All the materials utilized inall the restorations are combined into a single code when materials arerepresented. The codes are specified by tooth.41 = The presence of restorations without surfacein<strong>for</strong>mation is combined to a single code <strong>for</strong> the tooth. All materialsutilized in all the restorations are combined to a single code <strong>for</strong> thetooth, when materials are represented.51 = Only the presence of restorations without surface ormaterial in<strong>for</strong>mation is included in the coding.99 = The level of detail contained in Field 12.010: Toothdata detail / TDD concerning restorations, materials and/or surfaces isunknown.8.12.9 Field 12.009:Dental history data detail / HDDThis optional field should be included when prior data is available. This fieldincludes a subfield with a repeating set of in<strong>for</strong>mation items. Each subfield has onemandatory in<strong>for</strong>mation item. An example is 8.1.3.9.1.5 <strong>for</strong> the National Provider IdentifierNumber of Dentist that treated the patient.* The second in<strong>for</strong>mation item is the dental history additionaldescriptive text / HADT. It is a Unicode free text in<strong>for</strong>mation item. It isused <strong>for</strong> those codes that require text, such as 8.1.1 Name of Practice – thefull name of the practice where the patient was treated. Other referencecodes, such as 8.1.3.8.4 Chart Available – used when chart in<strong>for</strong>mation isavailable from the practice where the patient was treated, would not haveany in<strong>for</strong>mation recorded in HADT.Note that if HARC is set to 8.1.3.10 (the ADA code <strong>for</strong> CHART), the chartis contained in Field 12.999: Dental chart data / DATA. If the chart isalready in electronic <strong>for</strong>mat, it should be converted into Base 64 prior tosending (This is to avoid the use of any ‘reserved’ characters in XML). If itis physical, the most common approach is to scan the chart and transmit thePDF or JPEG of the scan, also converted to Base 64.8.12.10 Field 12.010: Tooth data detail / TDDThis optional field has subfields, each with a set of in<strong>for</strong>mation items.
