<strong>Child</strong> <strong>Health</strong> <strong>Check</strong>-<strong>Up</strong> Coverage and Limitations HandbookImmunizationsDescriptionProviders must assess and document in the child’s medical record the child’simmunization status, as appropriate for age and health history.RequiredIf it is determined at the time of the <strong>Child</strong> <strong>Health</strong> <strong>Check</strong>-<strong>Up</strong> that animmunization(s) is needed and appropriate (for age and health history) to provideduring the check-up, then immunization(s) must be provided at that time.RecommendedIf the child’s immunizations are not up to date according to age and healthhistory, providers should document why immunizations were not given at thetime of the <strong>Child</strong> <strong>Health</strong> <strong>Check</strong>-<strong>Up</strong>.Note: See Chapter 3 for information on reimbursement for immunizations.ImmunizationSchedule<strong>Child</strong> <strong>Health</strong> <strong>Check</strong>-<strong>Up</strong> providers should use the most current Recommended<strong>Child</strong>hood Immunization Schedule developed and endorsed by the AdvisoryCommittee on Immunization Practices, the Committees on Infectious Diseasesof the American Academy of Pediatrics, and Infectious Diseases of theAmerican Academy of Family Physicians.Note: The most current schedule is available on the Internet atwww.cdc.gov/nip. (Click on publications.) Providers may also obtain the currentschedule from their County <strong>Health</strong> Departments. The CDC’s NationalImmunization Information Hotline is 1-800-232-2522 and in Spanish is 1-800-232-0233.RequiredFrequencyThe provider must assess and document the child’s immunization status ateach <strong>Child</strong> <strong>Health</strong> <strong>Check</strong>-<strong>Up</strong>.October 2003 2-17
