong>mation item is the name/acronym o<strong>for</strong>ganizations, and may be up to 100 Unicode characters.8.18.4 Field 18.004: Source agency / SRCThis is a mandatory field.
