announced that it will cut<br />

Medicare reimbursement rates<br />

for hospice, forcing many<br />

hospice providers across the<br />

country to either significantly<br />

scale back the care they provide<br />

to terminally ill patients or to<br />

shut their doors altogether.<br />

The final rule, published in<br />

The Federal Register, became<br />

effective October 1, 2008.<br />

Palliative Care Program Begins<br />

New from Independent Health<br />

and Caring Hearts Home Care<br />

A new program, created by Independent<br />

Health and Caring Hearts Home Care, is<br />

making a positive impact on individuals<br />

with serious or chronic illness. The Home<br />

Connections Palliative Care Program,<br />

an affiliate of The Center for <strong>Hospice</strong> &<br />

Palliative Care, provides services to help<br />

patients and their families with independence<br />

and comfort in the home.<br />

Patient services include:<br />

• Pain and symptom management to<br />

improve quality of life<br />

• Education about their disease<br />

• Living with chronic illness and<br />

developing goals of care<br />

• Strategies for coping with side effects<br />

of their disease<br />

• Assistance with Advance Care<br />

Planning and decision making<br />

Home Connections works in partnership with the patient’s physician and<br />

other health care team members to provide coordinated care across settings that<br />

is tailored to patient and family needs. The program also offers assistance with<br />

<strong>Hospice</strong> <strong>Buffalo</strong> Recognizes<br />

Veterans for Their Service<br />

In honor of Veterans Day, <strong>Hospice</strong> <strong>Buffalo</strong> teams distributed more than 120<br />

veterans pins and certificates to area veterans being cared for by <strong>Hospice</strong> <strong>Buffalo</strong>.<br />

“We wanted to honor our veterans for serving our country,” said Bill Finn,<br />

president and CEO of <strong>Hospice</strong> <strong>Buffalo</strong>. “This is our second year of recognizing<br />

our veterans with a special pin and award certificate. We have been working at<br />

the state and local level to help ensure that veterans and their families receive<br />

high quality services through the end of life.”<br />

Movie to Air on<br />

Television<br />

Graceful Passages – The <strong>Hospice</strong><br />

Journals, a feature documentary<br />

film produced with <strong>Hospice</strong> <strong>Buffalo</strong>,<br />

will air on Thinkbright Television,<br />

on Sunday, Nov. 23, 2008 at<br />

9:00 p.m. Thinkbright airs locally<br />

on Time Warner Channel 21 and<br />

on digital channel 17.3 for those<br />

that have digital television.<br />

Graceful Passages offers an inside<br />

look into the poignant world of<br />

<strong>Hospice</strong> care as seen through the<br />

eyes of caregivers, patients and staff.<br />

The feature film was created by Jon<br />

R. Hand of <strong>Buffalo</strong>, an independent<br />

filmmaker who spent nearly 2 years<br />

with <strong>Hospice</strong> patients and staff.<br />

The remarkable film follows several<br />

patients and families into hospitals,<br />

homes, assisted living centers and<br />

<strong>Hospice</strong> inpatient settings.<br />

Thinkbright is operated by public<br />

television station WNED in <strong>Buffalo</strong>,<br />

New York and focuses on educators,<br />

students, families and members of the<br />

community. The channel operates<br />

“24/7” and features an innovative<br />

prime-time line-up devoted to<br />

themes. For example, Sunday<br />

evenings on Thinkbright are devoted<br />

to Family & Education.<br />

Raising the Bar<br />

For the past 30 years, <strong>Hospice</strong> <strong>Buffalo</strong> has<br />

provided exceptional care and support to more<br />

than 40,000 patients and families. <strong>Hospice</strong><br />

<strong>Buffalo</strong> has touched our friends, neighbors and<br />

our own families when serious illness becomes<br />

life threatening, and cure is no longer realistic.<br />

<strong>Hospice</strong> became a movement to change the way<br />

people live with illness, and to bring comfort and<br />

understanding to those touched by loss. The pioneering spirit that propelled<br />

<strong>Hospice</strong> <strong>Buffalo</strong> as one of the first hospices in the United States now exists<br />

in a world class program seeking new and innovative approaches to care for<br />

persons with advanced disease.<br />

Today, <strong>Hospice</strong> <strong>Buffalo</strong> is one of the most unique programs in the country.<br />

Our passion remains strong and we are committed that no-one need live<br />

in pain, that all have the right to control their care, and that healthy and<br />

honest communication is a right, not an option. Patients have many choices<br />

today when considering the prospect of living with advanced disease. And<br />

often, hospice presents a remarkable option for living fully, comfortably,<br />

and at home. Indeed, a recent journal article highlighted the fact that<br />

patients enrolled in hospice care live 30 days longer than similar patients<br />

without hospice care. <strong>Hospice</strong> truly is about hope.<br />

In 2008, we opened the St. John Baptist/<strong>Hospice</strong> <strong>Buffalo</strong> House, the only<br />

hospice-faith based partnership of this type in the country. Our Caring<br />

Hearts Perinatal Program has received national attention as a critical support<br />

for mothers whose pregnancy will not result in a healthy child. <strong>Hospice</strong><br />

<strong>Buffalo</strong> employs 10 physicians with more hospice and palliative care certified<br />

physicians than any hospice in the North East. Our program has the largest<br />

free-standing inpatient unit in the North East, and has routinely surpassed<br />

state and national standards for quality care at end of life.<br />

<strong>Hospice</strong> <strong>Buffalo</strong> is raising the bar as we continue to set new standards<br />

of care for Western New York and the nation. We are actively partnering<br />

with the University at <strong>Buffalo</strong> and our hospital partners to integrate<br />

palliative care training and service into acute care. Our partnerships with<br />

insurance providers are forging new venues for palliative care at home and<br />

pre-hospice care for persons with chronic disease, avoiding hospitalization<br />

and focusing on quality and comfort as disease advances.<br />

We believe that hospice can care for persons during the last year of life,<br />

not the last week. We believe hospice embraces hope and comfort, creating<br />

meaning and hope in a difficult time. We believe hospice is the way we<br />

would want to live; especially when we know it can add tremendous quality<br />

at assisted living residences and nursing homes.<br />

QUALITY OF LIFE<br />

<strong>Hospice</strong> Music Therapy Program<br />

On any given day, each of <strong>Hospice</strong>’s five music therapists can be found<br />

doing what they do best – improving our patients’ quality of life.<br />

Our board-certified music therapists are members of <strong>Hospice</strong>’s<br />

interdisciplinary team and are involved in assessing the strengths and<br />

needs of each client, developing a treatment plan, and implementing<br />

customized music interventions. Music therapists use both instrumental<br />

and vocal music techniques.<br />

Within our partner skilled nursing facilities, Alanna Ardito, LCAT,<br />

MT-BC; Bethany Miller, MT-BC; and Theresa Hoffman, MT-BC facilitate<br />

Life Rhythms music-making sessions for patients with various dementias.<br />

Each patient, rhythm instrument in hand, contributes to the creation of<br />

an interactive ensemble through song and improvisation. A number of<br />

families have noted that this is often the only hour of the day during<br />

which their loved one is alert and engaged. “In this setting, music therapy<br />

provides much needed sensory and cognitive stimulation, which increases<br />

or maintains a patient’s level of physical, mental, and social/emotional<br />

functioning,” states Abigail Unger LCAT, MT-BC.<br />

Abigail can be found facilitating individual, family, and group sessions<br />

with <strong>Hospice</strong> patients who reside in assisted living facilities or group<br />

homes for individuals with developmental disabilities. “Therapeutic<br />

interventions are implemented that address psychosocial issues, pain and<br />

symptom management, anxiety and relaxation, well-being, and expression.<br />

Through active engagement in music therapy, our patients experience<br />

maximized mental stimulation, increased interpersonal connections and a<br />

sense of belonging, decreased distress, as well as supported activity functioning<br />

and overall community building,” explains Abigail.<br />

As part of our Essential Care for Children team, Julie Palmer, MT-BC<br />

works with our pediatric patients and families. Music, one of the many joys<br />

of childhood, is a much needed distraction for these sick children, helping<br />

to dispel anxiety and instill a sense of normalcy in an otherwise chaotic<br />

and stressful time. Abigail notes that, “Through their active participation<br />

in music therapy, <strong>Hospice</strong>’s pediatric patients are given the opportunity to<br />

express themselves through an accessible and appropriate medium.”<br />

<strong>Hospice</strong>’s music therapists also<br />

provide services to our Home Care<br />

patients. Whether sessions are held<br />

within facilities or homes, music<br />

therapists design individualized care<br />

to address specific patient needs<br />

within their overall care plan. As<br />

part of our team approach, music<br />

therapists play an essential role in<br />

<strong>Hospice</strong>’s mission to provide optimal<br />

care when it is needed the most.<br />

Music therapists are valuable<br />

members of <strong>Hospice</strong>’s interdisciplinary<br />

team approach to patient care.<br />

Last year our music therapists<br />

conducted approximately 1,500<br />

music therapy sessions. Unfortunately,<br />

music therapy is not covered by<br />

Medicare or third party insurance<br />

and must be covered through<br />

fundraising efforts.<br />

The music therapists have a<br />

long wish list of items they would<br />

like for the program. Needs range<br />

from instruments such as maracas,<br />

bells and hand drums, to equipment<br />

for transporting and storing<br />

instruments, sheet music, and a<br />

portable sound system. If you would<br />

WHAT HOSPICE HAS DONE FOR MY FAMILY<br />

My dad has a chronic lung and heart condition. We were in the hospital<br />

at least once a week for either seizures or shortness of breath. He no<br />

longer drives and it was taking its toll on me and my sister, and mostly<br />

my mom and dad. She didn't know what to do or where to turn.<br />

They were not getting any answers.<br />

I recently started a new position at UB Family Medicine and came in<br />

to tour the <strong>Hospice</strong> campus with my 3rd year students. I saw the<br />

brochure on the above disease and gave it to my mom in the hospital.<br />

She called the next day and hospice came out.<br />

It has only been 2 weeks and already the quality of both my dad’s and<br />

my mom’s life has changed dramatically due to hospice. They are kind,<br />

knowledgeable and are giving them answers and options and most of all<br />

helping them at a time when they need it most. The advertisements are<br />

right – it’s never too early too call. Now we know someone really cares<br />

about what happens to my dad and is trying to make whatever time he<br />

has left the best it can possibly be!! I cannot thank you enough for all<br />

you do. I now feel peace knowing that someone cares about my dad and<br />

will do everything they can to make sure he is comfortable. Thank you<br />

from the bottom of my heart!!!!!<br />

<strong>Hospice</strong> <strong>Buffalo</strong> Recognizes Veterans for their Service, Continued from page 1.<br />

“Many Americans are unaware that more than 50,000 veterans die<br />

each month; that’s roughly 28 percent of all deaths in the United States,”<br />

commented Don Schumacher, president and CEO of the National<br />

<strong>Hospice</strong> & Palliative Care Organization (NHPCO). “Ensuring that all<br />

veterans receive compassionate care of the highest quality at the end of<br />

their lives is a responsibility we owe those who have served their country.”<br />

During this period of historically high deaths of veterans, there continues<br />

to be variability in the care delivered at the end of life. Nearly 40 percent<br />

of enrolled veterans live in areas that are considered rural, where community<br />

hospices and VA palliative care programs are often not readily available.<br />

On any given night, more than 200,000 veterans are without shelter and<br />

lack basic healthcare. Ensuring that all veterans receive compassionate,<br />

quality care at the end-of-life is a priority of NHPCO and the Department<br />

of Veterans Affairs <strong>Hospice</strong> and Palliative Care Program.<br />

<strong>Hospice</strong> care is part of the basic medical benefits package for veterans<br />

enrolled in the Veterans Health Administration (VHA). <strong>Hospice</strong> services<br />

are also covered by Medicare, Medicaid, most managed care organizations,<br />

private insurance plans, and HMOs. However, as is true with a majority of<br />

the American public, most veterans don’t know that these options exist.<br />

“It’s vital that we work closely with the Department of Veterans Affairs<br />

(VA) Medical Center, veteran service groups and community organizations to<br />

raise awareness of the unique needs of veterans at the end of life and improve<br />

access to hospice and palliative care at all sites and in all settings,” said Finn.

With Fond Memories<br />

Former BMW Master Technician and <strong>Hospice</strong> <strong>Buffalo</strong> patient, David<br />

Williams of Kenmore, lived life to the fullest. Although he was diagnosed<br />

with stage IV colon cancer in April 2006, he was determined to live and<br />

was able to remain at home under <strong>Hospice</strong> <strong>Buffalo</strong>’s care for more than<br />

two years.<br />

David drove himself to the Boulevard Mall each week and enjoyed<br />

walking the mall for exercise. Last winter, he converted his bike to a<br />

street-legal pedal assisted vehicle. “It gets 150 miles a gallon,” Dave smiled<br />

as he told <strong>Hospice</strong> staff about his latest project. “It took me five to six<br />

hours to put it together. The first time I took it out, I rode it for three<br />

hours because I had fun riding it.”<br />

Dave accomplished his life’s objective when he was able to celebrate<br />

his son’s wedding on September 20, 2008. “It was his BIG goal to make<br />

it to my wedding and much beyond,” said newlywed Brian Williams.<br />

“Unfortunately, he passed away shortly after on October 9, but was able<br />

to reflect to many how much of a wonderful time he had at the wedding.”<br />

Dave died peacefully in the <strong>Hospice</strong> Inpatient Unit on Oct. 9, 2008. He<br />

is survived by his three children, Amanda, Brian, and Meghan.<br />

The Society’s annual letter-writing evening, the kickoff of the Annual<br />

Appeal, takes place at the <strong>Hospice</strong> Mitchell Campus in late October.<br />

“This event gives our enthusiastic volunteers the opportunity to come<br />

together, be updated on the latest happenings in our organization, and<br />

write notes to more than 2,000 past donors. The continuing financial<br />

support of our mission is critically important to those patients and families<br />

in our care,” added Doug. Beth continued, “Last year, was a very successful<br />

and fortunate year for the Signature Society. The annual event raised<br />

over $275,000, the most in its 14 year history. Since the Signature<br />

Society’s inception in 1994, more than $2,611,000 has been raised by<br />

this dedicated group of volunteers.”<br />

Currently, more than 600 patients a day depend on the various<br />

programs and services of The Center for <strong>Hospice</strong> & Palliative Care.<br />

<strong>Hospice</strong> <strong>Buffalo</strong>, Life Transitions Center, the <strong>Hospice</strong> <strong>Buffalo</strong> Houses<br />

at the Mitchell Campus and at St. John Baptist, and Essential Care for<br />

Children all benefit from the unrestricted funds raised during the annual<br />

appeal. Doug & Beth summarized their feelings by saying, “We want to<br />

ensure the positive impact that <strong>Hospice</strong> has made in our community for<br />

another 30 years.”<br />

More than 600 individuals and families<br />

gathered for this year’s Walk to Remember.<br />

We couldn’t have asked for a more<br />

beautiful day to memorialize our babies<br />

lost to miscarriage, ectopic pregnancy,<br />

stillbirth, or early infant death. Some<br />

would say our sunny warm weather<br />

was a result of “Mother Nature” but we<br />

know better: it was all of our angels<br />

making sure the day was perfect. Over<br />

600 family members and friends walked<br />

to pay tribute to our babies.<br />

The following is a letter we received from a parent of one of<br />

Caring Hearts Perinatal patients. The Caring Hearts Perinatal<br />

Program is offered to expectant parents whose baby is diagnosed<br />

with a life-threatening condition before birth. The unique service is a<br />

program of The Center for <strong>Hospice</strong> & Palliative Care and is supported<br />

through a grant from The Children’s Guild.<br />

Grief can be a heavy burden that leadens every movement, every thought and<br />

every decision. However, grief is also a lens. It clarifies what is most important in<br />

life, magnifies the beauty of someone now lost and sharpens our sense of what has<br />

been found.<br />

When people ask about my daughter my responses most often confuse them.<br />

They are horrified by the story but perplexed by how calmly I tell the details, how I<br />

could cherish such moments. On May 18th of this year at 11:07 pm my first child<br />

was born, a daughter named Isabel. I held her, kissed her, dressed her and told her<br />

I loved her. At 1:05 am she died in the cradle of my arms. I was 34 weeks into my<br />

pregnancy but I knew that my baby was going to die at my 17-week appointment<br />

when she was diagnosed with a congenital kidney condition that was “incompatible<br />

with life.”<br />

Some said that I wasn’t given a choice. However, I did make a choice. I made it<br />

over and over for fifteen weeks. There was never a doubt that I would carry Isabel<br />

for as long as I could. She was safe within me; life outside was dangerous. She was<br />

still our dream come true for we had struggled to conceive her. The choice I could<br />

make was how I was going to carry her. Supported by the counselors of the Caring<br />

Hearts Perinatal <strong>Hospice</strong> program, we made the choice to carry her without dread<br />

in our hearts. We chose to open ourselves to loving her completely and cherishing<br />

every squirm, not knowing which one would be her last. We chose to live as fully as<br />

we could and leave most of the hard work of grief for later. There was a kernel of<br />

hope that the doctors were wrong but we prepared ourselves for losing her at the<br />

same time.<br />

We created a birth plan detailing our wishes for the day of her delivery and the<br />

hospital was notified in advance. Plans were made with the funeral director.<br />

Decisions were made regarding further testing. At the same time, we continued to<br />

work, meet with friends, pay the bills, grocery shop….all the ordinary tasks of life<br />

during an extraordinary time. We were keenly aware that this was the only time we<br />

would have with our daughter. We counted each moment, not as winding down to<br />

a terrible ending, but counting each moment as a memory to hold onto forever.<br />

The night of her birth was the most joyful moment of our lives for we finally had<br />

a chance to hold our daughter and share her with our families. Two hours seemed<br />

an eternity, a luxury of time we did not dare hope for. The curtain of grief was<br />

drawn when we were told she had passed. At the same time we clearly saw how<br />

much we had lost and how much we had gained in the past few months.<br />

To talk about Isabel is to tell a story of seeming paradoxes. Our experience<br />

strengthened and humbled us. Our lives were shattered and yet made whole. There<br />

is a peaceful center in the eye of grief. It’s only by going through the most turbulent<br />

part of the storm that one can find that place though.<br />

On October 12th I will join others in A Walk to Remember in honor of<br />

the 850,000 babies who die each year from miscarriage, stillbirth or early infant<br />

death. We walk in grief but we are not broken by it. We walk, transformed by<br />

our losses, but all is not lost.<br />

The 14th Annual ARTVOICE Mardi Gras, February 24, 2009<br />

The ARTVOICE Mardi Gras turns <strong>Buffalo</strong>’s Allentown, Elmwood and<br />

Chippewa district into its own French Quarter. The celebration includes<br />

more than 45 clubs and bars with various bands playing at the different<br />

venues. All proceeds will benefit the <strong>Hospice</strong> Foundation. The event<br />

kicks off at 5:00 p.m. with a motorcade parade traveling down Elmwood, into<br />

<strong>Hospice</strong> care is the most cost<br />

effective approach to care and<br />

actually saves Medicare and taxpayers’<br />

money compared to<br />

traditional care. Why would the<br />

White House and CMS want to<br />

cut one of the most cost effective<br />

programs, and a program with<br />

some of the highest quality outcomes<br />

under Medicare?”<br />

According to an independent<br />

2007 Duke University study,<br />

hospice saves Medicare an average<br />

of $2,300 per patient, amounting<br />

to a total savings of about $2<br />

billion a year.<br />

Considered to be the model<br />

for high-quality care for terminally<br />

ill patients, hospice focuses on<br />

caring, not curing. And, in most<br />

cases, is provided in the patient’s<br />

home. <strong>Hospice</strong> professionals are<br />

experts in providing pain and<br />

symptom management to the<br />

dying. Additional services include<br />

emotional and spiritual support<br />

to patients and their family caregivers<br />

as well as caregiver training.<br />

More than 1.3 million dying<br />

Americans received care from the<br />

nation’s hospice providers last year,<br />

a number that continues to rise.<br />

A Gallup poll found that nine<br />

out of 10 Americans, if faced with<br />

a terminal illness, would want to<br />

remain in their homes and receive<br />

the services that hospice provides.<br />

In fact, more than 80 percent of<br />

hospice care in the U.S. is provided<br />

in the home. And patient satisfaction<br />

with hospice approaches 100<br />

percent as shown by data collected<br />

from family caregivers.<br />

Camp Blue Skies Provides Fun<br />

While Helping Children Cope<br />

“From the moment your child arrives at Camp Blue Skies they are made<br />

to feel welcome,” said Jim Grande, LCSW, coordinator of Children’s<br />

Programs at Life Transitions Center, Inc. “They meet dedicated staff and<br />

other children who know exactly what it is like to experience the death<br />

of a loved one.”<br />

Camp Blue Skies is an annual camp for children between the ages<br />

of 7 and 16 which is run under the auspices of <strong>Hospice</strong> <strong>Buffalo</strong>. It is held<br />

each year at picturesque Cradle Beach Camp in Angola, New York. Camp<br />

Blue Skies is staffed by a team of professional grief counselors and trained<br />

volunteers. The children’s caregivers get a restful overnight break during<br />

which they can relax; knowing their child is having fun and is being well<br />

taken care of. If you are interested in Cradle Beach for your child or would<br />

Many contributors help <strong>Hospice</strong> provide its excellent and essential services to patients and their families in our community<br />

Mission Statement:<br />

The Center for <strong>Hospice</strong> & Palliative Care serves individuals<br />

with serious illness, and their families and caregivers through<br />

a continuum of care spanning the course of illness and<br />

bereavement. The Center’s specially-trained professionals,<br />

support staff and volunteers assist individuals to make choices<br />

about their care, support those who are grieving and educate<br />

the professional and lay communities about end-of-life issues.<br />

How To Access Our Services<br />

Affiliates of The Center for <strong>Hospice</strong> & Palliative Care are governed<br />

by the State of New York and most services are covered by a majority<br />

of insurance companies, including Medicare/Medicaid.<br />

Referring to the The Center for <strong>Hospice</strong> & Palliative Care is<br />

simple and convenient. Just call (716) 686-8000. An intake specialist<br />

will work with the patient, the family, and attending physician<br />

to initiate services and establish an individual plan of care.<br />

The Center for <strong>Hospice</strong> & Palliative Care consists of the<br />

on behalf of The Center for <strong>Hospice</strong> & Palliative<br />
