<strong>Child</strong> <strong>Health</strong> <strong>Check</strong>-<strong>Up</strong> Coverage and Limitations HandbookComprehensive <strong>Health</strong> and Developmental HistoryDescriptionThe comprehensive health and developmental history consists of documentingthe child’s physical, behavioral and developmental health.RequiredThe provider must assess the child’s health and developmental history, whichincludes past medical history, developmental history, and behavioral healthstatus (including mental health development)*, and document the findings in thechild’s medical record.* Note: See the Bright Futures web site at www.brightfutures.org for BrightFutures in Practice: Mental <strong>Health</strong> and AAP web site at www.aap.org forinformation on mental health and substance abuse.Behavioral <strong>Health</strong>and DevelopmentalScreening FormProviders, based on medical discretion, may use the Behavioral <strong>Health</strong> andDevelopmental Screening Form to determine the child’s behavioral anddevelopmental health. If the child has a positive response to any of thequestions on the Behavioral <strong>Health</strong> and Developmental Screening Form, thechild should be referred for further evaluation.Note: See Appendix E for the updated Behavioral <strong>Health</strong> and DevelopmentalScreening Form. Providers may photocopy this form for their use.RequiredFrequencyThe provider must assess and document a comprehensive health anddevelopmental history of the child at each <strong>Child</strong> <strong>Health</strong> <strong>Check</strong>-<strong>Up</strong>. If the child’sstatus remains the same, the provider must document this.AdditionalInformation orRecordsThe provider should request medically relevant additional information or recordsfrom health care professionals or organizations that previously provided healthcare to the recipient.HIV TestingIf the HIV status of a newborn’s mother was not determined during pregnancy,the provider may choose to provide education to the parent(s) or caregiver aboutHIV testing, and recommend HIV testing for the newborn.2-6 October 2003
