Please contactEnvironmental <strong>Health</strong> (EH) at (916) 875-8440 <strong>to</strong>determine if a health permit is required.You must submit plans <strong>to</strong> Environmental <strong>Health</strong>, <strong>Plan</strong> Reviewsection, located at 10590 Armstrong Ave., Mather, CA 95655anytime you Construct or remodel a building in<strong>to</strong> a new foodestablishment, orRemodel an existing food establishment.A building permit is required prior <strong>to</strong> any construction orremodeling work being done.In order <strong>to</strong> obtain your building permit you must submit plans <strong>to</strong>the local Building Inspection Department (BID) after submitting <strong>to</strong>Environmental <strong>Health</strong>, <strong>Plan</strong> Review section. A plan review fee paid (see page 4)The following is a table of in<strong>for</strong>mation <strong>to</strong> be included inEnvironmental <strong>Health</strong> plan submittals. Building InspectionDivision (BID) may require additional in<strong>for</strong>mation.DescriptionName <strong>and</strong> address of establishment.Establishment location relative <strong>to</strong> otherbuildings, trash enclosure areas, <strong>and</strong>parking.All rooms labeled <strong>for</strong> usage <strong>and</strong> doorschedule in<strong>for</strong>mation (self-closures, type).<strong>Food</strong> Equipment <strong>Plan</strong> All equipment labeled <strong>and</strong> identified,include manufactures’ name <strong>and</strong> modelnumbers.Plumbing <strong>Plan</strong>Materials <strong>and</strong> FinishesLighting <strong>and</strong> VentilationHot <strong>and</strong> cold water <strong>and</strong> waste linediagrams. Water heater size, type <strong>and</strong>location.Floors, cove base, walls <strong>and</strong> ceilingsmaterials.Overhead lighting location <strong>and</strong> type. The fee schedule may also be located on theEnvironmental Management Department website at:www.emd.saccounty.net/EMDfees.htmlExpedited review is conditional, based on availability of EH staff.If available, plans may be expedited <strong>and</strong> reviewed within 2working days at an additional fee <strong>to</strong> defray the cost of overtime.EH staff is available <strong>to</strong> answer questions <strong>and</strong> provide helpfulin<strong>for</strong>mation <strong>and</strong> materials regarding the plan review process.<strong>How</strong>ever, if a need arises <strong>for</strong> additional assistance, aConsultation Review is available, by appointment, at an hourlyrate.Continued on next page
