<strong>Child</strong> <strong>Health</strong> <strong>Check</strong>-<strong>Up</strong> Coverage and Limitations HandbookDental ScreeningDescriptionThe dental screening component of the comprehensive physical examinationshould consist of an examination to check for obvious abnormalities, such ascavities, inflammation, infection or malocclusion. Dental referrals are requiredbeginning at 3 years of age; earlier as medically indicated.RequiredThe provider must assess the child’s dental status and document the findings(under comprehensive physical examination) in the child’s medical record.Required DentalReferral forRecipients ThreeYears Old andOlderThe provider must refer children who are 3 years old and older for anassessment by a dentist and document this referral in the child’s medicalrecord. The provider may refer a younger child if it is medically necessary.RecommendedFollowing the initial dental referral, subsequent examinations by a dentist arerecommended every 6 months, or more frequently as prescribed by a dentist orother authorized provider.If a dental provider is not available in an area, children must still be referred to adentist. Providers should notify the area Medicaid office that the child needs adental visit.RequiredFrequencyThe provider must assess and document the child’s dental status at each <strong>Child</strong><strong>Health</strong> <strong>Check</strong>-<strong>Up</strong>.Sensory ScreeningsVision ScreeningDescriptionVision must be assessed and documented as part of the comprehensivephysical examination with age appropriate testing to determine if the child’svision is within the normal range.Note: AAP website at www.aap.org for information on vision screening.RequiredEffective October16, 2003The provider must assess (including an ophthalmoscopic examination) thechild’s vision status, and document the findings in the child’s medical record.2-10 October 2003
