<strong>Child</strong> <strong>Health</strong> <strong>Check</strong>-<strong>Up</strong> Coverage and Limitations HandbookComprehensive Unclothed Physical ExaminationDescriptionThe physical examination consists of a comprehensive, unclothed examinationto determine whether the child’s physical development is within the normalrange for the child’s age and health history. An infant must be undressed, andan older child must be undressed and suitably draped.RequiredThe provider must perform a comprehensive unclothed physical examination anddocument the findings in the child’s medical record.RecommendedEffective October16, 2003Based on the provider’s medical discretion, the following elements asappropriate for the child’s age and health history should be addressed:• Appearance• Skin• Head• Eyes, ears, nose, mouth, throat, teeth, gums• Nodes• Blood pressure beginning at three years of age and as indicated• Heart and femoral pulses• Pulse and respiration• Lungs• Abdomen• External genitalia• Pelvic examination on all sexually active females, and if not sexually active,may wish to consider beginning at age 18*• Hip abduction• Gait• Extremities• Spine• Neurological evaluation* Note: Provider may wish to refer female recipients for this service.RequiredFrequencyThe provider must perform and document a comprehensive, unclothed physicalexamination at each <strong>Child</strong> <strong>Health</strong> <strong>Check</strong>-<strong>Up</strong>.October 2003 2-9
