Acceptance into the <strong>Co</strong>-<strong>op</strong> <strong>and</strong> <strong>Internship</strong> Programs (the “Program”) is determined by the CMC, based on completion of two (2)semesters in good academic st<strong>and</strong>ing. Transfer students must complete at least one (1) semester at IIT before engaging in their firstwork term. <strong>Co</strong>ntinuation in a Program is dependent upon maintaining good academic st<strong>and</strong>ing.2. <strong>Student</strong>s in a Program must maintain, at all times, their file in the CMC database including, without limitation, a current resume <strong>and</strong>contact information. IIT will only recognize the co-<strong>op</strong>/internship status of those students whose work experience is monitored by theCMC.3. <strong>Student</strong>s who interview, or obtain an interview(s), for a co-<strong>op</strong> or internship position through the CMC must follow theCMC’s policy <strong>and</strong> procedure for interview sign-up <strong>and</strong> must show up on time on the interview day. CMC’s “No Show” policywill be enforced when necessary.4. Once students accept a co-<strong>op</strong> or internship offer with a company, they must submit the necessary paperwork needed to approve theco-<strong>op</strong> or internship. The CMC will then process the paperwork <strong>and</strong> enter the permit which allows them to register for a co-<strong>op</strong> orinternship course. Once students have accepted an offer <strong>and</strong> submitted their paperwork to the CMC for processing, they willNOT be allowed to interview <strong>and</strong>/or accept an offer with another company. Such behavior or attempted behavior is groundsfor the students’ immediate dismissal from participation in the Program.5. While participating in the Program, students are required to inform the CMC of all work-term conditions, including, withoutlimitation, hiring, discharge, work schedules, transfers, <strong>and</strong> the like. Additionally, students must immediately communicate ALLwork place changes <strong>and</strong> problems to their CMC advisor, <strong>and</strong> in any event, students must communicate with their CMC advisor at leastonce a semester.6. For students participating in the <strong>Co</strong>-<strong>op</strong> Program. Before starting the first work term, the student must devel<strong>op</strong> a <strong>Co</strong><strong>op</strong>erativeEducation Plan with the assistance of the workplace supervisor, the student’s faculty advisor, <strong>and</strong> the CMC advisor. No changes maybe made to the plan without the consent of the work place supervisor, the faculty advisor, <strong>and</strong> the CMC advisor.7. For students participating in the <strong>Co</strong>-<strong>op</strong> Program. A minimum of two (2) full-time work terms for graduate students, or (3) threefull-time work terms for undergraduate students, with the same employer is required (assuming suitable performance, availability ofbudget/relevant work, <strong>and</strong> student not changing major). This minimum can be fulfilled with any combination of full- <strong>and</strong>/or part-timework terms (two (2) part-time terms are considered the equivalent of one (1) full-time work term) prior to the student’s completion ofhis/her degree program. As a <strong>Co</strong>-<strong>op</strong> is an academic program, start <strong>and</strong> end dates of work terms must follow academic semesters. YOUMAY NOT TURN IN CO-OP PAPERWORK AFTER THE PUBLISHED DEADLINE. DEADLINES FOR CO-OPREGISTRATION WILL BE STRICTLY ENFORCED.8. <strong>Student</strong>s must register for the appr<strong>op</strong>riate co-<strong>op</strong> or internship course with the IIT Registrar for each work term, includingsummer. <strong>Co</strong>-<strong>op</strong> students who fail to register with IIT during co-<strong>op</strong> work terms may be required to apply for readmission to IIT, <strong>and</strong>may lose the grace period for loan repayments. <strong>Internship</strong> students working during the academic year must register for classes inorder to meet the required academic enrollment status.9. <strong>Student</strong>s will be expected to participate in an evaluation process each term at the work site. The student <strong>and</strong> supervisor willdetermine objectives <strong>and</strong> learning goals at the beginning of the term <strong>and</strong> will evaluate progress toward those objectives at the end ofthe term, which will be submitted to the CMC in the Work Term Evaluation.10. <strong>Student</strong>s working a full-time co-<strong>op</strong>/internship may take up to six (6) hours of course work during a work term. <strong>Student</strong>s working apart-time co-<strong>op</strong>/internship during the academic year must take a minimum of 4.5 credit hours. <strong>Co</strong>ursework loads over the limit duringa work term must be approved by the Undergraduate or Graduate <strong>Co</strong>llege Dean.11. Failure to adhere to these policies will result in dismissal from the Program, including, employment at the co-<strong>op</strong> or internship worksite, <strong>and</strong>/or denied access to the services provided by the CMC.Pursuant to the Family Educational Rights <strong>and</strong> Privacy Act of 1974, as amended, I hereby authorize Illinois Institute ofTechnology’s <strong>Career</strong> Management Center to release information contained in my educational records to prospective employers forthe purpose of providing assistance with my search for employment.__________________________________ __________________________________ _______________<strong>Student</strong> Name (printed) Signature Date

I will maintain my information in the system, which will include an updated resume <strong>and</strong> contactinformation.2. I will present honestly <strong>and</strong> accurately all my abilities, activities, qualifications, interests, resumecredentials, academic work <strong>and</strong> records, including courses taken, grades, positions held, <strong>and</strong> dutiesperformed.3. If applicable, as a c<strong>and</strong>idate for a co-<strong>op</strong> or internship <strong>op</strong>portunity, I will review <strong>and</strong> sign the IIT-CMC<strong>Student</strong> <strong>Agreement</strong> <strong>Form</strong> in addition to the IIT-NACElink Registration <strong>Agreement</strong>.4. I will interview only with employers for whom I am interested in working <strong>and</strong> whose eligibilityrequirements I meet.5. I will keep all interview appointments scheduled, or obtained, through the CMC <strong>and</strong> follow the CMC’spolicies <strong>and</strong> procedures for on-campus interviewing, if applicable. If I need to cancel, I will cancel atleast 24 hours in advance of the interview. I underst<strong>and</strong> that missing a scheduled interview, or failingto cancel an interview within 24 business hours, will result in the loss of my NACElink privileges forsix (6) months <strong>and</strong> that I will be expected to send a formal apology to both the recruiter <strong>and</strong> the CMCDirector for not showing up for the interview.6. I will notify employers of my acceptance or rejection of offers by the time mutually agreed upon byboth parties.7. I will honor an accepted offer <strong>and</strong> discontinue interviewing upon providing verbal or writtenacceptance. I underst<strong>and</strong> that reneging is unprofessional <strong>and</strong> may be cause for suspension orrevoking of access to the NACElink system, as well as dismissal from the <strong>Co</strong>-<strong>op</strong> <strong>and</strong> <strong>Internship</strong>Programs, if applicable.8. If applicable, once I accept a co-<strong>op</strong> or internship offer with a company, I will submit the necessarypaperwork needed to approve the co-<strong>op</strong> or internship before the published deadline date.9.
